The last Annual Compliance visit was conducted on May 25, 2025. The facility currently operates with a Five-Star Child Care License that was issued on March 23, 2017. The last Sanitation Inspection was completed on March 27, 2026, with a "Superior" classification and seven (7) demerits. The last Fire Inspection was conducted on March 6, 2026, and the facility was approved for daytime care only. The last Fire Drill was conducted on April 29, 2026. The last Lock Down/ Shelter-in-Place was on April 23, 2026. The last Playground Inspection was conducted on April 16, 2026. Upon arrival, I was greeted by the director, Wendy Sheehy. I stated the purpose of my visit and was assisted on a walkthrough of the facility where attendance was documented, and observations made. The children were playing inside and on playground and infants were napping. During today's visit, I completed a full assessment of Applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. A checklist was used to note the requirements I monitored today. On May 24, 2026, the North Carolina Secretary of State website was used to check the corporation status. As of this date it has been found active and current. Five (5) children’s records were monitored during today’s visit. Staff training worksheets were reviewed per the Divisions requirements. This facility does not provide transportation. All medication was monitored today and found in compliance. The following violations were cited, and technical assistance was offered to maintain compliance. Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. In space 4 and 5 there was no thermometer in the fridge. 15A NCAC 18A .2806(j)(2) 807 A safe indoor and outdoor environment was not provided for the children. A teacher in space 5 did not have shoes on. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space 2 there was a teachers purse sitting out on the cabinet. .2820(b) 861 Prohibited styrofoam and foam rubber products were accessible to children under 3 years of age and/or approved foam products were used without proper supervision. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. Teachers should always wear shoes in the classroom and outside. This is for the safety of the teacher and for the children in the event of an emergency. All fridges should have a working thermometer in them and must maintain at least 45 degrees. Attendance must be taken every morning to know how many children are present in the event of an emergency. ITS-SIDS policy states that a sleeping child must be laid on their back in a crib or mat until they can roll over. This is repeat violation. If a child falls asleep in a carrier he must be removed and placed in a crib or mat. Reminder: Please remember to compete a fire drill and playground inspection before the end of the month. All staff purses must be locked up and out of children's reach 5 feet off the ground. Children under 3 year's of age cannot have certain or dangerous items around them. Children of this age like to eat or chew on things and foam is a choking hazard. There was a cushion with exposed foam on the floor in a two year old class. Upon arrival, I was greeted by the director, Wendy Sheehy. I stated the purpose of my visit and was unassisted on a walk-through to the facility, where attendance was documented, and observations made. Today the children were playing outside and the infants were having tummy time and being held to get fed. The last Fire Drill was documented on October 25, 2025. The last Shelter-in-Place Drill was documented on Oct 23, 2025. The last Playground Inspection was documented on November 3, 2025. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. The following violations were cited, and technical assistance was offered. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In the toddler classroom the lesson plan was dated for last week 10/27-10/31/25. In the two year old classroom the teacher removed the covers on the couches to clean them but left the Styrofoam exposed around the children. Andrea Camper, assistant director accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Camper and three additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. On 10/17/24, a staff member used medication on a child's injury without written permission. 10A NCAC 09 .0803(1)(a & b) Violations must be corrected immediately. Upon arrival at the facility, I was greeted by the assistant director, Andrea Camper. Wendy Sheehy came in around twenty minutes after I arrived. I stated the purpose of my visit and was assisted on a walkthrough of the facility. ALLEGATION: There is a concern regarding rough handling of a child. OBSERVATION: Today I interviewed the director, Wendy Sheehy and the assistant director, and two staff members. The director and I discussed the allegations, and she and the two staff members were given the opportunity to share any concerns. Today, I completed an observation of the two-year-old classroom. Since the reporter did not give the time or day of this occurrence, I reviewed a video from the two-year-old classroom for about thirty minutes. During this time, I did see a teacher aggressively removing a child that was climbing on a shelf. The director stated that she does not know how to record and save a video. Today there were (13) children and (2) staff members in the two-year-old class. CONCLUSION: Based on the interviews of the director and two staff members and video footage of this allegation has been confirmed and is Substantiated. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. Today the consultant viewed a video for thirty minutes and witnessed a teacher aggressively grab a child to remove them from on top of a shelf. G.S. 110-91(10) 1757 A valid qualification letter was not on file and available to review at the facility. There was not a qualification letter in a staff members file. Upon arrival at the facility, I was greeted by the assistant director, Andrea Camper. Wendy Sheehy came in around twenty minutes after I arrived. I stated the purpose of my visit and was assisted on a walkthrough of the facility. ALLEGATION: There is a concern regarding rough handling of a child. OBSERVATION: Today I interviewed the director, Wendy Sheehy and the assistant director, and two staff members. The director and I discussed the allegations, and she and the two staff members were given the opportunity to share any concerns. Today, I completed an observation of the two-year-old classroom. Since the reporter did not give the time or day of this occurrence, I reviewed a video from the two-year-old classroom for about thirty minutes. During this time, I did see a teacher aggressively removing a child that was climbing on a shelf. The director stated that she does not know how to record and save a video. Today there were (13) children and (2) staff members in the two-year-old class. CONCLUSION: Based on the interviews of the director and two staff members and video footage of this allegation has been confirmed and is Substantiated. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. Today the consultant viewed a video for thirty minutes and witnessed a teacher aggressively grab a child to remove them from on top of a shelf. As of this date it has been found active and current. Four children’s records were monitored during today’s visit. The following staff/child ratios were observed: Space #1: 9 infants with Jenny, Hannah and Skylar Space #2: 7 children who are one-year-old children with Allison and Elka Space #3: 3 children who are two years old with Jennifer Space #4: 10 children who are two-years-old children with Kristine and Courtney Space #5: 15 children who are three-years-old children with Stephanie and Alyssa. Limited monitoring of child care requirements occurred during today’s visit. I monitored, health and safety requirements, Supervision, Discipline, nurture, or care of children; Staff/child ratio; Group size, Licensed capacity; Permit restriction; CPR training; First Aid training; ITS-SIDS training; and Criminal record checks. All staff trainings and qualifying letters are up to date and documentation was present. All emergency drills are current. All playground inspections are current. Fire and Sanitation inspections are current. The following violations were observed cited and technical assistance was provided. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In a classroom with children who are three years old, three outlets were observed uncovered and were not in use. This was corrected during the visit. 10A NCAC 09 .0604(c) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In a classroom with children who are two years old, plastic "grocery" bags were observed stored in children's cubbies and in a children's rest room. .0604(q) The above violations must be corrected immediately, and a letter sent to my office stating how the violations were corrected. A current staff member did not attend the initial training due to medical absence. The staff member has since completed the trainings. Documentation today, show all current staff members have been trained in the required subject matters stated above. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care.
