You, Alison Auton, lead teacher, assisted with today’s visit. The facility currently operates with a 5-star rated license issued on May 24, 2019, earning seven (7) points in staff education, seven (7) points in the program standards, and one quality point. The program's license restrictions include the following: Daytime care, Meets enhanced ratios and enhanced space, enhanced ratios reduced by 1, and certified Developmental Day. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment. Children throughout the facility were participating in routines, free play, lunch, rest time, and departure during the visit. Required Inspections: The last fire inspection was conducted on September 17, 2025. The last sanitation inspection was conducted on September 5, 2025. A “superior” classification was issued with two (2) demerits noted on the grade card. The last playground inspection was conducted on October 9, 2025. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on February 19, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on October 17, 2025. The last emergency drill documented was conducted on October 18, 2025. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. A checklist was used to note the requirements I monitored today. I reviewed the ABCMS Provider portal during the visit. Six (6) staff members are listed as employees of your facility. I confirmed all current staff members are included in your facility portal. Files Reviewed: Three (3) children’s files were reviewed during the visit. Three (3) new staff files and one (1) existing staff file were reviewed during the visit. Three violations were observed/documented during today’s visit: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. On October 7, 2025, two children were not signed out. On October 9, 2025, two children were not signed out. On October 10, 2025, one child was not signed out. On October 15, 2025, one child was not signed out. On October 27, 2025, one child was not signed in and three children were not signed out. 10A NCAC 09 .0302(d)(4) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. A child enrolled on August 25, 2025, had an emergency Vatoco Diazepam Nasal Spray that was not in the original labeled container. .0803(2)(a) 1314 Emergency information did not name childs health care professional. A child enrolled on August 25, 2025, did not have a health care professional listed on the application for enrollment form. .0802(c)(2) The violation(s) documented must be corrected immediately. On or before November 12, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Medication: I suggested that you monitor all medications received from the parent/guardian prior to the medication being administered to the child or prior to the medication being stored in the classroom. Emergency Information: I suggested you review all paperwork from parents on the date of each child’s enrollment to ensure all required documentation is on file. Consultation: We discussed the QRIS Modernization and the Pathway to the Stars. We reviewed all three (3) pathways, as well as the Family and Community Engagement Standards. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. Upon arrival, the purpose of today’s visit was reviewed with you, Angela Smith, Lead Teacher. Your last annual compliance visit was conducted on November 14, 2024. The facility currently operates with a 5-star rated license issued on May 24, 2019, earning seven (7) points in staff education, seven (7) points in the program standards, and one quality point. The program's license restrictions include the following: Daytime care, Meets enhanced ratios and enhanced space, enhanced ratios reduced by 1, and certified Developmental Day. A walk-through of the facility was completed today, all indoor and outdoor areas used by children enrolled were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during independent center play, teacher lead group time activities, transitions, handwashing, toileting, lunch, rest time, snack, outdoor gross motor activities, and departure. Required Inspections: The most recent fire inspection was completed on March 25, 2025. The last sanitation inspection was completed on March 20, 2025. A “superior” classification was issued with zero (0) demerits noted on the grade card. The last playground inspection was completed on April 10, 2025, and recorded. Emergency Drills: The last fire drill was completed on April 11, 2025, at 1:56pm and recorded. The last shelter-in-place drill was completed on March 7, 2025, and recorded. Curriculum: Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. Lead Water Testing: The most recent lead water testing results were completed on August 10, 2024. Lead testing must be completed every three years. I observed a power strip in front of the teachers desk that had two outlets not covered with outlet covers that was accessible to children. 