    Name of Operation: SILVER VALLEY ELEMENTARY Facility ID: 29000602 Consultant: RENA MCCAFFREY Operation Type: Center Case Number: Visit Date: 9/8/2025 Number Present: 14 Completed Date: 9/8/2025 Age: From 4 To 4 Total Minutes: 88 Time In: 09:40 AM Time Out: 11:08 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's Routine Unannounced visit was to monitor compliance with applicable child care requirements. This facility currently operates with a Five-Star Child Care License that was issued on January 5, 2018. The last Sanitation Inspection was on March 13, 2025, and received a “Superior” classification and (0) demerits. The last Fire Inspection was conducted on March 19, 2024, and the facility was approved for daytime care only. The last Fire Drill was conducted on August 28, 2025. The last Lock Down/ Shelter-in-Place was on September 4, 2025. Upon arrival, I was greeted by the lead teacher, Stephanie Yarborough. I stated the purpose of my visit and completed a walk-through of the classroom, where attendance was documented, and observations made. The children were playing in centers, and one teacher was in the reading corner with a child. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. During today’s visit the following violations were cited and technical assistance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last inspection was conducted on 3/19/24. The teacher stated that there has been a fire inspection conducted since then but did not have a copy of it. 10A NCAC 09 .0304(a) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received in my office on, or before 5:00 pm September 19, 2025. A Corrective Action Letter must include the following items: • The name of your center • Facility ID number • Each violation numbers • How you corrected each violation • Title of the person who signs the letter • Date Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received at my office by 5:00 pm September 19, 2025, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Technical Assistance: Please make sure that when fire inspections are conducted you get a copy of them each time. Ms. Yarborough stated that there was a fire inspection around April of this year but cannot find a copy of it. Please send me a copy of the fire inspection once you locate it. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Today four (4) children’s files were monitored and found to be in compliance. This facility does not transport children. Today medication was monitored. During today’s visit the following violations were cited and technical assistance was offered. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. One child with an Epi pen did not have a permission form signed by the parent to administer this medication. 10A NCAC 09 .0803(1)(a & b) The above violations must be corrected immediately. If you have a fever the CDC recommends self-isolation for 24 hours or longer, until you are fever free without any fever reducers. Please check our website for the latest Covid-19 information. The new guidelines are as follows: The Staff Medical Report, Health Questionnaire, Tuberculosis Test or Screening, and Emergency Information forms have been updated and are available of the Division on Child Development and Early Education website at https://ncchildcare.ncdhhs.nc.gov/Provider/Provider-Documents-and-Forms. Federal law also requires that an employer keep all medical information confidential and in separate medical files. Any staff medical statements, any proof of tuberculosis test or screening, and any completed health questionnaires must be included in a staff member's medical file, which must be maintained separately from that staff member's individual personnel file. Seven (7) points in program standards. One (1) quality point for education options met. Today we discussed the process of the Rated License Assessment. There you will find more checklists, forms, notes for clarification, and webinars. Also, reviewing the lower scoring items on your prior assessment can help in your preparation. The following recommendations were made: •Remove dramatic play toys (pirate ship) from the block center. These toys should be in the dramatic play area. •The block area must have a 6x9 area large enough for three children to each build a large structure in it. Please use your ECERS-R scoring book for the correct number and types of blocks for this area. •All center shelves should be labeled and have open containers for easy access. •Please see requirements for reading area. Books must be checked for any violent pictures and books that have torn pages or that have been marked on should be removed. The ECERS-R book provides a list of what kinds of books should be in your reading corner, for example, fantasy, fiction, self-help, multicultural etc. A minimum of twenty books should be available for a group of fifteen children in the ECERS rooms. One additional book per child over fifteen. •Make sure that your lesson plan reflects what is around your classroom. For example, if you are working on a pumpkin investigation there should be examples of the children’s artwork, graphs or findings that match your lesson plan. Take photos while the children are investigating the pumpkins and put them up around the classroom so that children may reflect later on what they learned. Use real life pictures not animated ones. •Rooms assessed for ECERS should have a music player available for use by the children. Children under four years of age can have use with assistance. A minimum of three different types of music should be available. •Most of the children’s work must be child-led art and show examples of 3D art. All staff should continue to review the assessment books and score their classrooms. Lead teachers should familiarize themselves with the questions following the indicators.
