You, Angela Battle, Director, assisted with today’s visit. The facility currently operates with a 3-stars rated license issued on June 24, 2020, earning five (5) points in staff education, two (2) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care and enhanced ratios. The Secretary of State website was checked on June 25, 2026, and your business Dulatown Outreach Center, Inc., was active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines, free play, rest time, snack, and departure during the visit. Required Inspections: The last fire inspection was conducted on September 23, 2025. The last sanitation inspection was conducted on September 8, 2025. A “superior” classification was issued with four (4) demerits noted on the grade card. The most recent playground inspection was conducted on June 17, 2026. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on July 30, 2025. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The most recent fire drill was conducted on June 17, 2026. The most recent emergency drill was conducted on May 19, 2026. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. No new staff members have been hired since the last visit. The following violations were observed/documented during the visit: Violation Number Comment Rule 541 The written feeding plan did not include the child's name, parent signature, and/or was not dated when received by the center. A child enrolled on February 24, 2026, had an infant feeding plan posted that was signed by the parent. .0902(a) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. A child enrolled on June 3, 2026, did not have a sleep position marked on June 19, 2026, at 8:30am, 8:45am, 9:00am, 9:15am, and 9:30am. A child enrolled on October 13, 2025, did not have staff initials and a sleep position marked on June 19, 2026, at 1:15pm, and on June 23, 2026, at 11:00am and 11:15am, at 11:30am no sleep position was marked. A child enrolled on April 8, 2026, did not have a sleep position marked on June 19, 2026, at 8:00am, 8:15am, 8:30am, 8:45am, 9:00am, 11:15am, 11:30am, 11:45am, 1:00pm, 1:15pm, 1:30pm, and 1:45pm, no sleep check was conducted at 1:30pm. If it is determined the information provided in the letter is not true, it may be considered falsification of information. Technical assistance was provided on the following: Infant Feeding Plan: I suggested that you review this requirement with all staff members employed in classrooms where children under 15 months of age are enrolled and remind staff members that they should review all infant feeding plans with the parents to ensure information is recorded correctly and that the feeding plans are fully completed when received by the facility. Infant Sleep Checks: I suggested that you discuss requirements for safe sleep checks as well as sudden infant death syndrome with all staff, especially all staff who work in the classroom for infants. I suggested utilizing the Caring for Our Children resource book for more information pertaining to safe sleep practices and sudden infant death syndrome. I suggested determining how safe sleep checks will be turned in to administrative staff and create a plan to review safe sleep checks prior to filing. Create a system for enhancing your filing system for safe sleep checks so that records do not become misplaced. Medication: Consultation As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. We discussed the QRIS Modernization and the Pathway to the Stars. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. I discussed with you that lead water testing is due every three years and suggested a calendar alert reminder to complete this task before its due. I reviewed with you the Clean water for Kids website and how to access information to complete process. Consultation As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. On the playground space used by infants and toddlers aged children, the bottom left corner of the fence adjacent to the office building measured five (5) inches. .0605(g) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In Space #3b, on October 18, 2023, at 11:30am, 11:45am, 12:00pm, 12:15pm, and 12:30pm, the sleep positions were not documented for one infant; on October 31, 2023, at 11:45am, 12:00pm, 12:15pm, 12:30pm, 12:45pm, 1:00pm, and 1:15pm one child’s sleep positions were not documented for one infant. .0606(g) The following Technical Assistance regarding the violation observed during today’s visit was provided: -Sanitation Inspections Reports: As a reminder an approved sanitation inspection report is required to be completed annually from when the last sanitation inspection was completed. I suggested contacting the Environmental Health office at least four to six weeks prior to when the sanitation inspection expires. The child care rule reference is 10A NCAC 09 .0304 On-going Requirements for a License (b). -Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart from the child care facility each day to ensure accurate records are kept. I suggested creating a system to check during the mid-day to ensure arrival times have been recorded. I also suggested as a closing activity having a staff member to check to make sure departure times have been recorded. -Entrapment Issues (Playground Fence) We discussed repairing the fence to ensure the opening at the bottom of the fence is less than 3.5 inches. I suggested reviewing with the staff the playground inspection form and to have staff inform you when hazards on the playground are found and so hazards can be addressed immediately. -Safe Sleep Checks I suggested reviewing with staff on how to complete the safe sleep checks record properly. I also suggested creating a plan to check safe sleep records daily and/or weekly to ensure documentation is recorded accurately. -Plastic Materials You, Angela Battle, Administrator, asked a question concerning baby wipes in plastic containers. We discussed to look at the plastic diaper wipes containers to make sure no warnings, such as keep out of reach of children, are written on the packaging. I suggested purchasing plastic pencil cases and place diaper wipes in the container and put the extra wipes in the plastic container either 5 feet off the ground and/or under locked storage. I discussed with you that plastic bags, materials that could be torn apart and toy parts small enough to be swallowed must be inaccessible to children under three years of age. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was eighty percent (80%). Please note any violations cited during future visits will negatively impact your compliance history. Dinah Brown, Child Care Consultant, accompanied on the visit today. You, Angela Battle, Administrator, assisted me with the visit today. The NC Secretary of State website was viewed on November 1, 2023, and Dulatown Outreach Center, Inc.. was listed as current and active. The facility currently operates with a 3-stars rated license issued on June 24, 2020, earning five (5) points in staff education, two (2) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care and enhanced ratios. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on August 14, 2023. Sanitation Inspection: The last sanitation inspection was completed on October 31, 2022. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last shelter-in-place drill was completed on August 28, 2023, at 2:30pm and recorded. The last playground inspection was completed on October 23, 2023. The last fire drill was completed on October 25, 2023, at 9:30am and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. Staff and children were observed during indoor free play, story time, naptime, and preparing for lunch. Files Reviewed: Children’s files, program files, and staff files were monitored today. Three (3) children's files were reviewed during today’s visit. Two (2) staff file were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violations were observed during the walk through today. I discussed these violations with you in detail at the end of the visit.
