This facility currently operates on a Temporary License effective February 24, 2025 to August 24, 2025 with restrictions of first and second shifts, maximum of 5 preschool children at any time, outdoor play is restricted to the fenced area only, children may not have access to chickens and other barnyard animals, and that the fireplace may not be used during operating hours. Today’s visit was conducted with Nuvia Bouadi, operator. During this visit, we walked through the entire lower floor and observed the indoor and outdoor environment. Six children were present today. They were observed finishing lunch, conducting personal care routines (toileting, and hand washing), transitioning into nap, and interacting with the operator. There were developmentally appropriate equipment, toys and materials available. The lunch served consisted of spaghetti, meatballs, cucumbers, carrot sticks, and milk. The facility was assessed for supervision, capacity, use of space, license restrictions, and health and safety requirements. Fire drills, medication administration, playground safety, and storage of hazardous products were also monitored. The license was posted along with the NC Summary of the Law. A first aid reference sheet was available. The daily schedule and activity plan were posted. The operator and children’s records were also monitored. The last fire drill was completed July 2, 2025 at 11:17am. The log reflected it took six minutes to evacuate your home. The most recent shelter in place drill was conducted on April 30, 2025 at 2:30pm. The most recent outdoor play area inspection was completed July 2, 2025. The following violations were documented during today’s visit: Violation Number Comment Rule 606 Each child was not held or placed in an appropriate feeding chair or other age appropriate seating apparatus to be fed. One child was observed in a pack-n-play with a bottle. A second child was observed laying on her mat with a bottle. The second child was also observed walking around the living room area while drinking from the bottle. .1706(k) 708 Indoor stairs with more than two steps were not made inaccessible to children two years of age or younger. The gate to the indoor steps leading to the second floor was not latched, and was hanging partially open. 10 A NCAC 09 .1719(a)(29) 726 Children were sleeping or napping in a room with a closed door between the child and the supervising staff. One child was observed napping in the child care room while the operator was in the living room and kitchen area. The door to the child care room was closed. .1711(a)(4)(D) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. The time of arrival was documented for three of the six children present today. .1721(e)(6) All violations must be corrected immediately. Please send me a compliance letter (written statement) describing how and when the above violations were corrected. The compliance letter must be received no later than August 6, 2025. The compliance letter should be sent to me via email, from the email address on file with the Division as the center’s official email address. If you are unable to correct all violations within the specified timeframe, send a compliance letter regarding violations that were corrected and a specific plan to correct the remaining violations. An extension will need to be requested from management. TECHNICAL ASSISTANCE & CONSULTATION: The following items were discussed during today' visit. You may want to consider them in order to avoid potential violations or improve the quality of your program. Remember that your own preschool age children count just as any other enrolled child. They must remain with the child care group, and follow all the child care requirements until their first day of kindergarten. We discussed that family members may not supervise the enrolled children, including your own children, unless they meet caregiver requirements. Written activity plans or lesson plans are required for family child care homes. Since ninety percent of a child's brain development happens before the age of five years old, meaningful and appropriate activities are critical for helping children develop cognitive, physical, social and emotional skills. Today I reminded you there are sample activity plan documents on the DCDEE website. Your activities should be aligned with the developmental domains according to the NC Foundations for Early Learning and Development. I encourage you to research training opportunities for both lesson planning and for NCFELD. The safe sleep policy must be posted in the area where the pack-n-plays are used. I observed you had it posted in the child care room, adjacent to where the pack-n-plays are used. I asked that you move it into the living room to maintain compliance. I also reminded you that this area cannot exceed 75 degrees and that you must document the safe sleep checks. You had questions today about placement of children under one year of age when they are napping. We discussed that they could not be placed on their belly to sleep, unless an acceptable waiver was available. Once the child can freely roll over and back again, they are able to assume whatever sleep position they like. However, you must place them in the sleep environment on their back, noting the documentation form if they roll over. I encouraged you to use the children’s file checklists on the Division’s website to ensure that all files are complete and accurate. I also encouraged you to review the forms to ensure they are completely filled in, with dates of enrollment, dates reviewed, and dates signed noted where applicable. Rated License: You provided me with a completed application for a Two through Five Star Rated License today. You stated that you had just