You, Courtney Amakonze, Lead Teacher, assisted me with the visit. You provided me with applicable program and staff records for review. This facility currently operates with a Five Star rated license effective December 13, 2021, meeting daytime care only, enhanced space, and enhanced ratios. This facility has earned seven points in program standards, seven points in staff education, and one quality point for having a staff benefits package and infrastructure of parent involvement. The license was posted, and the restrictions were in compliance. This facility currently uses Creative Curriculum for approved curriculum requirements. The program’s compliance history was 98 percent prior to today’s visit. The most recent sanitation inspection for your facility was conducted on January 12, 2026. A superior sanitation classification was issued with 0 demerits noted on the grade card. The most recent fire inspection for your facility was conducted on October 13, 2025, and this facility was approved for daytime care only. Limited monitoring was conducted. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during gross motor activities outdoors, teacher directed group activities, handwashing routines, lunch, and nap. NC Pre-K requirements were also monitored. This facility is closed during the summer. The last day of school for children is May 21, 2026. One new staff member has been employed since your most recent annual compliance visit. Staff records for one new staff and applicable records for existing staff were reviewed. The analysis date for the most recent lead water test was June 10, 2024. The following violations were observed: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space #1, one electrical outlet not in use was uncovered and accessible to children on the wall near the teacher's desk and the CD player. I suggested that you write “NC Pre-K” on the safety plug as a reminder that the electrical outlet must remain covered at all times when not in use. During nap time, you were playing soft music through the Smart Board, however, a video was playing on the screen. During the visit, you searched YouTube for “sleepy music with black screen.” This allowed for soft music to play during nap time without showing a visual display on the screen. QRIS Pathways to the Stars: -I suggested that you review the new QRIS Pathway to Stars by visiting https://ncchildcare.ncdhhs.gov/Provider/Licensing/Star-Rated-License/QRIS-Modernization. -I suggested that you review Environment Rating Scale information and resources at www.ncrlap.org for ECERS-3. -Reach out to the Partnership for Children of Lincoln and Gaston County for additional technical assistance and support at 704-922-0900. -You stated you intend to follow Pathway #1 for your upcoming rated license reassessment. Due to employing a new Teacher Assistant in February 2026, we discussed waiting to complete the three-month self-study and ordering the ECERS-3 assessment at the beginning of the 2026-2027 school year. The facilities contact information was reviewed and confirmed. This program currently operates with a Five star rated license effective December 13, 2021. The program’s compliance history was 95 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Children and staff were observed during free play activities indoors and gross motor activities outdoors. NC Pre-K requirements were monitored during today’s visit. One child was not signed out on September 20, 2024. 10A NCAC 09 .0302(d)(4) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The Criminal Background Check qualifying letter effective September 11, 2019, for Kelly Withrow, principal, employed on July 1, 2023, expired on September 11, 2024. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. Kelly Withrow, principal, employed on July 1, 2023, did not have a valid Criminal Background Check qualifying letter on file for review. The DCDEE Criminal Background Check qualifying letter for Kelly Withrow expired on September 11, 2024. G.S. 110-90.2(b) & (d) & .2703(e) 9999 A violation was found for which there is no item number. Pests shall be excluded from the child care center. Traps set for pests shall only be placed in areas that are inaccessible to children. Seven active ant mounds were observed on the playground for preschool children, outdoor space #1. This is a violation of sanitation requirement 15A NCAC 18A .2831(b). The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance statement by October 9, 2024. Please send your compliance letter to kristen.mauney@dhhs.nc.gov. Failure to correct the violations and send the written statement by the due date may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your violation correction letter that corrections have been made when they have not, it will be considered falsification of information. Technical Assistance regarding the violations observed: Arrival and departure records- All children must be signed in and out each day including the time of the child’s arrival and departure. I suggested that you designate one staff member to monitor the arrival and departure records at designated times each day to ensure all children have been signed in and signed out. Active ant mounds- You covered each active ant mound with a plastic cone and alerted children to stay away from the cones. Most of the children played in the covered mulch area where there were no ant mounds present. I suggested that children utilize the sidewalk or paved areas until all active ant mounds are treated. I reminded you to monitor the indoor and/or outdoor premises once a day, prior to initial use, ensuring debris, including hazards such as active ant mounds, and broken equipment are removed and disposed of. I suggested that you have one staff member monitor the outdoor premises at the beginning of each day before taking children outdoors. Criminal background checks- Prior to the expiration date of the qualification letter, staff must complete and submit required forms to complete a criminal background re-check. A qualification letter is valid for a maximum of five years from the date of issuance and must be kept on file available for review. Dominique Vanderbush, lead teacher, emailed me on August 26, 2024, requesting assistance for the criminal background re-check process for Kelly Withrow. I provided guidance and technical support contact information in response to her email that same day. The ABCMS criminal background system did not show any progress made for a criminal background re-check for Kelly Withrow. I suggested that Kelly Withrow change her NCID password and then try to gain access to the system. Login to ABCMS is controlled by NCID; if Mrs. Withrow needs assistance with logging in or changing her password, please contact myncid.nc.gov or 919.754.6000. The following areas were monitored during the visit: supervision, staff/child ratio, CPR, first aid, special training, criminal background checks, Emergency Medical Care Plan, administering of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, and permit restrictions. The visit was conducted with Dominique Vanderbush, Lead Teacher. You provided me with applicable program and staff records for review. This program currently operates with a Five star rated license effective December 13, 2021. The program’s compliance history was 97 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A walk-through of licensed indoor and outdoor space was completed today. Staff and children were observed during teacher directed group time and gross motor play outdoors. The most recent sanitation inspection for your facility was conducted on August 30, 2024. A superior sanitation classification was issued with 0 demerits noted on the grade card. The most recent lead water testing results were completed on June 10, 2024. The most recent fire inspection for your facility was conducted on October 9, 2023. I suggested that you contact the local fire marshal and request a fire inspection for your facility to be conducted prior to October 9, 2024. You stated one new staff has been employed since your most recent annual compliance visit conducted on March 18, 2024. Applicable staff records were monitored. The following violations of child care requirements were observed. I reminded you to monitor the indoor and/or outdoor premises once a day, prior to initial use, ensuring debris, including hazards such as active ant mounds, and broken equipment are removed and disposed of. I suggested that you have one staff member monitor the outdoor premises at the beginning of each day before taking children outdoors. Criminal background checks- Prior to the expiration date of the qualification letter, staff must complete and submit required forms to complete a criminal background re-check. Upon arrival, the purpose of today’s visit was reviewed with you, Dominque Vanderbush, Lead Teacher. You provided me with applicable program, staff, and children’s records for review. This program currently operates with a Five star rated license effective December 13, 2021. The program’s compliance history was 100 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements monitored today. Children and staff were observed during nap, handwashing routines, snack, free play activities indoors, and departure. NC Pre-K requirements were monitored during today’s visit. The most recent sanitation inspection for your facility was conducted on December 12, 2023. A superior sanitation classification was issued with 4 demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on October 9, 2023. The violations observed were corrected during the visit. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One child enrolled on August 21, 2023, and one child enrolled on August 22, 2023, did not have a signed acknowledgement from the parent for the Summary of the Law until November 13, 2023. The acknowledgement form given to parents at the beginning of the school year did not include a statement of receipt for the NC Summary of the Law and the information contained in the NC Summary of the Law was not included in the parent handbook packet of information.
