The last Annual Compliance visit was conducted on July 18, 2022. The facility currently operates with a Three-Star Child Care License that was issued on August 6, 2019. The last Sanitation Inspection was completed on April 20, 2023, with a "Superior" classification. The last Fire Inspection was conducted on February 23, 2023, and the facility was approved for daytime care only. The North Carolina State of Secretary website was checked on July 3, 2023 and found to be current and active. The last Fire Drill was conducted on June 12, 2023. The last Lock Down/ Shelter-in-Place was on June 12, 2023. The last Playground Inspection was conducted on June 15, 2023. Upon arrival, I was greeted by the director, Shebria Weavil. I stated the purpose of my visit and was assisted on a walk-through of the facility where attendance was documented, and observations made. The children were observed participating in circle time and indoor free play. During today's visit, I completed a full assessment of Applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections, and transportation. Today 3 children's files were monitored and found to be in compliance. The following violations were observed during today’s visit. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The last menu posted was for June. 10A NCAC 09 .0901(b) 607 Separate lavatories with hot and cold running water, soap, and individual sanitary towels or other approved hand-drying devices were not available for staff in food preparation areas or diaper changing areas. There were no hand drying or paper towels available in the boys or girls bathrooms. 15A NCAC 18A .2818(b) & (d) 615 Beds, cots and mats were not in good repair, properly handled, stored, or clean and sanitized between users. The sheets were stained and visibly dirty. 15A NCAC 18A .2821(a) 619 Diaper changing surfaces were not cleaned with a detergent solution and disinfected after each use. Diaper changing table was not cleaned and sanitized after each use. The diaper changing table was full of crumbs and dirt. 15A NCAC 18A .2819(c) 847 Parent's medication authorization did not include required information. A diaper cream did not have a starting date or ending date on the permission form. 10A NCAC 09 .0803(4)(6-9) 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. There was no fire extinguisher mounted in the van that transports children. 10A NCAC 09 .1003(c) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before July 21, 2023, and before 5:00 PM. The letter should include each item’s number of noncompliance cited, describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov . If a compliance letter is not received by my office by July 21, 2023, before 5:00 PM, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. Comments: Mrs. Shebria stated that the van did not have a fire extinguisher in the van because they had used the van this last weekend for a beach trip with her family. However, there must be a mounted fire extinguisher in the van at all times. Please make sure to mount your fire extinguisher before transporting children in the van. Today upon arrival the infant classroom sheets were stained and very dirty. Sheets must be cleaned once a week or whenever a sheet becomes soiled. The diaper changing table was not cleaned and sanitized after each child was changed. The diaper table was full of crumbs and had not been properly sanitized. One diaper cream had no expiration dates on the permission form. Please make sure that all diaper creams are signed and dated by the parent before applying it on a child. Each bathroom must have either paper towels or a hand dryer for children to dry their hands after washing them. The boys and girl’s bathrooms did not have anything to dry their hands on. There was no current food menu for this month posted. The last one posted was for June. Please contact Marcia Humphrey for the new playground area that has been constructed located on the side of the building next to the parking lot. A current menu must be posted for the parents to see. I would place the menu at the front door area so they can be seen. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to always maintain compliance with all the applicable laws and rules. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ on the home page under the ‘Services’ tab. UPDATES: -CBC qualifying letters are now valid for five (5) years regardless of the date of issue. New letters will not be sent out. -Current Rated Licenses are still in a ‘hold harmless’ state until June 2024. It has been determined that your facility will be in Cohort 3. This means that your preparation year will be from July 1,2025-June 30, 2026. Your reassessment year will be from July 1, 2026, to June 30, 2027.
