You, Charles Quick, Director, assisted me with the visit. Your program currently operates with a three-star license, issued November 9, 2023, earning 5 points in the education component, 2 points in the program standards component meeting enhanced space. Prior to the visit the compliance history score was 96%. The NC Secretary of State website was reviewed today and Learning Bridge Development Center, LLC was listed as current- active. Child Care Consultant, Natalie Pond, assisted with the visit today. The restrictions for their license were in compliance. Twenty-nine (29) children are enrolled and twenty-seven (27) children ages 1-4 were present today. A walk-through of the facility was completed today and all approved spaces were monitored for compliance. I observed children in the outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play. The caregivers were interacting and meeting the developmental needs for each of the children. One new staff member has been hired and all required paperwork was observed. Fire and sanitation inspections remain current. The following violation was observed. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. There was no documentation of a fire drill occurring in August 2025. .0604(t); .0302(d)(5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before October 1, 2025, must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. CLEAN CLASSROOMS FOR CAROLINA KIDS - Your facility’s status, according to the Clean Classrooms for Carolina Kids website, shows the following: • Water Testing-Completed in December 27, 2023. • Lead-Based Paint-November 25, 2024 - no hazards were observed • Asbestos-November 25, 2024 - no hazards were observed Ensure you keep all paperwork that you receive regarding this matter, and it may be requested to verify compliance. The allegations are as follows: There is a concern that a child was not treated in a nurturing manner. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. Files for new staff were reviewed and contained all the required information. Additionally, I observed the indoor and outdoor space used by the children. Children were observed in free play with age-appropriate materials and activities both indoors and outdoors. Staff/child interactions were appropriate and nurturing. Children bring lunch from home. Staff child ratios were monitored and found as follows: Space 1 – one year olds – 1:5 Space 2 – two year olds – 1:7 Space 3 – two and three year olds – 2:8 Space 4 – four and five year olds – 1:11 Allegation: There is a concern that a child was not treated in a nurturing manner. **On Tuesday, April 16, 2024, we received a self-report from Director, C.J. Quick. A parent was dropping his child off in space 3 at approximately 9:20 am. Mr. Quick was working in the outdoor play space when the parent came back out of the building with his son and stated he did not feel comfortable leaving his son. He stated the teacher in space 3 seemed “stressed out and looked worn out.” He further stated that the teacher in space 3 picked up a child and put her down where she wanted her. The parent stated he did not like the look on the teacher’s face. Mr. Quick asked the teacher what happened, and she admitted she did not use “gentle hands.” I was able to speak to the parent this morning and he confirmed the allegation. I interviewed the teacher in space 3. Per her report, on Tuesday, April 16, 2024, she was working in the preschool classroom by herself with a small group of children 2-3 years of age. The children were in group circle time, kicking, tripping one another, and throwing toys. She said was using a frustrated tone with the children because they were not listening to her. She said then she separated the children and forcefully sat a child down on the carpet. The teacher said her reactions were in appropriate and she apologized to the parent who observed her responding in a non-nurturing manner. Based on information received from interviewed staff, my investigation, and my observations, the finding regarding the allegation of staff not interacting with a child in a nurturing and caring manner was substantiated.
