Elaine Leonard, owner/operator, was present and assisted me during the visit. Only one child was present during today’s visit and was playing in centers. The operator's last annual compliance visit was conducted on 9/9/25. The facility currently operates with a four (4) star license that was issued on 10/11/16. The home's last fire inspection was conducted on 8/21/25. During today's visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were cited, and technical assistance was offered to maintain compliance. Violation Number Comment Rule 1846 An individual over the age of 15, who resides in a family child care home or a center located in a residence on a permanent or temporary basis, or those who had their 16th birthday after the initial licensing of a family child care home; did not complete and submit required forms within five business days. A foreign exchange student turned 16 years of age but could not obtain a criminal background check here in the US due to not having a social security number and only having a visa to be here. G.S. 110-90.2 & .2703(q) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received to my office on, or before 5:00 pm, March 17, 2026. A Corrective Action Letter must include the following items: • The name of your center • Facility ID number • Each violation numbers • How you corrected each violation • Title of the person who signs the letter • Date Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov . If a compliance letter is not received at my office by 5:00 pm, March 17, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violations may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. Technical Assistance: The foreign exchange student from Spain that temporarily lives with Ms. Leonard turned 16 years of age on February 5, 2026. I explained on my last visit once she turned 16 that she would have to obtain a criminal background check to remain at home. Ms. Elaine contacted me and criminal background checks to see how this process could work for this student because she already has a Visa, and she cannot obtain a social security number to process a criminal background check, however the agency “Greenheart” contacted me on 3/2/26 and said that all students have background checks completed before they can come to their foreign exchange home to stay. This student is leaving June 4, 2026, once her school leaves for summer. Mrs. Elaine will need to provide all documentation for the correction of this violation. If a criminal background check can be completed through “Greenheart” then we will need a copy of this and proof of her visa to be here. Any documentation you can provide will be helpful in resolving this violation. During today’s visit I reviewed her documentation that was present at the house. Kasandra (Foreign exchange student) had medical and immunization records prior to coming to the US. I monitored a copy of her passport which was issued on 1/7/22 and will expire on 1/7/27. Mrs. Elaine stated that she will contact “Green Heart” agency to get a copy of her criminal background check from "Green Heart" and proof of her visa. Mrs. Elaine also stated that back on August 26, 2025, she called the Raleigh criminal background check and explained her situation to the person she spoke with, and they told her that all she had to do was to use her address for her criminal background check. They did not mention anything regarding needing a social security number for Kassandra. In the future, please make sure you read and are knowledgeable of all rules and regulations before accepting a foreign student to live with you. Mrs. Elaine stated that at this time she does not have any children on second and third shift that Kassandra would encounter with. Kassandra leaves at 7:00 am and does not return until 5:30 pm each night because she participates in track each night so she doesn’t come in contact with the children in the daycare. RATED LICENCE ASSESSMENT: Today we discussed the Family and Community The facility has chosen to maintain 5-star level in Family and Community Engagement and Foundational Practices. This facility offers a family conference twice a year to discuss their child’s needs and progress towards individual goals and to receive feedback from families about their experiences at home and in the family child Care home. Engagement and Leadership-The operator offers an enrollment orientation that includes an opportunity for families and children to visit the family child care home prior to the first day of enrollment. Educational Opportunity- The facility will offer one educational opportunity for parents and Mrs. Elaine to learn together. Today I reviewed the process of the family child care CQI plan and Professional Development Plan. You will need to complete questions 1-4 to get started. This is an ongoing plan over the course of the year or until the goal has been achieved. At this time a new goal will be established. This facility is using an approved curriculum- Creative Curriculum and for formal assessment Ms. Elain will choose a formal assessment and let me know her choice by email. The formal assessment will be used during a parent teacher conference with the parents. They will discuss the progress of each child’s progress. NC General Statute 110-90(4)(d) all child care centers must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your center may have an impact on the total compliance history score and cause your score to drop below the mandated level. Should this occur, your center may not be eligible for an enhanced rated license at the end of your current license and will be at risk of being issued a renewal license. This facility’s compliance history was checked on 3/3/26 and was 99 % in compliance. Staff Education: The operator has an AAS degree in Early Childhood Education. She qualifies for seven points in Education. Program Standards: The operator also has approved policies and procedures. The operator qualifies for 5 points in Program Standards. Quality Point: The operator does not enroll more than four children under 24 months of the children who are birth to 5 years of age when meeting NCGS 110-91 (7) (B) (1) The operator is in the process of completing the CQI process. The last annual compliance was conducted on October 1, 2024. This family child care home currently operates with a Four-Star Child Care License that was issued on September 11, 2019. The last Fire Inspection was conducted on August 21, 2025, and was approved for first, second and third shift. Permit Restrictions: Your capacity is (8) children per shift. No more than (5) Preschool age children on any shift. You can serve no more than two (2) infants under age one. You can only serve children ages 0-12. The last Fire Drill was conducted on August 15, 2025. The last Lock Down/ Shelter-in-Place was on May 21, 2025. The last Playground Inspection was conducted on August 15, 2025. Upon arrival, I was greeted by the provider, Elaine Leonard. I stated the purpose of my visit and completed a walkthrough of the home, where attendance was documented, and observations made. The children were playing in centers inside the main playroom. In the home the playroom remains the primary care area and includes a variety of age-appropriate toys and materials. The outdoor play area also contained a variety of toys. The provider’s license and parental information were posted on a bulletin board in the playroom along with other required postings. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Provider information and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections and Transportation. The FCCH and Centers Located in a Residence Verification of Required Information for Operator and Additional Caregivers and FCCH and Centers Located in A Residence Operator’s Statements of Responsibility were monitored by DCDEE procedures. Today one (1) child’s record was monitored according to DCDEE procedures. Transportation is not provided, and no children were on medication. The following violations were cited, and technical assistance was offered. Violation Number Comment Rule 916 Operator did not maintain a record of on-going training in which he/she has participated.
