You, Angela Battle, Director, assisted with today’s visit. Your program currently operates with a five-star rated license issued on October 26, 2018. The restrictions on your license include daytime care only, no cooking allowed in building, school age children only, may care for children up to 19 years of age, and meets enhanced ratios. The Secretary of State website was checked on June 8, 2026, and your business, Dulatown Outreach Center, Inc., was current and active. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines, lunch, free play, outdoor gross motor play, and departure during the visit. Required Inspections: The last fire inspection was conducted on September 23, 2025. The last sanitation inspection was conducted on September 8, 2025. A “superior” classification was issued with four (4) demerits noted on the grade card. The last playground inspection was conducted on May 22, 2026. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on July 30, 2025. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on May 21, 2026. The last emergency drill documented was conducted on May 20, 2026. I reviewed the ABCMS Provider portal during the visit. Two (2) staff members are listed as employees of your facility. I confirmed all current staff members are included in your facility portal. Files Reviewed: Two (2) children’s files were reviewed during the visit. One (1) existing staff file was reviewed during the visit. A checklist was used to note the requirements I monitored today. The following violations were observed/documented during today’s visit: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. On May 8, 2026, one child was not signed in and one child was not signed out. On May 18, 2026, one child was not signed out. On May 21, 2026, four children were not signed out. On May 22, 2026, one child was not signed in and one child was not signed out. On May 27, 2026, four children were not signed out. 10A NCAC 09 .0302(d)(4) 830 A First Aid information sheet was not posted in a place for referral and/or the information sheet did not include all the required information. In space #1/2 there was not a First Aid Information sheet posted. .0802(h) The violation(s) documented must be corrected immediately. On or before June 22, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If the parent fails to sign the child in or out, it is the staff member’s responsibility to document the child’s arrival and departure times. I suggested that you designate one staff member to monitor the arrival and departure records at designated times each day to ensure all children have been signed in and signed out. Required Postings (First Aid): A First Aid information sheet shall be posted in a place for referral. The information sheet shall include first aid guidance regarding burns, scalds, fractures, sprains, head injuries, poisons, skin wounds, stings and bites. An information sheet may be requested free of charge from the North Carolina Child Care Health and Safety Resource Center at 1-800-367-2229. I suggested that a staff member create and use a checklist to ensure all information required to be posted is present, current, and accurate. Consultation: We discussed the QRIS Modernization and the Pathway to the Stars. We reviewed all three (3) pathways, as well as the Education standards, Continuous Quality Improvement Standards (CQI), and Family and Community Engagement Standards. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. We discussed changes to the environmental rating scales and the implementation of the “3’s”. G.S. 110-90.2 & .2703(r) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. Emergency Preparedness Plan (EPR): I suggested you set a reminder in your calendar at least twice per year for trained staff to review the Emergency Preparedness Plan to ensure all information is up to date. I suggested that you complete the annual review the EPR plan at the same time you review the Emergency Medical Care Plan. Documentation of the review shall be maintained at the center in a file designated for emergency preparedness and response plan documents. All substitutes and volunteers counted in ratio shall be informed of the child care center's Emergency Preparedness and Response Plan and its location. Documentation of this notice shall be maintained in individual personnel files or in a file designated for emergency preparedness and response plan documents. A checklist was used to note the requirements I monitored today. You program currently operates with a five-star rated license issued on October 26, 2018. The restrictions on your license include daytime care only, no cooking allowed in building, school age children only, may care for children up to 19 years of age, and meets enhanced ratios. The Secretary of State website was checked on June 23, 2025, and your business, Prime Time Youth Focus, was active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment. Children throughout the facility were participating in routines and free play during the visit. The last sanitation inspection was conducted on November 12, 2024. A “superior” classification was issued with four (4) demerits noted on the grade card. The last fire inspection was conducted on September 15, 2024. The last fire drill documented was conducted on June 17, 2025. The last emergency drill documented was conducted on May 21, 2025. The last playground inspection was conducted on June 18, 2025. The last lead water test was conducted on June 30, 2021. I reviewed the ABCMS Provider portal during the visit. There are no staff members currently included in your facility portal. Three (3) children’s files were reviewed during the visit. Two (2) existing staff files were reviewed during the visit. The following violations were observed/documented during today’s visit: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Two (2) current staff members were not entered into the Automated Background Check Management System (ABCMS) to notify the Division. G.S. 110-90.2 & .2703(r) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The most recent annual EPR plan review on file at the facility was dated July 19, 2017. .0607(e) 1955 The child care center was not free of lead poisoning hazards as defined in G.S. 1130A-131.7(7).The most recent lead water test was conducted on June 30, 2021. 