Upon arrival I met with Narquasia Christian, one of the center administrators, and I explained the purpose of my visit. Allegation: There are allegations of violations of childcare requirements. I spoke with Ms. Christian about the self-reported incident which occurred on 6/4/26. She stated a one-year old child was left unattended on the playground. The self-report from previous administrator L. McFadden-Brunson stated it was for fourteen minutes. I asked Ms. Christian who reported the incident. She provided me with written statements from the two teachers in the classroom and one floater. According to the floater, she was informed by another teacher that a child had been left on the playground. The floater then reported it to the administrator. I reviewed the incident report provided to the parent on 6/4/26. Ms. Christian stated she was not currently able to access the video footage from the date of the incident. Based upon reports, the incident occurred around 8:30 am on 6/4/26. The transition policy from one environment to another by the center was a name to face recognition sheet. Staff did not follow the center’s policies during this transition. Ms. Christian stated that she revised the transition policy for staff. She stated she developed a PowerPoint and trained the staff on the new procedure. Staff must now contact an administrator before transitioning into a new area, inside or outside. The administrator will observe staff using the name to face sheet, review the sheet for accuracy, and administrator will scan playground to ensure all children are accounted for. She also stated that staff have zones on the playground where they are to move about and supervise children. The staff/child ratio at the time of the incident in this classroom was 1:5. Ten children were present. I reviewed the attendance records for 6/8/26 and 6/9/26 in the classroom for one-year old children. Staff child ratio of 1:5 was followed. I reviewed the attendance records for the classrooms on 6/15/26. Staff child ratio for school-age children was in compliance. The children were not combined with preschool children. Conclusion: The self-report was substantiated. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. During today's visit in room 151, one staff member was observed with five infants. The center operates with reduced, enhanced ratio. The ratio should be 1:4. GS 110-91(7);.0713(a-d) 303 Children were not adequately supervised at all times. On 6/4/26, a one-year-old child was left unsupervised on a playground for approximately fourteen minutes. .1801(a)(1-5) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Violations cited during the visit were discussed with Ms. Christian. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 7/6/26. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. All requested documentation must accompany the Corrective Action Letter. The letter can be emailed to lori.rierson@dhhs.nc.gov. Technical Assistance: During today’s visit, a violation of staff/child ratio was observed in the room for infants. Your center has a five-star rated license under Pathway 2. All classrooms are required to operate under reduced, enhanced ratios. In the room for infants, this means the ratio is 1:4. There are no exceptions to the staff/child ratio requirements. Upon arrival I met with LeSeanda Brunson-McFadden, one of the center administrators, and I explained the purpose of my visit. Allegation: There are allegations of violations of childcare requirements. I spoke with Ms. Brunson-McFadden about the self-reported incident which occurred on 4/22/26. She stated a child’s grandmother contacted her regarding an incident she observed on the center classroom cameras that night. This child is in overnight care. Ms. Brunson-McFadden stated the staff member was suspended the following day pending a review. She was later terminated. I observed the video footage from the reported incident. A staff member was observed rough handling a child during a diaper change. The child was resisting and appeared to not want to have the diaper change. The staff member was removing clothing in a rough manner and was observed popping the child on the bottom with an open hand. The child did have a diaper on. Another staff member had reported to Ms. Brunson-McFadden the following day that when she arrived, she could hear the child crying in the classroom. She stated she went into the room to offer the staff member a break. This was observed on the camera footage. Ms. Brunson-McFadden stated that all staff were required to complete a positive discipline training by May 8, 2026. Conclusion: The self-report was substantiated. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A child did not want his diaper changed. A staff member handled the child in a rough manner, removing clothes in a rough manner, and popped the child on the bottom with an open hand. The child was wearing a diaper. G.S. 110-91(10) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Violations cited during the visit were discussed with Ms. Brunson-McFadden. The violation(s) documented above must be corrected immediately.
