You, Candy Walston, Owner/Operator, assisted with today’s visit. Your program currently operates with a four (4) star license effective January 14, 2020. The permit restrictions were in compliance including first shift and overnight, meets enhanced ratios and meets enhanced space. The Secretary of State website was checked on March 19, 2026, and your business, Forever Young Child Development Center, was not listed. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines, free play, lunch, rest time, and outdoor gross motor play during the visit. Required Inspections: The last fire inspection was conducted on July 30, 2025. The last sanitation inspection was conducted on February 18, 2026. A “superior” classification was issued with zero (0) demerits noted on the grade card. The last playground inspection was conducted on March 10, 2026. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on October 7, 2025. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on March 10, 2026. The last emergency drill documented was conducted on February 20, 2026. You stated that your program uses the Creative Curriculum and staff have begun formative assessments using the Desired Results Developmental Profile (DRDP) out of California DPI that is listed on the Division’s approved formative assessment list. I reviewed the ABCMS Provider portal during the visit. Four (4) current staff members are listed as employees of your facility. You reported that you have been working to add two staff members to the ABCMS portal but have experienced difficulties reaching their technical support. You provided me with a copy of the email thread of your attempts to contact ABCMS technical support for assistance. I provided you with the DCDEE_ABCMS_PROVIDER@dhhs.nc.gov email to contact for assistance. Files Reviewed: Three (3) children’s files were reviewed during the visit. One (1) existing staff file was reviewed during the visit. A checklist was used to note the requirements I monitored today. The following violation was observed/documented during today’s visit: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. One staff member is not listed in the ABCMS portal under your facility's roster. G.S. 110-90.2 & .2703(r) The violation(s) documented must be corrected immediately. On or before April 2, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Please send your letter to dinah.brown@dhhs.nc.gov. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 99%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Technical assistance was provided on the following: ABCMS: I discussed with you that you must notify the Division of all new child care providers who are hired or have moved into the child care facility within five business days by entering each staff member’s information into the Automated Background Check Management System (ABCMS) Provider Portal. Consultation: We discussed the QRIS Modernization and the Pathway to the Stars. We discussed pathways one and two, as well as the Education standards, Continuous Quality Improvement Standards (CQI), and Family and Community Engagement Standards. You stated that your facility was choosing to pursue pathway two and provided me with your application for a rated license for centers during today’s visit and the Family and Community Engagement Standards you chose. We reviewed your paperwork and documentation for the Family and Community Engagement Standards. We reviewed your formative assessment. You are continuing to work on your facility’s CQI Plan and the staff information and education worksheet. You stated that you will send me that paperwork once it is completed in order to move forward with your rated license process. You confirmed that all staff members have a WORKS account, that all transcripts have been submitted, and all WORKS accounts are up to date. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. Upon arrival, the purpose of today’s visit was reviewed with you, Candy Walston, Owner/Administrator, who assisted with the visit. Your last annual compliance visit was conducted on April 25, 2024. Your program currently operates with a four (4) star license effective January 14, 2020. The permit restrictions were in compliance including first shift and overnight, meets enhanced ratios and meets enhanced space. You have earned four (4) points in education, six (6) points in program standards and one (1) quality point for turnover less than twenty (20) percent for a total of eleven (11) points. A walk-through of the facility was completed today, all indoor and outdoor areas used by children enrolled were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during teacher lead group time activities, transitions, lunch, rest time, and departure. Required Inspections: The most recent fire inspection was completed on July 17, 2025. The most recent sanitation inspection was completed on March 21, 2025. A “superior” classification was issued with six (6) demerits noted on the grade card. The inspection that occurred prior to March 21, 2025, was conducted on February 26, 2024. The last playground inspection was completed on March 28, 2025, and recorded. Emergency Drills: The last fire drill was completed on March 28, 2025, at 9:50am and recorded. The last shelter-in-place drill was completed on February 10, 2025, and recorded. Curriculum: Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. Lead Water Testing: The most recent lead water testing results were completed on September 27, 2022. Lead testing must be completed every three years. Files Reviewed: Two (2) staff files were reviewed today. Three (3) children’s files were reviewed today. A checklist was used to note the requirements I monitored during today’s visit. I discussed the Staff and Training Worksheets and the Children’s Records forms with you today. