The last annual compliance visit was conducted on 7/17/25. The facility currently operates with a Four-Star Child Care License that was issued on July 9, 2024. The last Sanitation Inspection was completed on March 31, 2026, with a "Superior" classification and five (5) demerits. The last Fire Inspection was conducted on July 10, 2025, and the facility was approved for daytime care only. Ms. Lisa stated that she has contacted the fire inspector and they are scheduled to come out on July 1, 2026. Ms. Lisa will email me a copy of the new inspection once it occurs within 5 days. Upon arrival, I was greeted by the director Lisa Chaffin. I stated the purpose of my visit and was assisted on a walkthrough of the facility where attendance was documented, and observations made. The children in the preschool classroom were observed participating in centers and art, and the toddler class were playing outside. On June 10, 2026, the North Carolina Secretary of State website was used to check the corporation status of “Kidz Dreamz Klub”, and it was found to be current and active. The last Fire Drill was conducted on June 4, 2026. Today we discussed conducting one fire drill during naptime to document how long it took your teachers to evacuate the building. We spoke about making sure teachers are doing a full sweep of the classrooms prior to leaving the building and recording how much time and what if any issues arose and how you worked through the issues. Teachers should always take attendance once they are outside and when they get back inside during practice drills. Taking their clipboard with them outside and having all emergency contacts is another best practice. If the building should catch on fire and you cannot go back inside the center you will need emergency contact number to call the parents. You may also want to let the parents know that a practice evacuation during nap time is going to occur. The last Lock Down/ Shelter-in-Place was on June 4, 2026. Today we discussed because your facility is near a courthouse that lockdown should be practiced and document all procedures and any issues that came about during the process. Practicing these types of drills ahead of time gets the teachers prepared and children in the event of an emergency as well as being able to work through any issues that arise during the drill. The last Playground Inspection was conducted on June 4, 2026. During summer temperatures can rise in the afternoons very quickly along with storms. Please have teachers check the playgrounds for snakes around play equipment and watch for equipment that is hot to the touch. When children are outside, please make sure that children have access to water and keep children extra hydrated. During today's visit, I completed a full assessment of Applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. This facility does not provide transportation. Today two (2) children’s files were monitored for compliance. This program completed a water testing through “Clean Classrooms for Carolina Kids”, and the final report was emailed to the facility on July 25, 2025. Rule 15A NCAC 18A .2816 of the North Carolina Child Care Sanitation Guidelines covering Lead Poisoning Hazards in Child Care Centers In 2019, North Carolina enacted 15ANCAC 18A .2816, requiring all licensed child care centers to test for lead in water at every tap used for drinking or cooking. Testing is required to obtain a license and is required at least once every three years. If levels of lead at a tap are at or above the state hazard level, centers are required to restrict access to the tap and take additional actions to ensure children and staff have access to water that does not exceed state hazard levels. The following violations were observed during today’s visit and technical assistance was offered to maintain compliance. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. All staff were not in the ABCMS system and new staff were not recorded within 5 days of hire date. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately. A corrective action letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter should include each item’s number of noncompliance cited. Describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter must be received by my office on, or before June 23, 2026, and before 5:00 PM. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov . If a compliance letter is not received by my office by June 23, 2026, before 5:00 PM, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Technical Assistance: The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Please make sure this is renewed prior to expiration date. RATED LICENSE ASSESSMENT: Consultation: Today we reviewed the documents and requirements to complete the renewal of the facilities license. Today we discussed with the director, Ms. Lisa, the following items: The program meets all enhanced ratios and space currently. The owners have agreed to meet the new requirements for a 5-star level. This includes the reduced enhanced ratios requirement. The facility is using an approved curriculum by the Division “Creative Curriculum” for all ages and is using “Teaching Strategies” as a formal assessment. The center has an eighteen-month compliance history score of 97% prior to today’s visit. The facility has chosen to meet the 5-star requirement in Family and Community Engagement and Foundational Practices by committing to the following practices: Family and Community Engagement: Two-way communication with families. “Two-way communication” means that the child care center shares information with families about their child(ren) and that the families may share information with the center about their child(ren) in a manner that is available and accessible to the family. This facility offers a family conference twice a year to discuss their children’s needs and progress towards individual goals and to receive feedback from families about their experiences at home and at the child care center. Parents are offered an opportunity to set goals for their child. These conferences can be conducted virtually, on the phone or in person. Each parent conference must be documented with date, topics discussed for their child’s goals and signed by the parent. The formal assessment must