This visit was conducted with Melinda Lescar, Child Care Consultant and Diane Westmoreland, Operator. Mr. Westmoreland was present during the visit. Your program currently operates with a One-Star license. Restrictions include daytime, maximum of five (5) preschoolers at any time and fuel burning stove not used during operating hours. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 98 % prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. We tested the smoke detector during today’s visit, and it worked. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children in free play activities, outdoor , and lunch. I observed materials and activities that were available to the children. The last annual compliance visit was conducted on May 13, 2025. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed owner status of this facility. I have asked Diane Westmoreland who lives in the home and recorded her answer on the Verification of Required Information for Operator and Additional Caregiver Form with her signature. Be reminded that everyone living in the home must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 4/1/2026. A lock down drill was documented on 4/1/2026. An outdoor inspection was documented on 4/1/2026. There are not any pets in the home. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Medications were monitored per DCDEE procedures. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 2012 Individual who provides care for 5 hours or more a week, did not complete Recognizing and Responding to Suspicions of Child Maltreatment training. Ms. Westmoreland stated that they were locked out of their NCID accounts and are actively working with Yadkin Smart Start on getting access back into the system to complete their health and safety training. Mr. Westmoreland will need to complete the Health and Safety Trainings as well. It is recommended to sign into your NCID account once or twice a year to avoid being locked out to ensure you have access. Item # 2012- Mr. Westmoreland did not have a certificate for Recognizing and Responding to Suspicions of Child Maltreatment Training. It is recommended to review files regularly to ensure you have all the required documentation. You must notify your consultant 30 days prior to changing the ownership status of your facility. We discussed during the visit that you have a TB test but have to go get a copy. We discussed that Mr. Westmoreland would need to get a TB test and place it in his file. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • Public schools and licensed child care facilities in North Carolina are eligible to participate in the funding mechanisms to cover the costs of testing for lead in water and inspecting for asbestos and lead-based paint hazards beyond the May 1, 2024 deadline listed in 10A NCAC 41C .1002(a). Due to the recent impacts of Hurricane Helene and other natural disasters, this deadline has been revised statewide, and facilities that have not yet enrolled should complete the online process by May 31, 2025. If you have enrolled for only one or two of the program sections, log back in to complete surveys for the remaining section(s) to comply with the rule requirements. • The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. During today’s visit we discussed supervision regarding preschool age and school age children. A 4ft fence is required per local zoning. The fence must be maintained at 4ft. from the inside of the fencing. At naptime if music is required to sooth children, I encourage you to play music that encourages rest, When receiving authorization to administer medication forms please ensure parents have document the name of the diaper cream. I stated to Ms. Westmoreland I would email her the information regarding the Clean Water to complete the webinar and survey. Your TB test is required to be on file for you as the operator. If you are unable to locate, you will need to complete a new TB to have on file. At the completion of the visit, this visit summary was reviewed with and provided to you. Upon arrival, capacity was monitored as indicated on the attached worksheet. Child supervision was adequate during my time in your home. I observed positive interactions today. I observed children having indoor free play and lunch. Lunch consisted of chicken nuggets, green beans, apples, bread, and milk. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Two new infants were enrolled since your annual compliance visit in June 2023 which files I monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The only medication you accept is topical ointments such as diaper cream. Storage of hazardous items was monitored today and are stored in a locked hallway closet. All electrical outlets in the home are the safety type that do not require a cover. A shelter in place emergency drill was last documented 11/3/23 and a fire drill on 12/1/23. There are no pets at the home. The program does not transport children. The following violations were observed and cited today: Violation Number Comment Rule 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. A parent provides diaper cream for the provider to administer for an infant although written authorization completed and signed by the parent was not available for review. 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) 912 Written feeding schedule was not on file for each child under 15 months of age and/or was not updated regularly to reflect changes in the child's needs. A 15-month-old child in care today was enrolled at age 11 months. At the time of enrollment, a feeding schedule required for children under 15 months of age was not completed by the parent. .1706(i) 1808 Signed statement did not include the date the operator's safe sleep policy was given and explained to the parent/guardian. Diane Westmoreland, Owner/ Operator of the home assisted me with the visit. Your program currently operates with a one-star license. Restrictions include 1st shift care, minimum of five preschool children at any time, and fuel burning stove not used during operating hours. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS • CBC Qualification • General Health • Administering of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation The license was observed, and the restrictions were found in compliance. Mrs. Westmoreland stated the only person living in the home with her is her husband. Be reminded that you and your husband must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. The letter must not expire before a new letter is issued. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, capacity was monitored as indicated on the attached worksheet. Child supervision was adequate during my time in your home. I observed positive interactions today. I observed children having indoor free play and lunch. Lunch consisted of chicken nuggets, green beans, apples, bread, and milk. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Two new infants were enrolled since your annual compliance visit in June 2023 which files I monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The only medication you accept is topical ointments such as diaper cream. Storage of hazardous items was monitored today and are stored in a locked hallway closet. All electrical outlets in the home are the safety type that do not require a cover. A shelter in place emergency drill was last documented 11/3/23 and a fire drill on 12/1/23. There are no pets at the home. The program does not transport children.
