    Name of Operation: CALDWELL EARLY HEAD START Facility ID: 14000442 Consultant: GAIL DULA Operation Type: Center Case Number: Visit Date: 8/28/2025 Number Present: 16 Completed Date: 8/28/2025 Age: From 0 To 2 Total Minutes: 210 Time In: 10:15 AM Time Out: 01:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor your compliance with all applicable child care requirements during a temporary time period visit. You, Shannon Ferguson, assistant director, assisted me with the visit. The Secretary of State website was checked prior to the visit on today August 28, 2025, and The Enola Group Inc., was active and current. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, your consultant, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. Currently this center operates with a temporary license issued on 7/29/25, with the following restriction: Daytime care only Children in care were observed during indoor free play, outdoor gross motor activities, bathroom/handwashing routines, lunch, nap, and departure. Multiage ratios were in compliance. Supervision was in compliance. A fire drill is scheduled before the end of the week. A sanitation inspection was completed on 7/21/25, and you received a superior classification and zero (0) demerits. Water testing was completed on 7/24/25. A fire inspection was conducted on 6/26/25 for this new location and approved for daytime care only. A sample of four children’s records were reviewed with no violations. One new staff file and two existing staff files were reviewed with one violation cited. None of the enrolled children had medication. One violation was observed during the visit. A hand written summary was completed and reviewed during the visit due to a connectivity issue. The computer generated summary will be emailed to you. Violation Number Comment Rule 1757 A valid qualification letter was not on file and available to review at the facility. One staff with a hire date of 11/24/24 did not have a copy of their current qualification letter on file. G.S. 110-90.2(b) & (d) & .2703(e) The above uncorrected violation must be brought into compliance immediately. You shall submit a written, signed and dated statement or email to me at the address below detailing specifically how the above violation(s) was corrected. The violations must be corrected no later than September 11, 2025. Failure to correct the violations and send the written statement by the due date listed above, noncompliance, may lead to an administrative action. Technical assistance: #1757 One staff employed on 4/24/14 did not have a copy of the current qualifying letter on file. I suggested you review staff files regularly to ensure no qualifying letters have expired and that all staff has a current qualifying letter on file. Additionally, I suggested you use the staff file checklist found on the DCDEE website under the “Provider” tab to ensure all required documentation is on file. Consultation: -I provided a Preservice for Administrator form and a Legal designee form for the new director. I discussed submitted the required information. Ms. Pearson is in WORKS and has bee evaluated as an administrator. She is registered for the Emergency Preparedness Response class next week. -we discussed the QRIS system. You said that the facility plans to pursue Pathway 3. I suggested you wait until after the provider meeting next month so you will be more familiar with the process. -portable toys are available for the playground. Riding toys are also available to use on the riding track. You said additional equipment is being purchased. -we discussed the ABCMS system. Staff is showing however they are currently under the old ID. I suggested you call to inquire about the best way to change the staff into the new ID. As discussed, violations cited today and/or during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Your program must maintain a compliance score of at least 75%. Your compliance historyis currently 95%.