be attached to the parent conference form twice a year. The purpose of the formal assessment is to show all areas of growth over time; therefore, best practices would be to conduct the assessment upon enrollment and at the end of the year. This information will be most helpful to the child’s teacher to use for planning and working on individual goals throughout the year. The child care center offers at least two-family events. Educational Opportunity: The center administrator provides paid planning time for lead teachers to prepare information for family and teacher conferences, time to conduct the conferences and supports needed to conduct conferences by telephone, virtually or in-person. Today I reviewed your completed CQI plan for your facility and the Individual plans for lead teachers. This is an ongoing plan over the course of the year or until the goal has been achieved. Once this is achieved a new goal will be established. All facility and Individual goals will be reviewed for compliance during my next visit as well as coaching /training requirements for administrators and lead teachers. I have left an information sheet explaining these requirements with you during today’s visit. Education Standards: Lead Teacher- 5-star level Other staff- 5-star level Final Star Level in Education- 5-Star level The correction of all violations is required before we can renew your license. The permission form did not include a start and end date on it. Another medical action plan was not legible and a new one needs to be completed by the parent. 10A NCAC 09 .0803(4)(6-9) The above violations must be corrected immediately. This facility has earned 6 points in program standards. This facility has earned 3 points in education. One quality point is earned by the following: All fulltime lead teachers and teachers must complete 20 in-service training hours in addition to required annual hours. Part time have completed additional hours based on pro-rated annual hours. The last Sanitation Inspection was completed on February 29, 2024, with a "Superior" classification and (9) demerits. The last Fire Inspection was conducted on July 31, 2023, and the facility was approved for daytime care only. On August 14, 2024, the North Carolina Secretary of State website was used to check the corporation status of “Kidz Dreamz Klub”, and it was found to be current and active. Upon arrival, I was greeted by the cook, J. Williams, and the owner, Natalie Akashambatwa came 15 minutes later. The director Debra Hicks was teaching in the preschool classroom. I stated the purpose of my visit and was assisted on a walk-through of the facility where attendance was documented, and observations made. The children in the preschool classroom were observed participating in centers and the toddler class was preparing to go outside. During today's visit, I completed a full assessment of Applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. This facility does not provide transportation. The following violations were observed during today’s visit and technical assistance was offered. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. Technical Assistance: A fire inspection must be obtained annually and not lapse. This report must be emailed to your state consultant within one week of receiving the inspection. The last fire inspection was on July 30, 2023. The owner Mrs. Natalie called me on 8/19/24 to explain that a technician accidentally damaged their fire panel while servicing it. The company is in the process of finding a replacement panel. The fire chief Ms. Ryggs of Lexington city has allowed them to operate temporarily by completing a fire watch every hour and documenting it until the panel can be replaced. Today I monitored the recording of these checks on the hour. There are manual smoke detectors that are present in the facility in the event of a fire, but they have been made aware that they will need to call 911 in the event of a fire. Ms. Natalie will contact the fire department as soon as a replacement panel is found and installed to have a passed fire inspection completed as soon as possible. All in-service training required hours must be met annually. It is suggested that training be curtailed to the staff’s position. For example: if the teacher works with preschool children could take age-appropriate activities for preschool age children. All in-service hours that exceed the required hours only half of the required hours can be carried over for the following year. Reminders: Two staff members first aid and CPR training will need to be renewed in November 2024. Today will be their first Annual Compliance visit. The facility currently operates with a Three-Star Child Care License that was issued on May 30, 2023. The last Sanitation Inspection was completed on September 26, 2022, with a "Superior" classification and 13 demerits. The last Fire Inspection was conducted on July 31, 2023, and the facility was approved for daytime care only. On August 30, 2023 the North Carolina Secretary of State website was used to check the corporation status of this facility and it was found to be current and active. The last Fire Drill was conducted on June 14, 2023. The last Lock Down/ Shelter-in-Place was on April 19, 2023. The last Playground Inspection was conducted on August 28, 2023. Upon arrival, I was greeted by the director, Natalie Akashambatwa. I stated the purpose of my visit and was assisted on a walk-through of the facility where attendance was documented, and observations made. The children were observed participating in circle time and the toddler class was preparing to go outside. During today's visit, I completed a full assessment of Applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. The following violations were observed during today’s visit. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The last fire drill was completed on June 14, 2023. .0604(t); .0302(d)(5) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Four staff members did not have a medical prior to their start date. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff member had a TB test that was older than 12 months. Hire date was on June 6, 2023 and TB test was dated April 29, 2022. .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks.
