You, Tammy Woodel, owner, assisted me with the visit. You provided me with applicable program, staff, and children’s records for review. The most recent annual compliance visit was conducted on February 27, 2025. This facility currently operates with a Five star rated license effective September 13, 2024, meeting daytime care, 2nd shift, 3rd shift, enhanced ratios, enhanced space, enhanced ratios minus one child, and no children allowed on the second floor at any time for any reason; the office door must be closed. This facility has earned seven points in program standards, five points in staff education, and one quality point for all lead teachers and teachers having at least 5 years EC experience in no more than 2 different centers. The license was posted, and the restrictions were in compliance. This facility currently uses Creative Curriculum and Mother Goose Time. The program’s compliance history was 90 percent as of January 14, 2026. The NC Secretary of State website was reviewed on January 15, 2026, and Giggles Galore Learning Center, LLC was listed as current/active. The facilities contact information was reviewed and confirmed with you. A checklist was used to note the requirements monitored today. A walkthrough of the facility was completed today, and all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play activities indoors, lunch, and nap. The caregivers were interacting and meeting the developmental needs for each of the children. You stated there are no children enrolled on second and third shifts. The most recent sanitation inspection for your facility was conducted on August 12, 2025. A superior sanitation classification was issued with 0 demerits noted on the grade card. The analysis date for the most recent lead water test was April 19, 2024. This website also indicated no asbestos or lead-based paint hazards were identified for this facility. The most recent approved fire inspection for your facility was conducted on May 13, 2025. Files for one new staff member, one existing staff member, and applicable records for all staff were reviewed. All staff were listed on the ABCMS Provider Portal roster for this facility. Three children’s records were reviewed. The most recent monthly fire drill was conducted on January 9, 2026, at 9:13am. The most recent quarterly lockdown/shelter-in-place drill was conducted on January 9, 2026, at 2:12pm. The facilities EPR Plan was updated on December 10, 2025. The most recent monthly playground inspection was completed on January 9, 2026, by Tammy Woodel. The violation observed today was discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violation was observed: Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. One incident report dated May 15, 2025, for a child enrolled on April 7, 2025, and one incident report dated August 19, 2025, for a child enrolled on September 3, 2024, did not include a description of the first aid given. .0802 (e) The violation observed was corrected during the visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance for the violation observed was provided on the following: Incident reports- - We discussed the information required by child care rules for incident reports. You had a conversation with all staff members present during the visit and reviewed incident report requirements with them, including instructions to include a description of the first aid given to the child on the incident report. Staff recorded the first aid that was given to the child on the incident report form during the visit. I suggested that you review all incident reports once received to ensure all required information is obtained. This will allow you to gather any missing information from staff to be recorded prior to filing. Child care rule .0802(e) states the child care provider shall complete an incident report each time a child is injured as a result of an incident occurring while the child is in care. This incident report shall include: (1) facility identifying information; (2) the child's name; (3) date and time of the incident; (4) witness to the incident; (5) time the parent is notified of the incident and by whom; (6) piece of equipment involved, if applicable; (7) cause of injury, if applicable; (8) type of injury, if applicable; (9) body part injured, if applicable; (10) where the child received medical treatment, if applicable; (11) description of how and where the incident occurred, and the First Aid received; and (12) steps taken to prevent reoccurrence. This report shall be signed by the person completing it and by the parent, a copy given to the parent or a parent signature declining a copy and the report maintained in the child's file. Consultation: -I suggested that staff begin the criminal background recheck process for two staff members, Zelda Ussery and Shenicka Finger, whose qualification letters expire in April 2026. The visit was conducted with you, Tammy Woodel, owner/Lead Teacher. Sharon Reeves, owner, was also present. You provided me with applicable program and staff records for review. This program currently operates with a Five star rated license effective September 13, 2024. The program’s compliance history was 96 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The NC Secretary of State website was reviewed on August 29, 2025, and Giggles Galore Learning Center, LLC was listed as current/active. Limited monitoring was conducted. A walk-through of licensed indoor and outdoor space was completed. Staff and children were observed during gross motor activities outdoors, free play activities indoors, lunch and nap. One child enrolled in space #1 was visiting space #3 today. Space #1 was not in use and space #2 was used for storage. In space #3, one tub of Aveeno Baby Eczema therapy cream was observed in an unlocked cabinet below the diaper changing table secured with a child safety latch. 