10A NCAC 09 .0601(f) The violation(s) documented must be corrected immediately. On or before July 8, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. I reviewed with you the Clean water for Kids website and how to access information to complete process. Emergency Preparedness Plan (EPR): I suggested you set a reminder in your calendar at least twice per year for trained staff to review the Emergency Preparedness Plan to ensure all information is up to date. I suggested that you complete the annual review the EPR plan at the same time you review the Emergency Medical Care Plan. Documentation of the review shall be maintained at the center in a file designated for emergency preparedness and response plan documents. All substitutes and volunteers counted in ratio shall be informed of the child care center's Emergency Preparedness and Response Plan and its location. Documentation of this notice shall be maintained in individual personnel files or in a file designated for emergency preparedness and response plan documents. Upon arrival, the purpose of today’s visit was reviewed with Carla Norwood, Group Leader. Dinah Brown, Child Care Consultant, accompanied me on the visit. Angela Battle, Administrator, was not on site during the visit. When I asked if Ms. Battle was on her way, I was told she was unavailable today and would not be on the premises. The Legal Designee Form was reviewed, and Doris Brown-Dula was listed; therefore, we reviewed the visit summary with Ms. Brown-Dula. The last annual compliance visit was conducted on August 17, 2022. The North Carolina Secretary of State’s website was viewed before the visit, and Dulatown Outreach Center, Inc was listed as current/active. You program currently operates with a five-star rated license issued on October 26, 2018. The restrictions on your license include daytime care only, no cooking allowed in building, school age children only, may care for children up to 19 years of age, and meets enhanced ratios. Fire Inspection: The last fire inspection was made on August 22, 2022, by Chris Jacobs, Fire Inspector. The center was approved for daytime care only. Sanitation Inspection: The last sanitation inspection was made on October 31, 2022, by Logan Little, Environmental Health Specialist. A “superior” classification was issued with eight (8) demerits noted on the grade card. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I conducted the walk-through of the facility. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during indoor free play, outdoor free play, and routines. A checklist was used today for monitoring. Children’s and staff files were not monitored today and as discussed, I will return on Wednesday, August 2, 2023, to monitor files. A hand-written visit summary was prepared today due to connectivity issues with the Internet. The following violations were observed and corrected during the visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space #1, two (2) electrical outlets directly across from the science center were not covered with a safety plug. One (1) electrical outlet was not covered with a safety plug next to the cozy center. In space $4, one (1) electrical outlet was not covered on the wall adjacent to the bathroom used by children enrolled. 10A NCAC 09 .0604(c) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. A. Battle received two (2) of the five (5) hours of on-going training that were required based on education and experience. .1103(a) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Fifteen (15) emergency and identifying information forms located on the van used to transport children, did not include the child's picture. 10A NCAC 09 .1003(d) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. A. Battle last completed the health and safety trainings in 2017. .1103(b) 9995 A violation was found for which there is no item number. Per Sanitation Requirement .2820(c), Non-aerosol sanitizing, disinfecting, and detergent solutions, hand sanitizers, and hand lotions shall be kept out of reach of children when not in use, but are not required to be in locked storage. In space #1, one (1) bottle of hand sanitizer was located on the floor behind the television. Compliance History: The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was ninety-three percent (93%) as of July 25, 2023. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance: • We discussed keeping electrical outlets covered with a safety plug when not in use. The four electrical outlets were covered with a safety plug during the visit to correct this violation. • I reminded you that hand sanitizer must be stored at least five feet from ground level and inaccessible to children. The hand sanitizer that was on the floor behind the television was picked and placed on a shelf five feet from ground level. Reminders: • I will return on Wednesday, August 2, 2023, to review items not monitored today including but not limited to staff and children’s files. • From now until June 30, 2024, is your preparation period for the rated license assessment. As I discussed, please continue to prepare your environment, remove excess materials and store in an alternate location. You can review NCRLAP.org for webinars regarding the SACERS assessment. I suggested that you contact Southwestern Child Development Commission for trainings and guidance regarding the environment rating scale. • A new helpline has launched regarding Children and Challenging Behaviors.
