Facility ID: 29000727 Consultant: RENA MCCAFFREY Operation Type: Center Case Number: Visit Date: 9/23/2025 Number Present: 6 Completed Date: 9/23/2025 Age: From 2 To 4 Total Minutes: 156 Time In: 09:19 AM Time Out: 11:55 AM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to complete an annual monitoring of Applicable Child Care requirements. The last Annual Compliance visit was conducted on October 16, 2024. This facility currently operates with a Four-Star Child Care License that was issued on March 23, 2020. The last Sanitation Inspection was completed on February 27, 2025, with a "Superior" classification and eight (8) demerits. The last Fire Inspection was conducted on April 23, 2025, and the facility was approved for daytime care only. The last Fire Drill was conducted on August 8, 2025. The last Lock Down/ Shelter-in-Place was on August 11, 2025. The last Playground Inspection was conducted on September 16, 2025. Upon arrival, I was greeted by director, Brenda Milloway. I stated the purpose of my visit and was assisted on a walkthrough of the facility where attendance was documented, and observations made. The children were observed during outside activities. A checklist was used to note the requirements I monitored today. The North Carolina Department of the Secretary’s website was used to check the status of “The Childcare Center One Inc.” corporation on September 19, 2025. The corporation’s status is current/active. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground and Inspections. Today two (2) children’s files were monitored for compliance. Today I monitored a 2020 Chevrolet Express 15 passenger van. Tag number TJ6391 and it expires on 7/31/2026. This vehicle is insured through Progressive Premier Insurance expires on 2/28/26. This vehicle did not have a mounted fire extinguisher or first aid kit available. Medication was monitored and currently there are no children on medication. The following violations were cited, and technical assistance was offered. Violation Number Comment Rule 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. The van did not have a mounted fire extinguishers or first aid kit in the vehicle. 10A NCAC 09 .1003(c) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. C. Milloway could not find a copy of her training. If one cannot be found this training must be retaken. .1102(g) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before October 3, 2025. The letter should include: Each item number of noncompliance cited. Describe how and when the violations were corrected. Technical Assistance: Today we discussed that all vehicles must have a first aid kit and mounted fire extinguisher in a vehicle transporting children. C. Milloway needs to find her copy of her Recognizing and Responding to Suspicions of Maltreatment. Ms. Milloway stated that she took it but cant find it. Today we discussed how to organize staff files and the director had some questions regarding education for her staff under the new Pathways to the Stars. Reminder: All staff need to renew their First Aid and CPR training before 10/2025. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. The last sanitation inspection was conducted on March 22, 2024 and received a “Superior “classification and six (6) demerits. The last Fire Inspection was conducted on April 3, 2023, and was approved for daytime care only. The last Fire Drill was conducted on March 6, 2024. The last Lock Down/ Shelter-in-Place was on April 3, 2024. The last Playground Inspection was conducted on March 11, 2024. Upon arrival, I was greeted by Ms. Marcha and the director Brenda Milloway came about ten minutes later. I stated the purpose of my visit, and I conducted a walk-through of the facility, where attendance was documented, and observations made. The children were observed playing outside. Today, a partial assessment of Applicable Child Care Requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. During today’s visit the following violations were cited and technical assistance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was on April 3, 2023. 10A NCAC 09 .0304(a) 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Today when I arrived only one person was on the playground that was qualified to be counted in the ratios. There were fifteen children ranging from age two to five. GS 110-91(7);.0713(a-d) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff member did not have a medical in her file. 10A NCAC 09 .0701(a) 1757 A valid qualification letter was not on file and available to review at the facility. One staff member did not have a qualifying letter watching the children with one other staff member. G.S. 110-90.2(b) & (d) & .2703(e) You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Technical Assistance: Today when I arrived there were two staff members on the playground and only one of them had a qualifying letter. This means that you were over ratio with the children. If the staff member that was out on the playground wants to continue to be at the facility, she must have all the requirements needed otherwise there must be always at least two staff members that are qualified to