Name of Operation: UNION GROVE CHRISTIAN SCHOOL Facility ID: 2959000 Consultant: RENA MCCAFFREY Operation Type: Center Case Number: Visit Date: 12/17/2025 Number Present: 53 Completed Date: 12/17/2025 Age: From 3 To 4 Total Minutes: 120 Time In: 09:05 AM Time Out: 11:05 AM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to complete an annual monitoring of Applicable Child Care requirements. The last Annual Compliance visit was conducted on January 24, 2025. The facility currently operates with a GS-110 Notice of Compliance that was issued on February 6, 2009. The last Sanitation Inspection was completed on November 19, 2025, with a "Superior" classification and seven (7) demerits. The last Fire Inspection was conducted on October 15, 2025, and the facility was approved for daytime care only. The last Fire Drill was conducted on December 12, 2025. The last Lock Down/ Shelter-in-Place was on November 24, 2025. The last Playground Inspection was conducted on December 11, 2025. Upon arrival, I was greeted by the director, Amanda Lambert. I stated the purpose of my visit and was assisted on a walkthrough of the facility where attendance was documented, and observations made. The children were observed having snack time and engaged in teacher instruction. A checklist was used to note the requirements I monitored today. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections and Transportation. Today I monitored five children's files, and all were in compliance. This facility provides no transportation, and three children were currently on medication at this time. The following violations were cited and technical assistance was offered. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. A large piece of playground equipment need have sufficient mulch placed around the pole and at the bottom of the slide. .0605(j) 721 All equipment and furnishings were not in good repair. The S-hooks on the swings were coming apart and need to be repaired. The paint is chipping on the six seat bouncy equipment and has caused rust to appear. G.S. 110-91(6); .0601(b) 847 Parent's medication authorization did not include required information. One child did not have all the required information on the form. There was no ending date on permission to administer medication or pediatrician contact name and phone number. This was for an over the counter cough syrup from Hylands Cough and Cold Day Time medication. 10A NCAC 09 .0803(4)(6-9) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before 5:00 PM December 30, 2025. The letter should include: Each item number of noncompliance cited. Describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received to my office by 5:00 PM December 30, 2025, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. Technical Assistance: Today we discussed that the playground needed mulch around the bottom of the slide and of the pole. Paint was chipping on the six seated bouncy equipment and was rusted. This will need some sanding down and repainting. The S-hooks on the swings were coming loose and could cause the chain to come off the s hook and cause the child to fall to the ground. Today we discussed some options for repairing this. One child that was on a cough medicine needed to complete a new medication form in its entirety. The medication form must have a start and end date on the form as well as the child’s pediatrician name and contact information should any allergic reaction from the medication arise. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Reminders: Today we discussed sending out information to all parents that have chosen not to vaccinate their child that in the event of an outbreak example(measles) they must keep their child at home until the outbreak in the school is over. This facility currently operates as a GS-110 Notice of Compliance, that was issued on February 6, 2009. The last Sanitation was conducted on March 28, 2025, with a “Superior” classification and six (6) demerits. The last Fire Inspection was conducted on April 17, 2025, and the facility was approved for daytime care only. Today is the third day of their new year and since this facility was closed during the summer I will review lock down/shelter in place on another visit. The last playground inspection was conducted on August 12, 2025. Upon arrival, I was greeted by the director, Amanda Lambert. I stated the purpose of my visit and was assisted on a walkthrough of the facility, where attendance was documented, and observations made. The children were having snacks and engaging in teacher-directed activities and some classes were going outside. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. The following violations were cited, and technical assistance was offered. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. G.S. 110-91(6); .0601(b) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Today there was a teachers purse at a desk down by children. There were wipes with warnings for children on them on level with the children. .2820(b) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before 5:00 pm August 26, 2025. The letter should include: Each item number of noncompliance cited. Technical Assistance: Today we discussed that all purses must be up locked up. Purses can contain many things like medication, lotions and other hazardous items to young children. Any items with warnings for children must be kept locked up. Today as we walked the playground area mulch had plenty of cushion but because of the rain will need to be fluffed. On the large climber there were exposed bolts that will need to be cut down and filed so there are no sharp edges. The bolts need to be covered with some mulch or caps. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. The last Sanitation Inspection was completed on February 1, 2024, with a "Provisional " classification and (14) demerits. The last Fire Inspection was conducted on January 30, 2024, and the facility was approved for daytime care only. The last Fire Drill was conducted on January 24, 2024. The last Lock Down/ Shelter-in-Place was on January 26, 2024. The last Playground Inspection was conducted on February 5, 2024. Upon arrival, I was greeted by the director, Amanda Lambert. I stated the purpose of my visit and was assisted on a walkthrough of the facility where attendance was documented, and observations made. The children were observed eating snack. A checklist was used to note the requirements I monitored today. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Today I monitored five children's files. This facility does not transport children. The following violations were observed during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Today two swings on the playground outside did not have their S hooks closed. 10A NCAC 09 .0601(a) 1322 A written statement from each child's parent giving standing permission which may be valid for up to twelve months for participation in off premise activities that occur on a regular basis was not available. All children's files that I reviewed today did not have this form in their files. .1005(b)(4) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. This form was not present in all the children's files I reviewed today. .1804(c) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The mulch measured 2.5 inches deep around two very large pieces of equipment. .0605(k)(1-4) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. This form was not in all of the children's files. .0608(b)(1-6) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before February 21, 2024. The letter should include: Each item number of noncompliance cited. Describe how and when the violations were corrected. Technical Assistance: Today we discussed that all parents should read and sign the Shaken Baby Syndrome, Discipline and Outside the fence forms upon enrollment. Today I left copies of this form with Ms. Amanda and showed her where to access them on line. Today we discussed that all S hooks must be closed on swings and mulch should be six inches deep around all stationary equipment over 18 inches high. The swings did have rubber pads under them, but the mulch was only measuring 2.5 inches deep, two times the pivot point. There were two large pieces of equipment that the mulch measure 2.5 inches deep around them. Amanda Lamberts criminal background check will be due before May of 2024. First Aid and CPR is due this year in September for several staff. If you have any questions feel free to call me.
