The last Annual Compliance visit was conducted on November 26. 2024. The facility currently operates with a Five-Star Child Care License that was issued on May 18, 2019. The last Sanitation Inspection was completed on April 9, 2025, with a "Superior" classification and (6) demerits. The last Fire Inspection was conducted on May 8, 2025, and the facility was approved for daytime care only. The last Fire Drill was conducted on September 2, 2025. The last Shelter-in-Place was on September 2, 2025. Upon arrival, I was greeted by the lead teacher, Brittany Jones. I stated the purpose of my visit and was assisted on a walkthrough of the classroom where all centers and playground was monitored. Today was an early release day and the children only came in from 8:30 10:30. Today no children attended. A checklist was used to note the requirements I monitored today. Today I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, and Medication. Transportation is provided by the parents. No children are on medication at this time. The following violations were observed during today’s visit and technical assistance was offered. Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. There was no thermometer in the classroom refrigerator. 15A NCAC 18A .2806(j)(2) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Anew teacher T. Wyre did not get a TB test or screening done prior to start date. .0701(a) 1303 Application was not signed by the parent. A child's application was not signed by the parent. 10A NCAC 09 .0801(a) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. A parent was not provided with a copy of this form. .0608(b)(1-6) 1946 A valid provisional qualification letter was not on file and available for review. A new teacher T. Wyre started on 8/11/25 and did not get a criminal background check completed prior to start date. G.S. 110-90.2(b) & .2703(e) The above violations must be corrected immediately. Technical Assistance: Today we discussed that all new staff must have a qualifying letter prior to working with children. All new staff are required to have a TB test and physical prior to working with children. The classroom refrigerator must always have a thermometer in it to make sure the temperature does not fall below 45 degrees. Today we spoke about choosing a pathway to the stars. Ms. Brittany stated they are still researching that. I did offer to come and give TA on it. Ms. Brittany did state that they would like a TA visit. I will look at my calendar and get back with you on a date for our visit. Parents must sign and date all applications and paperwork and date them. All parents must receive and explain the purpose of Shaken Baby Syndrome and then sign and date that they receive it. This must be placed in the children’s file. This facility currently operates with a Five-Star Child Care License that was issued on May 8, 2019. The last Sanitation Inspection was conducted on April 9, 2025, and received a “Superior” classification and six (6) demerits. The last Fire Inspection was conducted on October 22, 2024, and the facility was approved for daytime care only. The last Fire Drill was conducted on March 14, 2025. The last Lock Down/ Shelter-in-Place was on January 24, 2025. Upon arrival at the front office of the school I was then directed to classroom 200. There I was greeted by the lead teacher, Terri Clement. I stated the purpose of my visit, and I conducted a walkthrough of the classroom, where attendance was documented, and observations made. The children were playing centers and painting easter eggs. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. The following violations were observed, and technical assistance was offered. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A. Scott did not have first aid training. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A. Scott did not have CPR training. .1102(d) 1757 A valid qualification letter was not on file and available to review at the facility. A. Scott's qualifying letter has expired as of 12/12/24. This teacher cannot work with children until a current qualifying letter has been obtained. G.S. 110-90.2(b) & (d) & .2703(e) The above violations must be corrected immediately. Ms. Scott cannot return until a qualifying letter is cleared and placed in her file. First aid and CPR training is needed for anyone working with young children on a regular basis. Kenny Sledge at Smart Start provides First Aid and CPR training. Her contact information is 336-479-6739 for the next available training. Reminders: Your fire inspection will need to be renewed before October 22, 2025. The children were observed participating in teacher directed group time activities and free play. A checklist was used to note the requirements I monitored today. I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records, Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections. GS110-91(1) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Ms. Brown did not complete this training with in the 90 days of hire date. .1102(g) The above violations must be corrected immediately. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Technical assistance: All new hired staff must complete the Health and Safety Trainings located on "Moodle" and must be completed during the first year of employment. The Recognizing and Responding to Suspicions of Maltreatment must be completed within 90 days of their hire date. All children must have a medical within 30 days of enrollment. New letters will not be sent out. -Current Rated Licenses are still in a ‘hold harmless’ state until June 2024. It has been determined that your facility is in Cohort 2. This means your prep year is from July 1, 2024, through June 30, 2025. Your reassessment year is from July 1, 2025, through June 30, 2026. Please make sure that all your staff’s education is in “Works” located on the Division of Child Development website at //ncchildcare.ncdhhs.gov/. During your preparation year you can request the Environmental Rating Scales and you determine how and when to use the scores.
