You, Sue Hamby, Program Coordinator assisted with today’s visit. Your program currently operates with a five-star license issued on November 17, 2024. The permit restrictions listed on your license including Daytime Care Only; School age children only; Playground area does not meet child care playground safety standards; Meets enhanced ratios; Meets enhanced space were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment. Children throughout the facility were participating in routines, snack, free play, and departure during the visit. Required Inspections: The last fire inspection was conducted on September 23, 2025. The last sanitation inspection was conducted on January 24, 2025. A “superior” classification was issued with eight (8) demerits noted on the grade card. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on January 24, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on September 11, 2025. The last emergency drill documented was conducted on June 2, 2025. A checklist was used to note the requirements I monitored today. I reviewed the ABCMS Provider portal during the visit. Four (4) staff members are listed as employees of your facility. I confirmed all current staff members are included in your facility portal. Files Reviewed: Four (4) children’s files were reviewed during the visit. One (1) existing staff file was reviewed during the visit. The following violation was observed/documented during today’s visit: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. One child was not signed in on September 2, 2025. On September 19, 2025, fourteen children were not signed out. 10A NCAC 09 .0302(d)(4) The violation(s) documented must be corrected immediately. On or before October 15, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Consultation: We discussed the QRIS Modernization and the Pathway to the Stars. We reviewed all three (3) pathways, as well as the Family and Community Engagement Standards. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. Upon arrival you, Sue Hamby, Program Coordinator, assisted with the visit. Your last annual compliance visit was conducted on January 8, 2024. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios in compliance. Children were observed during arrival, handwashing, toileting, snack, transitions, gross motor play, and departure. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on March 19, 2024. Sanitation Inspection: The last sanitation inspection was completed on December 6, 2023. A “superior” classification was issued with four(4) demerits noted on the grade card. Emergency Drills: The last fire drill was completed on October 16, 2024, at 7:35 am and recorded. A checklist was used to note the requirements I monitored today. Files Reviewed: One (1) staff file was reviewed today. Five (5) children's file were reviewed today. There were two violations observed during today’s visit. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Two children enrolled on September 3, 2024 had emergency medicines that were not in the original pharmacy labeled containers. .0803(2)(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Once child enrolled on September 3, 2024 had an emergency medication that expired in January 2024 on site. .0803(12) Rated License Status Rated License Status: Your program currently has seven (7) points in Program Standards, seven (7) points in Education and one (1) Quality Point (has reduced staff/child ratios by one in each group from the seven point level as required in Rule.2818(c) for a total of fifteen (15) points and a five (5) star rated license effective February 23, 2018. The permit restrictions were in compliance including 1st shift, school age, meets enhanced ratios, meets enhanced space, meets enhanced ratios reduced by 1 and playground does not meet child care requirements. Phyllis Crump completed the application for a reassessment on November 4, 2024. The rated license assessment request review was completed by Phyllis Crump on February 5, 2024. The request for the SACERS-U was completed on March 5, 2024. The SACERS-U assessment was conducted on May 8, 2024. The SACERS-U scores were reviewed and accepted on May 17, 2024, with Mary Kidder, Caldwell County School Wraparound Director and Phyllis Crump, Caldwell County Schools Wraparound Assistant Director. Prior to today’s visit, your compliance history was ninety-five (95) percent as of November 12, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Compliance Letter: You must correct the violation found during today's visit immediately. Upon arrival you, Sue Hamby, Administrator, assisted me with the visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a five star license effective July 24, 2017. The permit restrictions were in compliance including 1st shift, school age, meets enhanced ratios, meets enhanced space, meets enhanced ratios minus one and playground doesn’t meet childcare standards. Your last annual fire inspection was conducted on March 19, 2024. The facility was approved for daytime care only. Your sanitation inspection was conducted on December 6, 2023 with 4 demerits and a superior rating. Your last monthly fire drill was conducted on May 8, 2024, at 7:30am. Your last emergency lockdown drill was conducted on February 29, 2024. Children were observed during transitions, handwashing, toileting, snack, gross motor outdoor play, and departure. A checklist was used to note the requirements I monitored today. On May 3, 2024 twelve children were not signed out of morning care. On May 8, 2024, one child was not signed out of am care. On May 8, 2024, four children were not signed out of pm care. On May 9, 2024, three children were not signed out of pm care. On May 10, 2024, one child was not signed out of am care and eighteen children were not signed out of pm care. 10A NCAC 09 .0302(d)(4) Technical Assistance: Arrival and Departure: Specific arrival and departure times are needed to ensure safe tracking of the children. Your program currently operates with a five star license effective July 24, 2017. The permit restrictions were in compliance including 1st shift, school age, meets enhanced ratios, meets enhanced space, meets enhanced ratios minus one and playground doesn’t meet childcare standards. Your last annual fire inspection was conducted on March 19, 2024. The facility was approved for daytime care only. Your sanitation inspection was conducted on December 6, 2023 with 4 demerits and a superior rating. Your last monthly fire drill was conducted on May 8, 2024, at 7:30am. Your last emergency lockdown drill was conducted on February 29, 2024. Children were observed during transitions, handwashing, toileting, snack, gross motor outdoor play, and departure. A checklist was used to note the requirements I monitored today. Upon arrival you, Sue Hamby, Administrator, assisted me with the visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a five star license effective July 24, 2017. The permit restrictions were in compliance including 1st shift, school age, meets enhanced ratios, meets enhanced space, meets enhanced ratios minus one and playground doesn’t meet childcare standards. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. You conducted the walkthrough of the facility with me. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios in compliance. Children throughout the facility were participating in group time, free play in activity areas, and outdoor gross motor time. The center's compliance history was at 92% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A sample of five children’s files were reviewed. One new staff file was reviewed. A sample of one existing staff file was reviewed. The most recent sanitation inspection for your facility was conducted on December 6, 2023. A superior sanitation classification was issued with four demerits. The most recent fire inspection for your facility was conducted on September 15, 2023. The facility was approved for day time care. The following violation(s) were observed and discussed with you today: Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. One incident report completed on 9/8/23 did not have the type of injury, the piece of equipment involved, or the cause of the injury completed on the report. .0802 (e) The violation(s) documented must be corrected immediately. On or before January 22, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. If you email the compliance letter, it must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed.