10A NCAC 09 .0604(c) Consultation: Technical Assistance: Outlet Covers: I suggested that staff conduct a daily classroom safety walk through each morning before children arrive to ensure that all outlets are covered and covers are in place, including in power strips to ensure the health and safety of children and maintain compliance as required by child care rules. Arrival and Departure: I suggested a staff member review arrival and departure sheets daily to ensure each child has been signed in and out each day as required by child care rules. Your last annual compliance visit was conducted on November 17, 2023. You, Christy Smith, assisted with the visit. The facility currently operates with a 5-star rated license issued on May 24, 2019, earning seven (7) points in staff education, seven (7) points in the program standards, and one quality point. The program's license restrictions include the following: daytime care, meets enhance ratios and enhanced space, enhanced ratios reduced by 1, and certified Developmental Day. Your last annual fire inspection was conducted on March 27, 2024. The facility was approved for daytime care only. Your sanitation inspection was conducted on April 27, 2023, with 4 demerits and a superior rating. Your last monthly fire drill was conducted on April 23, 2024 at 9:21am. Your last emergency lockdown drill was conducted on May 3, 2024. Your last monthly playground inspection was conducted on May 7, 2024. Children were observed during independent center play, lunch, teacher directed activities, handwashing, toileting, transitions, lunch, and nap/rest. NC Pre-K and Developmental Day requirements were monitored and observed to be in compliance. A checklist was used to note the requirements I monitored today. The following violations were observed: Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The most recent sanitation inspection was conducted on April 27, 2023 and not conducted within one year. 10A NCAC 09 .0304(b) 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. An annual fire inspection was conducted on March 27, 2024. A copy was given to me during the routine unannounced visit conducted on May 15, 2024. 10A NCAC 09 .0304(a) Technical Assistance: Sanitation Inspections: As a reminder, sanitation inspections must be conducted annually within the same month the last sanitation inspections was conducted. I suggested contacting the Environmental Health Inspector at least one month prior to when the last sanitation inspection was conducted. Fire Inspection Reports: As a reminder, fire inspections must be submitted to DCDEE within one week of the completed fire inspection. Consultation: Prior to today’s visit, your compliance history was ninety-seven (97) percent as of May 14, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. You, Christy Smith and Christina Gray, Lead Teachers, assisted me with the visit today. The Secretary of State website was checked on November 15, 2023, and your business Caldwell County Schools., is not listed. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on October 31, 2023. I reminded you to send me of a copy of the most recent fire inspection report. Sanitation Inspection: The last sanitation inspection was completed on April 27, 2023. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills/Playground Inspections: The lockdown drill was completed on September 1, 2023, at 9:25am and recorded. The last playground inspection was completed on November 1, 2023. The last fire drill was completed on October 24, 2023, at 1:38pm. A complete walkthrough of licensed indoor and outdoor spaces was conducted. NC Pre-K and Developmental Day: NC Pre-K and Developmental Day requirements were monitored and in compliance. A checklist was used to note the requirements I monitored today. Staff and children were observed during indoor and outdoor free play. Files Reviewed: Children’s files, program files, and staff files were monitored today. Seven (7) children's files were reviewed during today’s visit. Six (6) staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violations were observed during the walk through today. I discussed these violations with you in detail at the end of the visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. In space #3, on October 18, 2023, one child’s departure time was not documented; on October 19, 2023, one child’s departure time was not documented; and on October 25, 2023, two children’s departure times were not documented. 10A NCAC 09 .0302(d)(4) 1898 Staff did not complete the health and safety training within one year of employment. In Space #3, one staff member employed 8/15/22 did not have verification of completion of Health and Safety Trainings in all topic areas required within the first year of hire. .1102(a) The following Technical Assistance regarding the violations observed during today’s visit was provided: -Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart the child care facility to ensure accurate records are kept. I suggested creating a system and review with staff of the procedures of recording sign-in and sign-out times. Healthy and Safety Training: Healthy and Safety training must be completed within one year of employment and every five years thereafter in all required topic areas. Consider using a calendar for training due dates for employee training. Childcare Requirement .1102(a) Trainings cited during today’s visit, must be completed within 2 weeks. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-seven (97) %. The period for background check qualification renewals was revised from three years to five years. 2. The provisional qualification period was reduced to forty-five days.
