The last annual compliance was conducted on March 10, 2025. Upon arrival, I was greeted by the assistant director, Andrea Camper. I conducted an assisted walk-through of the facility. Today the children are observed playing in centers and teachers directed activities in NC Pre-K. A checklist was used to note the requirements monitored during today's visit today. This facility currently operates with a Five-Star Child Care License that was issued on May 15, 2019. The last Sanitation inspection was conducted on October 20, 2025, and received a “Superior” classification and four (4) demerits. The last Fire inspection was conducted on March 17, 2025, and the facility was approved for daytime care only. Today a Ford 15 passenger van was monitored for general safety. Tires had plenty of tread on them and there was a mounted fire extinguisher and first aid kit in the van. The tag number LEK-2542 expires on October 31, 2026, and is insured by Breeden Insurance Services. The last Fire Drill was conducted on January 5, 2026. The last Lock Down/ Shelter-in-Place was on October 23, 2025. The last Playground Inspection was conducted on January 6, 2026. During today's visit, I completed a full assessment of Applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. Today we monitored three (3) children’s files. During today’s visit the following violations were cited and technical assistance was offered to maintain compliance. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Ms. O. Slemp's qualification letter expired on 2/1/26. G.S. 110-90.2(b) & .2703(n)&(o) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last practice was conducted on 10/23/25. .0604(u);.0302(d)(8) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before 5:00 pm February 25, 2026. The letter should include: Each item’s number of noncompliance cited. Technical Assistance: Today we discussed that lock down or shelter in place are required to be completed every 3 months for the children’s safety. All renewal of qualification letters must be completed prior to their expiration date. Mrs. O. Slemp qualification letter expired on February 1, 2026. Mrs. Slemp did go to get her qualifying letter renewed but for some reason it has not yet been recertified. Note: Criminal background checks can get backed up or may require more time for an out of state background check to take place. For this reason, all criminal background checks should be completed far enough in advance that does not expire. When I checked online the application had been submitted and the teacher did call criminal records to see if there was any progress on her application. They told her because she was from another state that the process was taking longer. Currently criminal records have extended her qualification letter until 2/19/26 to allow more time to process the check. Please be aware that if the 2/19/26 comes around and she has not received clearance from the state she may have to be out until her qualification letter comes back passed. REMINDERS: A. Camper qualification letter will need to be renewed before 6/2/26. The following First aid and CPR training courses will need to be renewed before 4/18/26. Marla, Autumn, Andrea, and Jessica Autumn needs to complete Administering Medication in Moodle for Health and Safety classes. Today all information on the renewal of your license was discussed in a TA visit following this annual compliance visit. I conducted an unassisted walk-through of the facility. Today the children are observed in circle time and playing in centers. A checklist was used to note the requirements monitored during today's visit today. This facility currently operates with a Five-Star Child Care License that was issued on May 15, 2019. The last Annual Compliance visit was conducted on April 16, 2024. The last Sanitation inspection was conducted on February 25, 2025, and received a “Superior” classification and four (4) demerits. There was a teachers purse sitting out where children could reach it and large trash bags were located in a cabinet near the floor where children could access them. This cabinet was unlocked. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There was a bottle of "equate hand soap" under a cabinet with no lock on it. .2820(b) 843 A drug or medicine was administered after its expiration date. The last Sanitation inspection was conducted on March 13, 2024, and received a “Superior” classification and thirteen (13) demerits. The last Fire inspection was conducted on March 19, 2024, and the facility was approved for daytime care only. The facility is currently listed as individual owner. Today a Ford 15 passenger van was monitored for general safety. Tires had plenty of tread on them and there was a mounted fire extinguisher and first aid kit in the van. The tag number DJH2901expires on July 31, 2024, and is insured by Selective Insurance Company of America. The last Fire Drill was conducted on March 1, 2024. The last Lock Down/ Shelter-in-Place was on March 10, 2024. The last Playground Inspection was conducted on April 12, 2024. During today's visit, I completed a full assessment of Applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. Today we monitored five (5) children’s files. During today’s visit the following violations were cited and technical assistance was offered. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. Play sink faucet needs to be tightened. G.S. 110-91(6); .0601(b) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the hallway the laundry room was open and unlocked. There was a Preston De-icer in the van. .2820(b) 851 When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. There is a child on a Pro-air inhaler and there was not a permission form to administer medication. These are only good for six months. .0803(13)(a-e); .2318(3) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before 5:00 pm April 29, 2024. The letter should include each item’s number of noncompliance cited, describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received by my office on, or before 5:00 pm April 29, 2024, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Technical Assistance: Please remind all staff to keep closet doors locked that have chemicals in them. Please make sure that all chemicals are in a locked closet and not on the van where children are present. All children that use inhalers or Epi pens must have an action form signed by the parent and permission to administer the medication. Please have your teachers check their classrooms once a day for broken toys or furniture. UPDATES: Center for Disease Control and Prevention new guidelines as of March 1, 2024. If you, your staff, or the children you care for should test positive for COVID-19, the CDC has given out new guidelines to follow. If you have a fever the CDC recommends self-isolation for 24 hours or longer, until you are fever free without any fever reducers. Please check our website for the latest Covid-19 information.
