The last Annual Compliance visit was conducted on January 15, 2025. The facility currently operates with a Four-Star Child Care License that was issued on February 25, 2022. The last Sanitation Inspection was completed on November 24, 2025, with a "Satisfactory" classification and five (5) demerits. The last Fire Inspection was conducted on April 10, 2025, and the facility was approved for daytime care only. The last Fire Drill was conducted on December 1, 2025. The last Lock Down/ Shelter-in-Place was on December 1, 2025. The last Playground Inspection was conducted on December 1, 2025. Upon arrival, I was greeted by the lead Pre-K teacher Rachel Tillotson. Mrs. Mary the director, came down later in my visit. I stated the purpose of my visit and completed a walkthrough of the facility where attendance was documented, and observations made. The children were observed playing in centers. A checklist was used to note the requirements I monitored today. The North Carolina Department of Secretary’s website was used to check the status of the corporation (Mary Myers Daycare Inc.) on December 14, 2025. The facility’s owner is current/active. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. This facility does not transport children. The following violations were cited and technical assistance was offered. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Ms. M. Dietz did not complete this training within 90 days of hire date. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Ms. Dietz did not complete this training within 90 days of hire date. .1102(d) 1314 Emergency information did not name child's health care professional. One child's application did not have the the name and contact number of the child's pediatrician or hospital preference listed. .0802(c)(2) 1757 A valid qualification letter was not on file and available to review at the facility. Ms. Dietz qualification letter expired on 10/5/25. G.S. 110-90.2(b) & (d) & .2703(e) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Medical information was kept in the main staff files. .0701(d) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before 5:00 PM December 29, 2025. The letter should include: Each item number of noncompliance cited. Describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received to my office by 5:00 PM December 29, 2025, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. Technical Assistance: Today we discussed that all criminal record checks must not expire. Ms. M. Dietz criminal background check expired on 10/5/25. Since this is a renewal of her background check she has 15 days from today’s date to receive a new qualification letter from the Division. If one is not received by the end of 15 days from today Ms. Dietz may not return until one is acquired. When a qualification letter is received I must have a copy emailed to me and a copy must be printed and placed in her file. Ms. Dietz did not receive first aid and CPR training within the first 90 days of hire date. Ms. Lindsay stated that she is scheduled to take first aid and CPR training tonight 12/16/25. For your correction letter I will need a copy of these training’s. We also discussed that all medical information cannot be in the main staff file. HIPPA laws state that all medical information must be in a separate file. The following items must be kept in a separate: Staff medical, TB test or TB screening, and Health Questionnaire. All children’s applications upon entering the facility must have the child’s pediatricians’ name and contact number listed as well as the hospital of choice should an emergency arise. Pathways To The Stars: Today we discussed that you should begin the process of completing your CQI plan for the facility and each staff member will begin their individual improvement plan. We talked about possibly having an outreach assessment conducted to see what areas of improvement are needed before taking the Rated License Assessment. We discussed what choices they would make for the Family Engagement process. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. This facility currently operates with a Five-Star Child Care License that was issued on May 15, 2018. The last Sanitation Inspection was conducted on October 22, 2024, and received a “Superior” classification and received seventeen (17) demerits. The last Fire Inspection was conducted on April 10, 2025, and the facility was approved for daytime care only. The last Fire Drill was conducted on April 2, 2025. The last Lock Down/ Shelter-in-Place was on April 2, 2025. The last Playground Inspection was conducted on April 2, 2025. Upon arrival, I was greeted by the director, Mary Myers. I stated the purpose of my visit, and I walked through the facility where attendance was recorded, and observations were made. The children threes and fours were outside playing, and the twos were inside playing in centers. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. The following violations were observed during today’s visit and technical assistance was offered. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. On arrival the Pre-K classroom was outside and were over ratio by five children. The ages in that group were 3 and 4's. GS 110-91(7);.0713(a-d) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. D. Lell did not have a signed copy of shaken baby syndrome training in her file. .0608(d)(1-4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. C. Tillotson did not have a copy of her recognizing and responding to suspicions of maltreatment in her file. .1102(g) The above violations must be corrected immediately. Technical Assistance: Today we discussed that for the children’s safety ratios must be always maintained. Today the teacher was very overwhelmed in the room with too many children to manage by herself. The lead teacher was on vacation. When the staff is out on vacation, plans need to be put in place for coverage in that class. When ratios are not met this causes stress on the children and staff and decreases the quality of care for the children. Staff must complete health and safety training within one year of the hire date. Recognizing and responding must be completed within 90 days of hire. Shaken baby Syndrome training must be completed on the first day of hire. Reminder: D. Lell will need to renew her first aid and CPR training before September of 2025. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. UPDATES: -Current Rated Licenses are still in a ‘hold harmless’ state until June 2024. It has been determined that your facility is in Cohort 2. This means your prep year is from July 1, 2024, through June 30, 2025. Your reassessment year is from July 1, 2025, through June 30, 2026. Please make sure that all your staff’s education is in “Works” located on the Division of Child Development website at //ncchildcare.ncdhhs.gov/. During your preparation year you can request the Environmental Rating Scales and you determine how and when to use the scores. On December 28, 2023, the Raleigh office received a report regarding concerns with the facility’s sick policy. Child Care Consultant, Mrs. Anita Graham assisted me during today’s visit. Upon arrival at the facility, we were greeted by a staff member. The director, Mary Myers soon arrived to assist with the visit. I discussed the allegation with the director. Today there were (14) children present, ages two through four years. Limited monitoring was conducted during today's visit, permit restrictions, adequate/approved space, supervision, staff/child ratio. A sampling of children’s records and program records. Allegation #1: There are concerns regarding the sick policy not being followed. Observation #1: During today’s visit we conducted a walkthrough of the facility. I interviewed the two staff members and the director. I reviewed a copy of the sick policy and asked if a copy of the policy was given to the families at the time of enrollment. We reviewed a sampling of children’s records each had a signed copy of the center’s sick policy. The director stated that their sick policy is given at the time of enrollment and posted on the front entrance. Conclusion #1: Based on conversations with the director, staff and a review of children’s and program records, this allegation is unsubstantiated. The following violations were observed, cited, and technical assistance was provided. Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Three children's files did not have a medical. GS110-91(1) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. This facility currently operates with a Four-Star Child Care License that was issued on February 25, 2022. The last Sanitation Inspection was conducted on September 27, 2022, and received a “Satisfactory” classification with (0) demerits. The last Fire Inspection was conducted on May 24, 2023, and the facility was approved for daytime care only. The last Fire Drill was conducted on July 11, 2023. The last Lock Down/Shelter-in-Place was on July 11, 2023. The last Playground Inspection was conducted on July 11, 2023. Upon arrival, I was greeted by the director, Mary Myers. I stated the purpose of my visit and was assisted on a walk-through of the facility, where attendance was documented, and observations made. The children were observed participating in circle time with their teacher. The children were counting and answering open ended questions from their teacher. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. The following violations were observed during today’s visit. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. The pre-k teacher had 13 children, ages three and four year old's without an assistant.
