Name of Operation: ELIZABETH'S PLAYHOUSE Facility ID: 29000369 Consultant: RENA MCCAFFREY Operation Type: Family CC Home Case Number: Visit Date: 4/1/2026 Number Present: 3 Completed Date: 4/1/2026 Age: From 3 To 5 Total Minutes: 100 Time In: 09:05 AM Time Out: 10:45 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Rated License Assessment Announced/Unannounced: Unannounced The purpose of today's visit was to complete the provider's renewal of the three-year rated license reassessment. Upon my arrival at the Family Child Care Home, I was greeted by the provider, Elizabeth Ray. I stated the purpose of my visit and conducted a walkthrough of the home and playground area. Today children were playing in the front living area of the home. The living room continues to be the primary daycare space. It contains activity areas, a diaper change table, and information required to be posted. The children also use the hall bathroom as well as the room in the basement which is set up with several appropriate age toys. First aid supplies are stored in the bathroom. The children play in an outdoor area that is fenced separately from the operator's pets. This area contains developmentally appropriate toys and materials for use by children. The operator's last annual compliance visit was conducted on 8/20/25. The facility currently operates with a four (4) star license that was issued on November 2, 2017. The last Fire Inspection was conducted on 8/21/25 and was approved for daytime care only. The following were monitored: Supervision, staff child ratio, CPR/First Aid training, specialized training, storage of medications, adequate and approved space, program records, and the license was posted with restrictions met. The center has an eighteen-month compliance history score of 100% prior to today’s visit. The following violation was cited during today’s visit and technical assistance was offered to maintain compliance. Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The provider needs to enter everyone in the household into the ABCMS system. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before 5:00 pm April 15, 2026. The letter should include each item’s number of noncompliance cited, describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov . If a compliance letter is not received in my office by 5:00 PM April 15, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violations may be documented if each violation has not been properly corrected or otherwise addressed. Technical Assistance: As stated in G.S. 110-90.2 & .2703(r) childcare operators are to notify the Division of any new childcare providers working who were hired or moved into the childcare facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Should you need assistance please contact the Criminal Background Check Unit at (919) 814-6401 and someone will assist you. RATED LICENCE ASSESSMENT Consultation: Today I met with Mrs. Elizabeth to answer any questions she may have about the “Pathways to the Stars” process and to review the requirements to meet all standards for 5-star license. Mrs. Elizabeth has chosen pathway two. The programs current restrictions are as follows: serves no more than 2 infants less than 1 yr. old. weekend transportation cares for EC up to age 18. To meet the requirements for a 5-star standard you must adhere to the following restrictions. No more than 4 children under the age of 24 months of the children who are birth to 5 years of age when meeting NCGS110-91 (7)(b)(1). Educational Standards Mrs. Elizabeth has an associate’s degree in EC/CD and 28 years’ experience this meets the 5-star standards in education. The FCCH has chosen to meet the 5-star requirement in Family and Community Engagement and Foundational Practices by committing to the following practices: Family and Community Engagement This facility communicates with their families through DOJ; this allows families to communicate any information necessary with their child’s caregiver. Communication Option FCCH will offer a family conference twice a year to discuss their children’s needs and progress towards individual goals and to receive feedback from families about their experiences at home and at the child care center. Parents are offered an opportunity to set goals for their child. These conferences can be conducted virtually, on the phone or in person. These parent/ caregiver conferences will need to be documented each time and parents will need to sign and date when the conference occurs. The child’s formal assessment should be attached to the Child Care Consultant to review. Engagement and Leadership The facility offers an enrollment orientation that includes an opportunity for families and children to visit their child’s teacher and classroom prior to the first day of enrollment. Educational Opportunity The facility will offer an educational opportunity annually for parents and teachers to learn together. Today I reviewed your progress of the FCCH Operator CQI plan and Professional Development Plan. You have completed questions 1-4 to get started. This is an ongoing plan over the course of the year or until the goal has been achieved. Once this is achieved a new goal will be established. This FCCH is using an approved curriculum (Creative Curriculum and using Teaching Strategies) as a formative assessment tool, and the operator will maintain 5 hours annually of coaching or training. Today I left an information sheet on requirements and how to meet these standards. It appears that this facility has met all standards for a Five-Star license once the violation has been corrected. Thank you for your assistance during today’s visit. If you have further questions or concerns, I can be reached at rena.mccaffrey@dhhs.nc.gov or by calling; 336-580-5670. You may also contact my supervisor, Kaye Adkins at kaye.adkins@dhhs.nc.gov. Thank you for all that you do to serve the children and families of North Carolina. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000). All information in this report has been reviewed with me today.I understand that it is my responsibility to maintaincompliance with applicable NC Child Care Requirements at all times