completed the NC Early Childhood Education Credential coursework last week, and wanted to include that in this rated license assessment. We discussed that your temporary licnese cannot expire, and that I will need time to process the paperwork for the rated license assessment submission. Therefore, the packet will be processed on August 8, 2025. Only information available at that time will be considered. The components of the star rated license were reviewed: Staff Education: I observed today that you do not have a n account in the WORKS system. I asked that you create an account in the WORKS portal and submit your educational information to be evaluated. This is the only documentation that can be considered toward the rated license assessment. I encouraged you to submit the information as soon as possible. Only information observed in the database when your license is processed can be used. Therefore, delays in submitting your information or failure to address any returns may result in the assessment being completed without your education information. I will contact you to review the number of points to be earned in this component. Program Standards: Once the violations documented today have been corrected, the facility will meet minimum licensing standards. You stated today that you had participated in training sessions regarding the Environment Rating Scale assessment tools, however you had not conducted a self-study. Therefore, this facility will earn one point in Program Standards, once the corrections have been made. Quality Point: You stated today you wished to meet this point through the Education Option of completing 20 on-going training hours in addition to the annual on-going training requirement. We discussed that you just completed EDU 119, which earned 48 training hours. You also stated you were enrolled in additional EDU hours for the fall semester. This facility will earn the quality point. Once all components have been assessed, I will contact you to determine the points and stars to be earned. Once issued, the rated license will be mailed to you from the Raleigh office. Please review it for accuracy, and post it immediately. Contact me with any questions or concerns. We also discussed that you may re-apply for a higher star rating at any time by contacting your child care consultant. I notified you that the pathway system would apply to any applications made after July 1, 2025. Therefore, I encouraged you to review the pathway information available in the Star Rated License Modernization section of our website. Training classes: Health & Safety : This series of trainings include 10 topic areas. I observed today that you have completed CPR, First Aid, and Recognizing and Responding to Suspicions of Child Maltreatment. The remaining topic areas must be completed within your first year of licensure, and every five years thereafter. I encourage you to use the Health and Safety training log to ensure all topic areas are covered. These trainings will be monitored during your annual compliance visit. Emergency Preparedness and Response in Child Care: I observed that you completed this training on March 27, 2025. I reminded you that the facility’s Emergency Preparedness and Response plan must be created in the Emergency Management portal within four months of completing the training. Please remember to review and update your plan at least annually. You will also continue to conduct the shelter-in-place or lockdown drills every three months. On-going training: You will need to track your on-going training classes using the on-going training log, with your certificates attached. The health and safety training and college early childhood coursework can also count towards the on-going hours. CPR and First Aid training, however does not count. The number of hours required are based on your education and experience. Until you complete the North Carolina Early Childhood Credential, you will be required to earn 20 hours per year. We also discussed that you must have a professional development plan that is completed during your first year of licensure and is updated annually thereafter. Annual Licensing Fee: Pursuant to GS 110-90(1a) the Division establishes an annual fee for licensed child care facilities. The invoices for the fees are assessed based on your licensed capacity and mailed after the 1st of November every year. You will only receive one invoice and payment is due in FULL within 30 days. Please closely monitor your email and be on the lookout for your invoice. Please don’t delay making your payment and be aware that payment plans are NOT available. In addition, please be aware that failure to pay a license fee within 30 days of receipt will result in late charges. If payments are made late and late charges are applied, the fee as well as the late charge must be resolved. Please be reminded that, failure to pay a license fee could also result in the facility's child care license being revoked. Provider Portal for ABCMS (the criminal records check database): As stated in G.S. 110-90.2 & .2703(r), child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility (household members in a Family Child Care Home) within five business days. Previously, written notification such as the “Change of Information” form was used, however the process of notifying the Division has changed and is now captured in ABCMS. Once you receive your Star Rated License, you will need to use a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have already completed the training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff and household members are noted on the roster. This information should be updated in ABCMS in an ongoing basis. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted.
