You, Shannon Greene, Program Coordinator, assisted with today’s visit. Your program currently operates with a five-star license issued on November 13, 2023. The permit restrictions listed on your license including Daytime Care Only; School age children only; Playground area does not meet child care playground safety standards; Meets enhanced ratios; Meets enhanced space were in compliance. The Secretary of State website was checked on April 8, 2026. Caldwell County Schools was not listed. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today; all licensed and approved space was monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines, free play, lunch, and gross motor play during the visit. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on August 2, 2023. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on March 17, 2026. The last emergency drill documented was conducted on February 20, 2026. I reviewed the ABCMS Provider portal during the visit. I confirmed six (6) current staff members are included in your facility portal. File Reviewed: One existing staff file was reviewed during the visit. No new staff have been employed since the last monitoring visit. Rated License: Phyllis Crump, Administrator, has chosen to meet the requirements for Pathway #1, Program Assessment. I received your Application for Assessment for a Rated License for Centers on March 25, 2026. You stated that you have completed the three-month self-study that began in November 2025. As a result of the self-study and mock SACERS assessments that you had conducted from an outside agency, your program is receiving supplemental materials. You stated that you will send the self-study, facility and individual CQI plans, Family and Community Engagement Standards, Staff Education and Experience Worksheet, and the Rated License Assessment Request Review form to me by April 13, 2026. I will submit your request to NCRLAP for your Environment Rating Scale at Gamewell Elementary Wrap Around when your request for review has been received. Individual CQI Plans must be completed annually and a copy placed on file for review. One violation was observed/documented during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. On March 25, 2026, one child was not signed out. On March 27, 2026, two children were not signed out. The violation(s) documented must be corrected immediately. On or before April 22, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. You, Jennifer Branch-Saylor, Director, assisted me with today’s visit. Your program currently operates with a five (5) star license effective November 13, 2023. The permit restrictions were in compliance including Daytime care only; School age children only; Playground does not meet child care playground safety standards; Meets enhanced ratios; Meets enhanced space. Your program earned seven (7) points in education, six (6) points in program standards and no quality point. The last sanitation inspection was conducted on January 29, 2025. A “superior” classification was issued with zero (0) demerits noted on the grade card. The last fire inspection was conducted on March 5, 2025. The most recent fire drill was conducted on June 3, 2025, the most recent emergency drill was conducted on May 27, 2025. Children throughout the facility were participating in routines, free play, rest time, snack, and indoor gross motor play during the visit. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. One new staff member was hired since the last visit. I also suggested you remind staff members to sign children in and out in the event that the parent fails to do so. Medication: I suggested you have all medication brought to the office when a parent brings in a new medication to ensure all medication is appropriate and not expired before taking the medication to the classroom. Please remind parents that any medication they fail to retrieve within 72 hours of completion of treatment, or withdrawal of authorization will be discarded. Additionally, I suggested that you assign a staff member to each room to check all medications and Permission to Administer Medication Forms at least once a month to prevent further non-compliance regarding medication. Consultation As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. You, Jennifer Branch-Saylor, Director, assisted me with today’s visit. Your program currently operates with a five (5) star license effective November 13, 2023. The permit restrictions were in compliance including Daytime care only; School age children only; Playground does not meet child care playground safety standards; Meets enhanced ratios; Meets enhanced space. Your program earned seven (7) points in education, six (6) points in program standards and no quality point. The last sanitation inspection was conducted on January 29, 2025. A “superior” classification was issued with zero (0) demerits noted on the grade card. The last fire inspection was conducted on March 5, 2025. The most recent fire drill was conducted on June 3, 2025, the most recent emergency drill was conducted on May 27, 2025. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on February 19, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment. Children throughout the facility were participating in routines, free play, rest time, snack, and indoor gross motor play during the visit. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. One new staff member was hired since the last visit. The following violations were observed/documented during the visit: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. On May 30, 2025, one child was not signed out. On June 6, 2025, one child was not signed out. 10A NCAC 09 .0302(d)(4) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One child enrolled on June 6, 2025, had a allergy Epi-Pen medication at school for emergency use that expired in April 2025. .0803(12) The violation(s) documented must be corrected immediately. Medication: I suggested you have all medication brought to the office when a parent brings in a new medication to ensure all medication is appropriate and not expired before taking the medication to the classroom. Please remind parents that any medication they fail to retrieve within 72 hours of completion of treatment, or withdrawal of authorization will be discarded. Additionally, I suggested that you assign a staff member to each room to check all medications and Permission to Administer Medication Forms at least once a month to prevent further non-compliance regarding medication. The center’s compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 95 percent as of today. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance on violations was reviewed with you. #1921 Staff working in part-time, full-day or track out school age care programs required to complete BSAC training, did not complete it within three months of employment. One staff employed on 9/13/23 did not complete BSAC training. Training must be completed within three months from the hire date. We suggested you have a checklist for new employees to ensure that all requirements are met within the required timeframes. Consultation: -we reviewed transitions of children from the afterschool space to the gym and cafeteria. Continue to ensure that supervision is maintained. -we reminded you that a lockdown or shelter-in -place drill needs to be conducted before the end of the month. -BSAC training can be taken online.