15A NCAC 18A .2820(d) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space #1, a tube of Aquaphor healing ointment expired on March 31, 2025 and the authorization from the parent expired on July 16, 2025. .0803(12) 871 Center staff did not comply with the safe sleep policy. In space #3, a visual safe sleep check was documented for one sleeping infant, seven months of age, at 11:24am on 8/20/2025 by L.H. and a safe sleep check was not documented again until 11:44am by L.H. 10A NCAC 09 .0606(a) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In space #3, a sleep position was not documented for one seven-month-old infant on 8/5/2025 at 1:10pm, 8/13/2025 at 11:49am and 11:59am, 8/14/2025 at 1:58pm, 8/26/2025 at 11:15am, and 9/3/2025 at 11:26am, 11:36am, and 11:46am. .0606(g) The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance statement by September 18, 2025. Failure to correct the violations and send the written statement by the due date may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Technical Assistance regarding the violations observed: Safe sleep- I suggested utilizing a timer to assist staff with remembering to visually check sleeping infants at least every fifteen minutes. Visually checking on sleeping infants at least every fifteen minutes, per your facilities safe sleep policy, is critical in protecting sleeping infants from sudden infant death syndrome. You have implemented checking sleeping infants every ten minutes rather than every fifteen minutes. I encouraged you to discuss requirements for safe sleep checks as well as sudden infant death syndrome with all staff, especially all staff who work in the classroom for infants. I suggested utilizing the Caring for Our Children resource book for more information pertaining to safe sleep practices and sudden infant death syndrome. Safe Sleep Policy- -Your facility’s safe sleep policy must be followed at all times. I suggested that you review the facility’s safe sleep policy with your infant teachers and all staff members to ensure the policy is followed at all times. Consider holding a staff meeting to review your safe sleep policy with all staff members. Additionally, I suggested you contact your local Resource and Referral Agency to request additional technical assistance regarding safe sleep. Storage of Medication- The tub of Aveeno Baby Eczema therapy cream was moved to a shelf above five feet from the floor during the visit. Medications including prescription and non-prescription items shall be stored in a locked cabinet or other locked container and shall not be stored above food. Remind staff of appropriate storage of medications and appropriate locations and locks to be used. Locked storage rooms and cabinets shall include those which are unlocked with a combination lock, electronic or magnetic device, keypad, key, or equivalent locking device. Keys and electronic or magnetic unlocking devices shall be kept out of the reach of a child and shall not be stored in the lock. Designated emergency medications shall be kept inaccessible to children, but are not required to be in locked storage. Non-prescription diaper creams and sunscreen shall be kept inaccessible to children when not in use, but are not required to be in locked storage. “Inaccessible to children” is defined as a vertical distance of at least five feet from the finished floor. I suggested that you and staff conduct safety checks of each classroom to ensure all medications are stored appropriately. Medication- -I suggested that you monitor all medications at least monthly to ensure the medication has not expired and the authorization from the parent is on file and has not expired. Any medication remaining after the course of treatment is completed, after authorization is withdrawn or after authorization has expired shall be returned to the child's parents. Any medication the parent fails to retrieve within 72 hours of completion of treatment, or withdrawal of authorization, shall be discarded. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The visit was conducted with you, Tammy Woodel, owner. You provided me with program, staff, and children’s records for review. The NC Secretary of State website was viewed before the visit and Giggles Galore Learning Center, LLC was current/active as of February 27, 2025. The facilities contact information was reviewed and confirmed with you. This program currently operates with a Five star rated license effective September 13, 2024. The program’s compliance history was 97 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements monitored today. No children were currently enrolled on second or third shift. Children and staff were observed during gross motor activities outdoors and lunch. The most recent sanitation inspection for your facility was conducted on October 14, 2024. A superior sanitation classification was issued with 4 demerits noted on the grade card. The most recent lead water test results were completed on April 19, 2024. This website also indicated no asbestos or lead-based paint hazards were identified. The most recent approved fire inspection for your facility was conducted on May 6, 2024. Violations observed today were discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violations were observed: Violation Number Comment Rule 1327 Accurate records were not maintained for all children. One child enrolled on 10/21/2024 was marked present on the attendance record for January 21, 2025, January 22, 2025, January 23, 2025, and January 24, 2025, however, there were no arrival and departure times documented for this child on these dates. There were also no safe sleep checks documented for this child on these dates. The Owner and Lead Teacher stated this child was not present on these days. G.S. 110-91(9) 1831 At least one child care provider, who has completed ITS-SIDS training was not present in the infant room, while children were in care. ITS-SIDS training for one substitute, employed on 3/4/2024, expired on 12/9/2024. The substitute relieved the Lead Teacher in space #3, the classroom for infants, to leave for an appointment. At that time, the substitute was the only caregiver in space #3 caring for one infant and one child one year of age. .01102 (f) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Five staff members did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training, one of the health and safety training topics, within five years of the previous training. One staff member employed on 6/5/2017 completed the training on 5/7/2019. One staff member employed on 6/5/2017 completed the training on 5/16/2019. One staff member employed on 10/3/2017 completed the training on 5/16/2019. One staff member employed on 2/18/2019 completed the training on 5/8/2019. Technical Assistance was provided on the following: Accurate Record Keeping- Accurate records must be maintained for all children. As a reminder, if you submit attendance records to other agencies for reimbursement, records must be accurate. I suggested that you review all attendance records and arrival and departure records for accuracy at the end of each week or more frequently. ITS-SIDS Training: I suggested that you review a staff members file prior to allowing that staff member to work in the classroom for infants to ensure the staff member has current ITS-SIDS training. You may utilize the File Checklist for Staff as well as the On-Going Training Documentation available on the DCDEE website under Provider Documents and Forms to assist you with keeping track of staff records, including ITS-SIDS training. In centers that are licensed to care for infants, the child care