The last Annual Compliance was conducted on March 1, 2024. Currently this program operates with a Four-Star License issued on May 1, 2018. Upon my arrival, I was greeted by the teacher assistant, Shannon Light. The lead teacher, Ms. Jessica Murphy, is out with an illness. Ms. Light assisted me during my visit and attendance was recorded, and observations were made. The children were immersed in their daily routines and free play in centers. I observed developmentally appropriate materials and activities that were available to the children in the classroom. The last Sanitation Inspection was last completed on November 12, 2024, with a “Superior” classification and (2) demerits. The last Fire Inspection was conducted on February 17, 2025. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. As of 2/17/25 your facility is at a 94 percent compliance rate. Medication storage, emergency medication, action plans and parent authorization to administer medications were monitored. There are no children on medications currently. The program does not provide transportation. Staff and Training Worksheets were completed on site today and signed by the teacher assistant Ms. Light. Today two children’s records were monitored according to DCDEE procedures. A Fire Drill was documented on January 17, 2025. A Lock Down /Shelter-In-Place Drill was documented on January 28, 2025. The following violations were cited during the visit and technical assistance was offered. Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child's file did not have a physical within 30 days of enrollment date. GS110-91(1) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. One child did not have a copy of their immunizations on file within 30 days of enrollment. 10A NCAC 09 .0302(d)(2) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Upon my arrival, I was greeted by the lead teacher, Ms. Michelle Wyatt, who assisted me during my visit. Today attendance was recorded, and observations were made. The children were immersed in their daily routines and free play in centers. I observed developmentally appropriate materials and activities that were available to the children in the classroom. A Sanitation Inspection was last completed on January 17, 2024, with a “Superior” classification and (0) demerits. The center’s last Fire Inspection was conducted on April 6, 2023. All indoor and outdoor areas were observed during the visit. Storage of hazardous items was monitored. Medication storage, emergency medication, action plans and parent authorization to administer medications were monitored. There are no children on medications currently. The program does not provide transportation. Staff and Training Worksheets were emailed prior to today’s visit and returned for review. Staff records were monitored per DCDEE procedures. A Lock Down /Shelter-In-Place Drill was documented on January 24, 2024. A Playground Inspection was documented on February 29, 2024. The following violations were cited during the visit: Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One child did not have a signed form by the parent acknowledging receipt of North Carolina Child Care Law's. GS 110-102 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not have first aid with-in 90 days of hire. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Technical Assistance: The substitute teacher was unaware of several rules for both staff and children because she is in a temporary position. I have discussed with her some of these rules today. Rated License Information: This facilities Rated License Reassessment is resuming on July 1, 2024. During today’s visit I spoke with the substitute teacher Michelle Wyatt, and she stated that a new teacher is coming around the end of March. Due to this change in staff, they have chosen to wait until the new teacher arrives and her education is uploaded to “Works”. I will go through and review everything with them after summer. Today, I left the ERS assessment request with you. Once you are ready to have the ERS assessments completed, please complete the form, and return it to me by e-mail. Be mindful that NCRLAP has asked that any schools that are closed during the summer should send their request no later than March 29, 2024, to be evaluated before the close of summer. COVID-19 INFORMATION: If you, your staff, or the children you care for should test positive for COVID-19, you are required to be out for five (5) days and upon return wear a mask for an additional five (5) days. GS 110-102 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not have first aid with-in 90 days of hire. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff did not have CPR with-in 90 days of hire. .1102(d) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Two children did not have their medicals in their files. GS110-91(1) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. One child did not have an immunization record on file. 10A NCAC 09 .0302(d)(2) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff did not have this training with-in 90 days of employment. .1102(g) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before March 14, 2024. The letter should include each item’s number of noncompliance cited, describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received by my office by March 14, 2024, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Technical Assistance: The substitute teacher was unaware of several rules for both staff and children because she is in a temporary position. I have discussed with her some of these rules today. Ms. Wyatt stated that she will pass on all of these rules with the new teacher when she starts. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. During today’s visit I spoke with the substitute teacher Michelle Wyatt, and she stated that a new teacher is coming around the end of March. Due to this change in staff, they have chosen to wait until the new teacher arrives and her education is uploaded to “Works”. I will go through and review everything with them after summer. Today, I left the ERS assessment request with you. Once you are ready to have the ERS assessments completed, please complete the form, and return it to me by e-mail. Be mindful that NCRLAP has asked that any schools that are closed during the summer should send their request no later than March 29, 2024, to be evaluated before the close of summer. COVID-19 INFORMATION: If you, your staff, or the children you care for should test positive for COVID-19, you are required to be out for five (5) days and upon return wear a mask for an additional five (5) days. The last Annual Compliance was conducted on March 21, 2023. Currently this program operates with a Four-Star License issued on May 1, 2018. Upon my arrival, I was greeted by the lead teacher, Ms. Michelle Wyatt, who assisted me during my visit. Today attendance was recorded, and observations were made. The children were immersed in their daily routines and free play in centers. I observed developmentally appropriate materials and activities that were available to the children in the classroom. A Sanitation Inspection was last completed on January 17, 2024, with a “Superior” classification and (0) demerits. The center’s last Fire Inspection was conducted on April 6, 2023. All indoor and outdoor areas were observed during the visit. Storage of hazardous items was monitored. Medication storage, emergency medication, action plans and parent authorization to administer medications were monitored. There are no children on medications currently. The program does not provide transportation. Staff and Training Worksheets were emailed prior to today’s visit and returned for review. Staff records were monitored per DCDEE procedures. Children’s records were monitored per DCDEE procedures. Program records and required postings were monitored . A Fire Drill was documented on February 29, 2024. A Lock Down /Shelter-In-Place Drill was documented on January 24, 2024. A Playground Inspection was documented on February 29, 2024.
