Name of Operation: SOUTH LEXINGTON DEVELOPMENTAL CENTER Facility ID: 29000703 Consultant: RENA MCCAFFREY Operation Type: Center Case Number: Visit Date: 4/30/2026 Number Present: 18 Completed Date: 4/30/2026 Age: From 6 To 12 Total Minutes: 51 Time In: 09:04 AM Time Out: 09:55 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's Routine Unannounced visit was to monitor compliance with applicable child care requirements. This facility currently operates with a Five-Star Child Care License that was issued on May 29, 2024. This facility qualifies as a Developmental Day program and can serve children from ages 5-21 years of age. The last Fire Inspection was conducted on December 18, 2025, and was approved for daytime care only. The Sanitation inspection was conducted on December 1, 2025, and received a “Superior” classification and zero (0) demerits. The last Fire Drill was conducted on March 24, 2026. The last Lockdown/Shelter in Place was conducted on February 24, 2026. Upon arrival, I was greeted by the assistant principle, Monique Wells. I stated the purpose of my visit and was assisted on a walk-through of the facility where attendance was documented, and observations made. The children were observed participating in routines, and indoor teachers directed activities. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. The following violations were observed during today’s visit and technical assistance was offered to maintain compliance. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. All staff were not in the ABCMS portal. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received in my office on, or before 5:00 pm May 13, 2026. A Corrective Action Letter must include the following items: • The name of your center • Facility ID number • Each violation numbers • How you corrected each violation • Title of the person who signs the letter • Date Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov . If a compliance letter is not received to my office on, or before 5:00 PM May 13, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violations may be documented if each violation has not been properly corrected or otherwise addressed. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. Technical Assistance: Childcare Centers: The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. This facility currently operates with a Five-Star Child Care License that was issued on June 3, 2024. This facility qualifies as a Developmental Day program and can serve children from ages 5-21 years of age. The last Fire Inspection was conducted on March 6, 2025, and was approved for daytime care only. The sanitation inspection was conducted on April 23, 2025, and received a “Superior” classification and zero (0) demerits. The last Fire Drill was conducted on August 8, 2025. The last Lockdown/Shelter in place. This facility just started back so I will monitor this on my next visit. The last Playground inspection was on August 13, 2025. Upon arrival, I was greeted by the new assistant principle, Monique Wells. I stated the purpose of my visit and was assisted on a walk-through of the facility with the Pre-K coordinator, Ms. Sonya Cooper and Ms. Wells, where attendance was documented, and observations made. The children were observed participating in routines, and indoor teacher directed activities. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. The following violations were observed during today’s visit and technical assistance was offered. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Large trash bags where in space 311 where children could reach them and an electrical whole punch was also found in this classroom. .2820(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Three staff did not have first aid trainings on file. M. DeVere, C, Cardange and E. McNair. .1102(c) 1757 A valid qualification letter was not on file and available to review at the facility. A new staff member, M. Wells did not have a qualification letter on file. G.S. 110-90.2(b) & (d) & .2703(e) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The mulch around the swings did not the required levels of cushion for the height of the equipment being used. .0605(k)(1-4) The above violations must be corrected immediately. Technical Assistance: During today’s visit I was introduced to the new assistant principal Monique Wells. Ms. Wells is new to position, so we did a review of files and what is required to be on file. We talked about frequency of training and how long forms must be kept on file. Ms. Cooper assisted in this visit. Upon reviewing Ms. Wells’ file, she had not yet received her qualification letter. The application was completed on August 8, 2025, and is in processing. I explained that all new hires must have a qualification letter, medical, TB or TB screening, and health questionnaire and emergency form before day one of hire and may not work with children until all requirements have been received. Today we discussed that all staff must complete First Aid training within 90 days of hire date. Hazardous items such as large trash bags and electrical items such as hot glue guns, laminators or anything that can become heated with use can cause a child to get burned and must be locked up out of children’s reach. Playgrounds must have the required amount of cushion around fall zones such as near the bottom of slides or climbers over 18 inches. This cushion (mulch) will help support a fall from height. Ms. Sonya stated that a maintenance request was already put in for more mulch to be added and tilled around these areas.