administrator and any child care provider scheduled to work in the infant room shall complete ITS-SIDS training. ITS-SIDS training shall be completed within two months of an individual assuming responsibilities in the infant room and every three years thereafter. Child care administrators, as defined in G.S. 110-86(2a), shall complete ITS-SIDS training within 90 days of employment and every three years thereafter. Completion of ITS-SIDS training shall be included once every three years in the number of hours needed to meet on-going training requirements in this Section. At all times, one child care provider who has completed ITS-SIDS training shall be present in the infant room while children are in care. A certificate of each staff member's completion of this course shall be maintained in the staff member's file in the center. On-going Health and Safety Training: I suggested that you refer back to the Health and Safety Training Record for each staff member to check when each training is due. Keeping track of completed training will assist you with ensuring all training required has been completed within five years. As a reminder, Recognizing and Responding to Suspicions of Child Maltreatment training is a part of the on-going health and safety training requirement. Child care administrators and staff members shall complete health and safety training within one year of employment, unless the staff member has completed the training within the year prior to beginning employment. Health and safety training shall be completed as part of on-going training so that every five years, all of the topic areas set forth in 10A NCAC 09 .1102(b) will have been covered. Consultation: -Emergency medications, including seizure medication, may be stored in a cabinet or on a shelf at a vertical distance of five feet or higher from the floor, and are not required to be locked. Permission to administer emergency medication authorization must be renewed after six months. -Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Upon arrival, the purpose of today’s visit was reviewed with Shenicka Finger, Lead Teacher. The visit was conducted with you, Tammy Woodel, owner/Lead Teacher. You needed to leave before the visit ended, therefore, Shenicka Finger, legal designee, signed the visit summary. You accompanied me on the walkthrough of the facility and provided me with applicable program and staff records for review during the visit. This program currently operates with a Four star rated license effective December 6, 2017. The program’s compliance history was 99 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The NC Secretary of State website was reviewed on September 6, 2024, and Giggles Galore Learning Center, LLC was listed as current/active. Limited monitoring was conducted. A walk-through of licensed indoor and outdoor space was completed. Staff and children were observed during free play activities indoors and gross motor activities outdoors. You stated no children are enrolled on second and third shift. Space #1 and space #2 were not in use. You divided outdoor space #1 into two separate spaces. I measured the two divided spaces during the visit. The larger outdoor space intended for preschool children will be referred to as space #1A and the smaller outdoor space intended for infants and toddlers will be referred to as space #1B. Space #1A allows for 21 children at 100 square feet per child. Space #1B allows for 12 children at 100 square feet per child. I provided you with an updated space calculation sheet. The updated playground diagram will be sent to you once completed. The most recent sanitation inspection for your facility was conducted on May 14, 2024. A superior sanitation classification was issued with 4 demerits noted on the grade card. The most recent lead water testing results were completed on April 19, 2024. I reviewed each indicator noted in the report and provided technical assistance. You had an opportunity to state corrections. The following violations of child care requirements were observed during today’s visit. The violation and technical assistance were thoroughly reviewed with you and documented in the Visit Summary given to you at the conclusion of the visit. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. Standing water was observed in a bucket full of toys on the playground for infants and toddlers, outdoor space #1B. Children were playing in the bucket of standing water before the staff member dumped the water. 15A NCAC 18A .2832(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Four staff members did not complete Recognizing and Responding to Suspicions of Child Maltreatment training at least every five years. The administrator and one staff member both employed on 6/5/2017 completed Recognizing and Responding to Suspicions of Child Maltreatment training on 5/7/2019. I suggested that you have one staff member monitor the outdoor premises at the beginning of each day before staff take children outdoors. Recognizing and Responding to Suspicions of Child Maltreatment training: Sharon Reeves, Tammy Woodel, Jessica Bingham, and Zelda Ussery must complete Recognizing and Responding to Suspicions of Child Maltreatment training by 9/23/2024. I reminded you that Recognizing and Responding to Suspicions of Child Maltreatment training is a part of health and safety training and must be completed every five years. I suggested that you utilize the Health and Safety Training Record to assist with keeping track of when on-going health and safety trainings are due. Rated License Reassessment: The Environment Rating Scale was ordered for your facility on April 18, 2024, during your preparation year, per your request. The Rated License Assessment Request Review Form was completed and signed by you on March 25, 2024. The Application for Assessment for a Two Component Star Rated License was also completed and signed by you. All indicators with a score below 4 points listed on the Environment Rating Scale Assessment report were reviewed in an effort to enhance quality. I reviewed these items with you and made suggestions to improve your score for the next Environment Rating Scale assessment. The ECERS-R assessment was completed on June 18, 2024, and you received a score of 5.63. The ITERS-R assessment was completed on July 2, 2024, and you received a score of 5.27. The overall average score was 5.45. We discussed program standards points during the visit and you accepted 7 points in program standards and will comply with highest voluntary enhanced staff to child ratio requirements. I reviewed these specific staff to child ratios with you. I asked if you had any objections to the score and/or items that scored low. You stated that you did not want to submit a grievance.
