Name of Operation: HAPPY VALLEY PRE-K Facility ID: 14000420 Consultant: DINAH BROWN Operation Type: Center Case Number: Visit Date: 10/7/2025 Number Present: 13 Completed Date: 10/7/2025 Age: From 3 To 5 Total Minutes: 185 Time In: 10:20 AM Time Out: 01:25 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. You, Paige Christian, Lead Teacher, assisted me with today’s visit. Your program currently operates with a five-star license issued on June 25, 2025. The restrictions listed on your license include daytime only, meets enhanced ratios minus one, Certified Developmental Day, Meets enhanced ratios and Meets enhanced space, all restrictions were observed to be in compliance. Required Inspections: The last fire inspection was conducted on September 26, 2025. The last sanitation inspection was conducted on October 15, 2025. A “superior” classification was issued with eight (8) demerits noted on the grade card. The most recent playground inspection was conducted on September 26, 2025. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on January 16, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The most recent fire drill was conducted on September 11, 2025. The most recent emergency drill was conducted on September 15, 2025. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment. Children throughout the facility were participating in routines, lunch, group time, free play, rest time and outdoor play during the visit. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. No new staff members have been hired since the last visit. The following violations were observed/documented during the visit: Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. A copy of the incident report for a child enrolled on August 25, 2025,was not maintained in the child's file. .0802 (e) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. I observed a used Band-aid, two broken plastic eggs, black tape, pieces of plastic trash and wrappers on the playground in the mulched areas. 10A NCAC 09 .0604(p) You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 100%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Technical assistance was provided on the following: Incident Reports: We discussed that after completing an incident report in its entirety a signed copy should be given to the parent/caregiver and another copy placed in the child’s folder. Playground Inspections: Per Child Care requirement 10A NCAC 09.0604(p), you must monitor indoor and outdoor areas prior to children utilizing the playground to ensure debris and broken equipment is discarded. I suggested that a designated staff member monitor classrooms and playgrounds each morning to discard trash, debris, and broken items before children arrive or access the playground. Consultation As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days.
