You, Julie Henthorne, Owner/Operator, assisted with today’s visit. Your program currently operates with a four-star license effective December 27, 2018. The permit restrictions were in compliance including 1st shift and meets enhanced ratios. You stated that you do not plan to sell your business in the near future. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines, free play, rest time, and departure during the visit. Required Inspections: The last fire inspection was conducted on February 3, 2026. The last sanitation inspection was conducted on July 8, 2025. A “superior” classification was issued with four (4) demerits noted on the grade card. The most recent playground inspection was conducted on May 3, 2026. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on July 1, 2023. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The most recent fire drill was conducted on May 4, 2026. The most recent emergency drill was conducted on November 11, 2025, and March 3, 2026. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. No new staff members have been hired since the last visit. The following violations were observed/documented during the visit: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member employed on August 1, 1997, had a First Aid Training certificate on file that expired on March 21, 2026. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member employed on August 1, 1997, had a CPR Training certificate on file that expired on March 21, 2026. .1102(d) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. An emergency drill was conducted and documented on November 11, 2025, and again on March 3, 2026. .0604(u);.0302(d)(8) The violation(s) documented must be corrected immediately. On or before May 28, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Please send your letter to dinah.brown@dhhs.nc.gov. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 93%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Technical assistance was provided on the following: Emergency Drills: Shelter-in-place or lockdown drills must be practiced within every three months. A shelter-in-place or lockdown drill must be completed by (date). I suggested that you make a note on a calendar or set a reminder on your computer to conduct emergency drills at least every three months. Shelter-in-place or lockdown drills are to be conducted within three months of the previous drill rather than once every quarter. CPR/First Aid: Staff must complete First Aid and CPR training within ninety (90) days of employment and must renew certification on or before the expiration date. I suggested that you register staff for CPR and First Aid training as soon as they are hired and at least six months prior to the certification’s expiration date. CPR and First Aid training must be completed in person by an approved agency. Verification of each staff member's completion of this course from an approved training organization shall be maintained in the staff member's file in the center. Consultation As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Upon arrival, the purpose of today’s visit was reviewed with you, Julie Henthorne, Administrator, assisted with today’s visit. Your last annual compliance visit was conducted on August 22, 2024. Your program currently operates with a four (4) star license effective December 27, 2018. The permit restrictions were in compliance including Daytime care only; Meets enhanced ratios. A walk-through of the facility was completed today, all indoor and outdoor areas used by children enrolled were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during teacher lead group time activities, transitions, and departure. Fire Inspection: The most recent fire inspection was completed on January 6, 2025. Sanitation Inspection: The last sanitation inspection was completed on June 6, 2024. A “superior” classification was issued with eight (8) demerits noted on the grade card. Playground Inspection: The last playground inspection was completed on February 13, 2024, and recorded. Emergency Drills: The last fire drill was completed on January 6, 2025, at 10:00am and recorded. The last shelter-in-place drill was completed on December 6, 2024, and recorded. Files Reviewed: No New staff have been hired since your annual compliance visit on August 22, 2024. There were two violations observed during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. One child enrolled on September 1, 2020, was not signed in on February 10, 2025, and February 11, 2025. One child enrolled on April 8, 2019, was not signed in on February 10, 2025, and February 11, 2025. 10A NCAC 09 .0302(d)(4) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. I observed one container of Clorox Disinfectant Wipes with multiple warnings in the unlocked kitchen area on the counter top beside the stove that was accessible to children. .2820(b) Technical Assistance: Arrival/Departure Records: I suggested that a staff member review arrival/Departure sheets daily to ensure that each child has been signed in and out each day to maintain compliance with child care rules. Safe Environment/Hazardous Products: I suggested that staff conduct a daily walk through of the facility to ensure that all products with multiple warnings including keep out of reach of children are kept in locked storage and not accessible to children to maintain compliance with child care rules. One staff member required to have 10 hours of on-going training had 2.5 hours of training completed and on file for review. Two staff members required to have 10 hours of on-going training had 2 hours of training completed and on file for review. .1103(a) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The Administrator had a certificate of completion on file for review dated March 11, 2024. The previous completed ITS-SIDS training certificate on file for review was dated January 29, 2021. .1102(f) 1827 The Center did not have a person on staff who completed the EPR in Child Care training within the required timeframe and/or documentation of completion of the training was not on file or in a file designated for emergency preparedness and response plan documents. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 92 percent as of September 14, 2023. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent.
