You, Hannah Laws, Program Coordinator, assisted with today’s visit. The facility currently operates with a 5-star rated license issued on November 6, 2023, earning seven (7) points in staff education and six (6) points in the program standards for a total of 13 points total. The program's license restrictions include the following: Daytime care only, school-age only; playground area does not meet child care playground safety standards; meets enhanced space, and meets enhanced ratios. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines, snack, outdoor gross motor play, and departure during the visit. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on February 11, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on December 11, 2025. The last emergency drill documented was conducted on December 3, 2025. A checklist was used to note the requirements I monitored today. I reviewed the ABCMS Provider portal during the visit. I confirmed all current staff members are included in your facility portal. Files Reviewed: Two (2) children’s files were reviewed during the visit. One (1) existing staff file was reviewed during the visit. The following violation was observed/documented during today’s visit: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. On Novembern21, 2025, three children were not signed out. 10A NCAC 09 .0302(d)(4) The violation(s) documented must be corrected immediately. On or before December 30, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The facility currently operates with a 5-star rated license issued on November 6, 2023, earning seven (7) points in staff education and six (6) points in the program standards. The program's license restrictions include the following: daytime care, school-age only, meets enhanced space, meets enhanced ratios, and playground area does not meet child care playground safety standards. During today’s visit, the children were observed during transitions, lunch, rest and digest, and indoor free play. A walk-through of the facility was completed today. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on March 19, 2024. Sanitation Inspection: The last sanitation inspection was completed on December 6, 2023. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills: The last shelter-in-place drill was completed on July 18, 2024, and recorded. The last fire drill was completed on July 18, 2024, at 3:30pm and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. Files Reviewed: Three (3) staff files were reviewed today. The following violations were observed during the walk through today. On Monday, June 24, 2024 and on Monday, July 22, 2024, one child was administered a prescription medication the date was not recorded at the time the medication was given. .0803(13)(a-e); .2318(3) Technical Assistance: Arrival and Departure: I recommend staff review all arrival and departure sheets daily to ensure each child has been signed in and out at the correct time. This process will ensure the safety of all children enrolled in the program and to maintain compliance with all child care rules and requirements. Medication Administration: I recommend staff review all medication administration sheets daily to ensure all required information has been documented appropriately and correctly. Consultation: Prior to today’s visit, your compliance history was ninety-six (96) percent as of August 6, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Lead Testing: Child Care Rule .0601(f)- Safe Environments Child Care Rule Effective January 1, 2024 Rule Amendment- This rule was amended to include the specific requirements that child care centers must be free of asbestos hazards. This is in addition to the existing requirement that they be free of lead poisoning hazards. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. Dinah Brown, Child Care Consultant, assisted me with today’s visit. You, Amber Waters, Program Coordinator, assisted me with the visit today. The facility currently operates with a 5-star rated license issued on November 6, 2023, earning seven (7) points in staff education and six (6) points in the program standards. The program's license restrictions include the following: daytime care, school-age only, meets enhanced space, meets enhanced ratios, and playground area does not meet child care playground safety standards. During today’s visit, the children were observed in snack time, indoor gross motor play, and indoor free play. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on September 13, 2023. Sanitation Inspection: The last sanitation inspection was completed on December 6, 2023. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last shelter in place drill was completed on January 31, 2024, at 3:30pm and recorded. The last fire drill was completed on January 30, 2024, at 3:45pm and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. Files Reviewed: Children’s files, program files, and staff files were monitored today. Three (3) children's files were reviewed during today’s visit. Two (2) staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violations were observed during the visit today. I discussed these violations with you in detail at the end of the visit. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The sanitation inspection documentation available for review was dated December 6, 2023. The prior sanitation inspection documentation available for review was dated October 20, 2022. 10A NCAC 09 .0304(b) 1898 Staff did not complete the health and safety training within one year of employment. One staff member employed on October 19, 2022, did not have verification of completion of the health and safety training within the first year of hire. .1102(a) The following Technical Assistance regarding the violations observed during today’s visit was provided: Sanitation Inspections: As a reminder, sanitation inspections must be conducted annually within the same month the last sanitation inspections was conducted. I suggested contacting the Environmental Health Inspector at least one month prior to when the last sanitation inspection was conducted. Health and Safety Trainings: As a reminder, Health and Safety training must be completed within one year of employment. I suggested using the staff file checklist to highlight important dates when training must be completed. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-six (96%). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter: You must correct the violations found during today's visit immediately.
