Name of Operation: SOUTHWOOD MORE AT FOUR Facility ID: 29000604 Consultant: RENA MCCAFFREY Operation Type: Center Case Number: Visit Date: 3/3/2026 Number Present: 0 Completed Date: 3/3/2026 Age: From 0 To 0 Total Minutes: 77 Time In: 09:30 AM Time Out: 10:47 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's Routine Unannounced visit was to monitor compliance with Child Care Requirements. Upon arrival I entered the main office of the school and was then directed to classrooms 104 and 200. I was greeted by the lead teacher, Mrs. Beck in classroom 104 and Ms. Anderson in room 200. I stated the purpose of my visit. Today was a teacher’s workday so no children were present during today’s visit. The last sanitation visit was conducted on 12/1/25 and received zero (0) demerits and a “Superior” Classification. The last Fire Inspection was conducted on 10/06/24 and was approved for daytime care only. I will come back on another visit to monitor when children are present. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions and qualified criminal background checks, staff training and general safety. The last Fire Drill was completed on February 10, 2026. The last Lock-down or Shelter-in-Place was conducted on February 25, 2026. The following violations were cited, and technical assistance was provided to maintain compliance. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection I received was during annual compliance visit on 10/6/25. That fire inspection was dated December 16, 2024. Ms. Anderson stated that there was an inspection conducted on 10/8/25 but there were items that needed to be corrected. Ms. Anderson stated that they will conatact the fire marshall. 10A NCAC 09 .0304(a) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before March 13, 2026. The letter should include: Each item number of noncompliance cited. Describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received at my office by 5:00 pm March 13, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. TECHNICAL ASSISTANCE: Today we discussed that a passed fire inspection must be completed annually. A copy of the fire inspection once completed must be emailed to your consultant within 5 business day. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. T. Walker will need to renew her FA /CPR training in June of 2026 H. Anderson will need to renew her FA/CPR training in April of 2026. Ms. Anderson will need to renew her criminal background check before August of 2026.
