    Name of Operation: Blossoming Minds Facility ID: 29000774 Consultant: RENA MCCAFFREY Operation Type: Center Case Number: Visit Date: 2/18/2026 Number Present: 23 Completed Date: 2/18/2026 Age: From 0 To 5 Total Minutes: 190 Time In: 09:35 AM Time Out: 12:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of my unannounced visit was to monitor for compliance with applicable childcare requirements. This is their first Annual Compliance visit. The facility currently operates with a Four-Star Child Care License that was issued on September 22, 2025. The last Sanitation inspection was conducted 10/27/25 with a ‘Superior’ classification and zero (0) demerits. The last Fire inspection was conducted on 2/19/25 and was approved for day-time care only. The facility does not transport children currently. The last Fire Drill was conducted on January 30, 2026. The last Lock Down/ Shelter-in-Place was on January 30, 2026. Playground Inspection was conducted on January 30, 2026. Upon arrival, I was greeted by the lead teacher Melanie Biaso. The director, Zylie Smith, came later in the visit. I stated the purpose of my visit, and I completed a walk-through of the classroom where attendance was documented, and observations were made. The children were observed playing outside on the playgrounds. During today's visit, I completed a full assessment of Applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. The Restrictions on this facility are as follows: The building capacity is 45 children serving ages 0-12. 1st shift only. This facility is meeting enhanced ratios. All employees’ files were monitored for compliance today as well as three (3) children’s files. The Department of the Secretary’s website was used to check the status of the corporation (Blossoming Minds Learning Center LLC) on February 15, 2026. This facility is current/active. Compliance History: As of 2/15/26 your compliance history is 100 percent. The following violations were cited today, and technical assistance was offered to maintain compliance. Violation Number Comment Rule 718 Swings did not have surfacing that extends two times the length of the pivot point to the surface below beyond the external limits of the equipment. The swings did not have enough mulch under them to meet this requirement. .0605(m)(2) 721 All equipment and furnishings were not in good repair. There is a small plastic climber that is bowing in the center and coming apart. G.S. 110-91(6); .0601(b) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. A child in the two's did not have a signed permission form to administer Desitin diaper cream. 10A NCAC 09 .0803(1)(a & b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Ms. R. Bowman did not have a medical on file. 10A NCAC 09 .0701(a) 1314 Emergency information did not name child's health care professional. One child did not have a choice of hospital in the event of an emergency on file. .0802(c)(2) 1907 A copy of the Prevention of Shaken Baby and Abusive Head Trauma policy was not given to or explained to parents of newly enrolled children up to five years of age on or before the first day the child receives care at the center. One child did not have a signed copy of this requirement on file. .0608(b) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before March 3, 2026. The letter should include: Each item number of noncompliance cited. Describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received at my office by 5:00 pm March 3, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. You and your staff were provided with an opportunity to ask questions regarding childcare requirements. Technical Assistance: During today’s visit we discussed the following: Your last fire inspection was conducted on February 19, 2025. As of tomorrow, I will need a copy of a passed inspection. The last fire inspection was conducted on 5/2/25 and had a violation that needed to be corrected. Mrs. Zylie spoke with fire inspector today and he will be out here first thing tomorrow morning. She will email me a copy of the inspection tomorrow once it’s completed. Two new employees, A. Hancock and K. Caldwell, will also need to be added to the staff and training worksheet. The staff and training worksheets will need to be updated with in-service training hours totaled and signed and dated by Mrs. Zylie. Please return this to me by 2/20/26 before 5:00 pm. Playground areas can have many hazards around them. This is why it’s very important to maintain mulch around fall zones like swings and at the end of slides. Today the mulch needs to be replaced or raked back into these areas. One climber was bowing and the steps were coming apart from the climber. This will need to either be repaired or removed. All staff must have a physical prior to their starting date. R. Bowman did not have a medical on file. All diaper creams must have permission to administer and must be signed and dated by the parent. One child in the two’s the parent did not sign the permission form. REMINDERS: T. Salinas will need to have her qualification renewed before 10/15/26. Please make sure tis is completed in time for her letter to be processed. Every staff member must have a printed copy of a qualification letter on file. J. Poole – First aid and CPR training will need to be renewed before 11/26. During today’s visit I reviewed individual staff’s CQI plans as well as the facility CQI plans.
