The last Annual Compliance visit was conducted on February 5, 2025. This facility currently operates with a Five-Star Child Care License that was renewed and issued on May 1, 2024. The last Sanitation inspection was conducted on October 9, 2025, with a “Superior” classification and zero (0) demerits. The last Fire Inspection was conducted on May 6, 2025, and the facility was approved for daytime care only. The last Fire Drill was conducted on December 16, 2025. The last Lock Down/ Shelter-in-Place was on September 3, 2025. Upon arrival, I signed in at the front school office and then proceeded to the preschool classroom. I was greeted by the lead teacher, Angela Byerly. I stated the purpose of my visit, and I proceeded on a walkthrough of the classroom where attendance was documented, and observations made. The children were observed participating in free play in centers and one on one instruction with teacher. A checklist was used to note the requirements I monitored today. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. This classroom is a combined EC/ NC PreK class. NC Pre-k children receive a Dial-4 assessment one time a year within the first 90 days of attendance. The results are reviewed with the parent and parent conferences are conducted two times a year. The teachers use Dojo to communicate with the parents and Ms. Byerly provides the parents with a monthly calendar and newsletter on what the children are working on that month. This facility uses Creative Curriculum and Teaching Strategies. Transportation is provided through Davidson County Schools. Currently no children are on medication. During today’s visit I monitored three (3) children’s files, and all were in compliance. All staff files were found to be in compliance. Your signature on the staff/training worksheets will serve as verification that all information provided is accurate and complete. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. As of 12/31/25, your facility is at a 98 percent compliance rate. The following violations were cited and technical assistance was offered. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. Technical Assistance: Today we discussed lockdown or shelter in place must be conducted every three months. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. This facility currently operates with a Five-Star Child Care License that was issued on May 1, 2024. The last Sanitation inspection was conducted on March 27, 2025, with a “Superior” classification and (4) demerits. The last Fire Inspection was conducted on May 6, 2025, and the facility was approved for daytime care only. The last Fire Drill was conducted on August 28, 2025. Lockdown/shelter in place is scheduled for this Friday September 3, 2025. Upon arrival, I was greeted by the lead teacher, Angela Byerly. I stated the purpose of my visit and completed a walkthrough of the classroom, where attendance was documented, and observations made. The class was participating in teacher directed actives. The children were engaged in playing dress up in dramatic play area and building a racetrack in the block area. The playground area had fresh new mulch around the play structures and tricycles to ride and a dramatic area outside with dolls to engage with. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. The following violations were observed during today’s visit and technical assistance was offered. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Ms. E. Best started on 8/25/25 and did not have a medical on file. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. The last Annual Compliance visit was conducted on March 20, 2023. The facility currently operates with a Five-Star Child Care License that was issued on October 17, 2018. The last Sanitation Inspection was completed on January 25, 2024, with a "Superior" classification and (5) demerits. The last Fire Inspection was conducted May 9, 2023, and the facility was approved for daytime care only. The last Fire Drill was conducted on February 1, 2024. The last Lock Down/ Shelter-in-Place was on February 2, 2024. The last Playground Inspection was conducted on February 16, 2024. Upon arrival, I was greeted by the lead teacher, Angela Byerly. I stated the purpose of my visit and was assisted on a walkthrough of the classroom where attendance was documented, and observations made. The children were observed participating group time. A checklist was used to note the requirements I monitored today. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections and Transportation. Transportation is provided through Davidson County Schools. The following violations were observed during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The top rail of the fence post needs to repaired. There is a large climber that has collapsed and is roped off so the children can not play on it. It has been set to be removed completely. There are stress cracks all over this piece of equipment. 10A NCAC 09 .0601(a) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. Technical Assistance: Today, Ms. Byerly stated she wanted to wait until the outside playground climber is removed before submitting the request for assessment. The climber outside is not in use due to the top level has broken and collapsed. There are several stress cracks all over the equipment. The teacher has taped off this piece of equipment and is not allowing children to play on it. Today, I spoke with the principle, and she is aware of this, and it has been set to be removed. Rated License Information: The facility’s rated license reassessment will resume on July 1, 2024. During today’s visit we reviewed the Environment Rating Scale assessment and discussed ways to improve the assessment score. We also discussed centers; proper hand washing procedures, asking open ended questions, using math concepts through the day, and that interactions are very important. I suggested to Ms. Byerly to visit the NCRLAP website for videos on handwashing. Today, we discussed when the time comes to submit your request, we will ask for an EC evaluator for your classrooms. We reviewed your staff’s education, and it appears that the program should remain eligible for 7 points in education standards, 7 points in programmatic. The center is also eligible for the Quality point by meeting (75) percent of the lead teachers have a BS/BA in ECE/CD and (75) percent of the lead teachers having an AS in ECE/CD. Once you are ready to have the ERS assessments, please let me know in writing. Be mindful that NCRLAP has asked that any schools that are closed during the summer should send their request no later than March 29, 2024, to be evaluated before the close of summer. Education: Please ensure that all the new staff member’s education information is current in the W.O.R.K.S. database. Each staff member must register in the WORKS system as soon as they are hired at this location. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment before the end of the temporary time-period. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements.
