Name of Operation: CHILD'S DAY CARE HOME Facility ID: 29000455 Consultant: RENA MCCAFFREY Operation Type: Family CC Home Case Number: Visit Date: 3/31/2026 Number Present: 5 Completed Date: 3/31/2026 Age: From 2 To 5 Total Minutes: 88 Time In: 09:22 AM Time Out: 10:50 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to conduct an Annual Compliance visit and to monitor compliance with Applicable Childcare Requirements. This Family Child Care Home currently operates with a One-Star Child Care license that was renewed on January 14, 2026. The last Fire Inspection was conducted on June 11, 2025, and the home was approved for daytime care only. During today’s visit I was greeted by the owner, Layne Phifer, and she assisted me with my visit. The basement floor area of this home is the primary care space used for children. Upon my arrival there were five (5) children present, no school-age children were present during today’s visit. The children were practicing writing their letters. Today, I observed that developmentally appropriate materials and activities were available to the children in the home and on the playground. Outside the children have bikes, balls and a basketball net to play with. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, and programs records were monitored, Emergency Drills, Medication, and Playground Inspections. There is no one on medication currently and this program does not provide transportation. The FCCH and Centers Located in a Residence Verification of Required Information for Operator and Additional Caregivers and FCCH and Centers Located in A Residence Operator’s Statements of Responsibility were monitored by DCDEE procedures. The license was posted, and restrictions were met. Today the pets were up to date on their rabies vaccinations. During today’s visit the ABCMS was monitored for compliance as well as criminal background on the provider and husband who reside in the home. Today (2) children’s files were monitored according to DCDEE procedures. Program records and required postings were monitored and found to be in compliance. The last Fire Drill was documented on March 4, 2026. The last Lock-Down/Shelter-in-Place was documented on January 6, 2026. The last Playground Inspection was documented on March 5, 2026. Today the following violations were cited, and technical assistance was offered to maintain compliance. Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. This facility was not in the ABCMS system. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before 5:00 pm April 14, 2026. The letter should include each item’s number of noncompliance cited, describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov . If a compliance letter is not received in my office by 5:00 PM April 14, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violations may be documented if each violation has not been properly corrected or otherwise addressed. Technical Assistance: The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Reminders: Your fire inspection will be coming up for renewal on June 11, 2026. You were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to always maintain compliance with all the applicable laws and rules. UPDATES: Checkout our “What’s New” at DCDEE website https://ncchildcare.ncdhhs.gov . As measles cases continue to rise across the country, NCDHHS is taking proactive steps to ensure the state remains prepared for any potential cases or outbreaks. In the United States, measles cases surged from 59 cases in 2023 to 285 cases in 2024. As of March 27, 2025, a total of 483 confirmed measles cases were reported by 20 jurisdictions. Measles is a highly contagious viral disease that can lead to serious health complications, especially in babies and young children. While there have been no reported cases of measles in North Carolina so far in 2025, public health officials are urging residents, health care providers and child care centers to take the necessary precautions to protect themselves, their communities and those at highest risk, especially unvaccinated children. NCDHHS strongly urges all residents to ensure they are up to date on the MMR (measles, mumps, rubella) vaccine. Children should receive the first dose of the vaccine at 12-15 months and the second dose at 4-6 years of age. Adults who have not been vaccinated or are unsure of their status should consult their health care provider. Please see the CDC’s measles FAQ and vaccinations website for more information.