be around children for the number of children present today. All medical information must be kept in a separate file from their main staff file. This is a HIPPA requirement. Medical, TB test or screening and Health questionnaire must be kept in a separate file. Staff must have medical and TB before the first day of work. As for your fire inspection they must be completed in advance of the expiration date. One staff member will need to have the first two weeks of orientation completed by Friday the 19th. This staff member will also need to complete Recognizing and Responding to Suspicions of Child Maltreatment before July 2, 2024. The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. This facility currently operates with a Four-Star Child Care License that was issued on March 10, 2020. The last sanitation inspection was conducted on March 22, 2024 and received a “Superior “classification and six (6) demerits. The last Fire Inspection was conducted on April 3, 2023, and was approved for daytime care only. The last Fire Drill was conducted on March 6, 2024. The last Lock Down/ Shelter-in-Place was on April 3, 2024. The last Playground Inspection was conducted on March 11, 2024. Upon arrival, I was greeted by Ms. Marcha and the director Brenda Milloway came about ten minutes later. I stated the purpose of my visit, and I conducted a walk-through of the facility, where attendance was documented, and observations made. The children were observed playing outside. Today, a partial assessment of Applicable Child Care Requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. During today’s visit the following violations were cited and technical assistance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was on April 3, 2023. 10A NCAC 09 .0304(a) 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Today when I arrived only one person was on the playground that was qualified to be counted in the ratios. There were fifteen children ranging from age two to five. GS 110-91(7);.0713(a-d) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff member did not have a medical in her file. 10A NCAC 09 .0701(a) 1757 A valid qualification letter was not on file and available to review at the facility. One staff member did not have a qualifying letter watching the children with one other staff member. G.S. 110-90.2(b) & (d) & .2703(e) You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. Technical Assistance: Today when I arrived there were two staff members on the playground and only one of them had a qualifying letter. This means that you were over ratio with the children. If the staff member that was out on the playground wants to continue to be at the facility, she must have all the requirements needed otherwise there must be always at least two staff members that are qualified to be around children for the number of children present today. All medical information must be kept in a separate file from their main staff file. This is a HIPPA requirement. Medical, TB test or screening and Health questionnaire must be kept in a separate file. Staff must have medical and TB before the first day of work. As for your fire inspection they must be completed in advance of the expiration date. One staff member will need to have the first two weeks of orientation completed by Friday the 19th. This staff member will also need to complete Recognizing and Responding to Suspicions of Child Maltreatment before July 2, 2024. The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received in my office on, or before 5:00 pm April 26, 2024. The letter should include each item’s number of noncompliance cited, describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received in my office before 5:00 pm April 26, 2024, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. COVID-19 INFORMATION: Center for Disease Control and Prevention new guidelines as of March 1, 2024. If you, your staff, or the children you care for should test positive for COVID-19, the CDC has given out new guidelines to follow. If you have a fever the CDC recommends self-isolation for 24 hours or longer, until you are fever free without any fever reducers. Please check our website for the latest Covid-19 information. DCDEE website https://ncchildcare.ncdhhs.gov UPDATES: All providers received information on July 2, 2019, regarding immediate changes to Health Care Information Compliance for child care operators and staff pertaining to child care rule 10A NCAC 09 .0701. The new guidelines are as follows: The Staff Medical Report, Health Questionnaire, Tuberculosis Test or Screening, and Emergency Information forms have been updated and are available of the Division on Child Development and Early Education website at https://ncchildcare.ncdhhs.nc.gov/Provider/Provider-Documents-and-Forms. Federal law also requires that an employer keep all medical information confidential and in separate medical files. Any staff medical statements, any proof of tuberculosis test or screening, and any completed health questionnaires must be included in a staff member's medical file, which must be maintained separately from that staff member's individual personnel file.