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. Upon arrival, the purpose of today’s visit was reviewed with you, Candy Walston, Owner/Administrator, who assisted with the visit. Your last annual compliance visit was conducted on April 25, 2024. Your program currently operates with a four (4) star license effective January 14, 2020. The permit restrictions were in compliance including first shift and overnight, meets enhanced ratios and meets enhanced space. You have earned four (4) points in education, six (6) points in program standards and one (1) quality point for turnover less than twenty (20) percent for a total of eleven (11) points. A walk-through of the facility was completed today, all indoor and outdoor areas used by children enrolled were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during teacher lead group time activities, transitions, lunch, rest time, and departure. Required Inspections: The most recent fire inspection was completed on July 17, 2025. The most recent sanitation inspection was completed on March 21, 2025. A “superior” classification was issued with six (6) demerits noted on the grade card. The inspection that occurred prior to March 21, 2025, was conducted on February 26, 2024. The last playground inspection was completed on March 28, 2025, and recorded. Emergency Drills: The last fire drill was completed on March 28, 2025, at 9:50am and recorded. The last shelter-in-place drill was completed on February 10, 2025, and recorded. Curriculum: Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. Lead Water Testing: The most recent lead water testing results were completed on September 27, 2022. Lead testing must be completed every three years. Files Reviewed: Two (2) staff files were reviewed today. Three (3) children’s files were reviewed today. A checklist was used to note the requirements I monitored during today’s visit. I discussed the Staff and Training Worksheets and the Children’s Records forms with you today. There was one violation observed and documented during today’s visit, Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The last inspection that took place prior to March 21, 2025 occurred on February 26, 2024. 10A NCAC 09 .0304(b) Technical Assistance: Sanitation Inspection: I suggested that you reach out to Environmental Health at least one month prior to the expiration of the current inspection to ensure that an inspection is conducted at least annually and to maintain compliance with child care rules. Compliance History: Prior to today’s visit, your compliance history was ninety-nine (99) percent as of April 9, 2025. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Compliance Letter: You must correct the violation found during today's visit immediately. Please send me a letter verifying compliance by April 23, 2025. Please include in that letter each item number and explain in detail how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Lead Testing: Child Care Rule .0601(f)- Safe Environments Child Care Rule Effective January 1, 2024 Rule Amendment- This rule was amended to include the specific requirements that child care centers must be free of asbestos hazards. This is in addition to the existing requirement that they be free of lead poisoning hazards. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The program is free, and the first step is to watch one of the pre-enrollment webinars, which are available at www.cleanwaterforUSkids.org/carolina or paste https://bit.ly/3CK-webinar All licensed child care programs must register by May 1, 2024, at www.cleanwaterforUSkids.org/carolina and watch one of the pre-enrollment webinars. Child Care Centers have until July 1, 2024, to complete the required lead testing. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Your last annual compliance visit was conducted on May 9, 2023. You, Candy Walston, Administrator, assisted me with the visit. Your program currently operates with a four (4) star license effective January 14, 2020. The permit restrictions were in compliance including first shift and overnight, meets enhanced ratios and meets enhanced space. You have earned four (4) points in education, six (6) points in program standards and one (1) quality point for turnover less than twenty (20) percent for a total of eleven (11) points. Your last annual fire inspection was conducted on July 21, 2023. The facility was approved for day and night time care. There are no children currently enrolled for night time care. Your last monthly fire drill was conducted on April 3, 2024, at 1:45pm. Your last emergency shelter-in -place was conducted on February 9, 2024. Your last monthly playground inspection was conducted on April 3, 2024. Children were observed during independent center play, nap/rest, and handwashing. A checklist was used to note the requirements I monitored today. The following violation was observed: Violation Number Comment Rule 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff member employed on August 29, 2016 did not have a current Recognizing and Responding to Suspicions of Maltreatment on file for review. The Recognizing and Responding to Suspicions of Maltreatment on file for review was dated September 9, 2016. .1103(b) Technical Assistance: I recommend using the health and safety training log to document all required health and safety trainings to ensure that each training has been taken and recorded within the required timeframe as required by child care rules. The health and safety training log is located on the DCDEE website under provider documents. Consultation: Prior to today’s visit, your compliance history was one hundred (100) percent as of April 24, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent.
