The NC Secretary of State website was viewed on September 6, 2023, and It’s a Small World Daycare was not found due to being individually owned. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. Your program currently operates with a four-star license effective June 30, 2022, earning four (4) points in staff education, five (5) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhanced ratios, and maximum capacity of 52 children and 5 adults per septic tank restrictions for Building #1 and in Building #2 the maximum capacity is 15.. During today’s visit, the children were observed outdoor gross motor, teacher directed activities, singing, art activities, lunch, and naptime. Inspections/Emergency Drills: The most recent sanitation inspection for your facility was conducted on March 18, 2025. A superior sanitation classification was issued with eight (8) demerits noted on the grade card. The most recent fire inspection for your facility was conducted on August 6, 2025. Emergency Drills/Playground Inspections: The last lockdown emergency drill was completed on July 17, 2025, at 10:45am and recorded. The last playground inspection was completed on August 4, 2025. The last fire drill was completed on August 1, 2025, at 10:35am and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. Files Reviewed: Children’s files, program files, and staff files were monitored today. Five (5) children's files were reviewed during today’s visit. One (1) staff file was reviewed, and two (2) new staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violation was observed during the visit today. I discussed this violation with you in detail at the end of the visit. Violation Number Comment Rule 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. In the open kitchen separated from Space #1 by a half wall and gate, a Diet Mt. Dew drink bottle was on the counter and could be seen by the children in Space #1. .0901(i) The following Technical Assistance regarding the violations observed during today’s visit was provided: Role Modeling Healthy Habit: I reminded you that drinks, such as soft drinks, must be kept in a container with no business logos and/or manufacturing labels can be seen by children in care. I suggested that you speak to all staff and remind them to keep in an approved container. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-seven percent (97%). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Consultation: -Provider Access to ABCMS I reviewed the ABCMS Portal Roster for your child care program on August 21, 2025, and you have linked five (5) staff to the facility’s ABCMS Portal page. I reviewed for accuracy that staff were linked in the ABCMS Portal to the staff and training worksheet you provided. I discussed with you that two staff needs to be added to your Business ABCMS Portal. We discussed that the two staff are locked out of the NCID accounts and will have to resolve this issue before they can be added. I reminded you that when new staff are hired and/or when staff are terminated you are required to update the ABCMS Portal Page. -Child Care Rule Effective January 1, 2024 Lead Testing: Child Care Rule .0601(f)- Safe Environments We discussed the test results that were available for review for your facility on the Clean Classrooms for Carolina Kids website for lead testing water testing, lead-based paint testing, and asbestos testing for Building #1M. The lead water testing results were completed on May 25, 2025. On August 20, 2025, I checked the Clean Classrooms for Carolina Kids website, and it stated that your facility had no results available for review for lead-based paint or asbestos testing. We discussed this issue and you stated that you will contact the Clean Classroom for Carolina Kids to find out what documentation is needed to complete this task For Building #2, the lead testing water results were conducted on March 6, 2025. The Clean Classrooms for Carolina Kids™ team determined that It's A Small World Daycare 2 was exempt from the lead-based paint assessment and asbestos requirements. The facility provided the appropriate documentation to show all buildings were built after 1978 and no lead-based paint was used in order to meet the building age exemption criteria per 10A NCAC 41C.1004. No additional action is required by the facility. I reminded you that this task must be completed as soon as possible or a violation may be cited at a future visit. -QRIS Moderation For more information on the new QRIS system, please visit the DCDEE website, homepage click the What’s new Tab and click onto the QRIS System link. I discussed with you the three different pathways available through the new QRIS System and the resources available for review on the DCDEE website. On August 20, 2024, one child’s departure time was not documented and on August 27, 2024, July, one child’s departure time was not documented. 10A NCAC 09 .0302(d)(4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member employed on November 29, 2023, did not have available for review training documentation for completing the Recognizing and Responding to Suspicions of Child Maltreatment by the required due date of February 29, 2024. The documentation available for review was dated March 21, 2024. .1102(g) The following Technical Assistance regarding the violations observed during today’s visit was provided: -Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart the child care facility each day. I suggested creating a system and review with staff of the procedures of recording arrival and departure times. I also suggested as part of the classroom mid-day and closing procedures that staff check the arrival and departure sheets to ensure accurate records are documented. -Documentation of Trainings (Recognizing and Responding to Suspicions of Child Maltreatment): We discussed reviewing all staff files to ensure the required training documentation are on file and available for review by the required date. I suggested creating a calendar of when staff need to complete all required trainings and/or highlight on the staff file checklist when specific training must be completed. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was eighty-nine percent (89) %. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter: You must correct the violations found during today's visit immediately. Please send me a letter verifying compliance by September 11, 2024. You, Tracy Queen, Owner/Administrator, assisted with the visit today. The NC Secretary of State website was viewed on September 6, 2023, and It’s a Small World Daycare was not found due to being individually owned. Your program currently operates with a four-star license effective June 30, 2022, earning four (4) points in staff education, five (5) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhanced ratios, and maximum capacity of 52 children and 5 adults per septic tank restrictions. You, Tracy Queen, Owner/Administrator, stated four school-age children are enrolled in the licensed building and are cared for in Space #1 for afterschool care. Mrs. Queen also stated school-age children are cared for in the unlicensed building on the premises. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on September 29, 2022. Sanitation Inspection: The last sanitation inspection was completed on May 12, 2023. A “superior” classification was issued with twelve (12) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last lockdown drill was completed on September 1, 2023, at 10:37am and recorded. The last playground inspection was completed on September 1, 2023. The last fire drill was completed on September 5, 2023, at 10:32am and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. Staff and children were observed during indoor free play, art activities, lunch, and naptime. Files Reviewed: Children’s files, program files, and staff files were monitored today. Five (5) children's files were reviewed during today’s visit. Two (2) staff files were reviewed and two (2) new staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violations were observed during the walk through today. I discussed these violations with you in detail at the end of the visit. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. In Playground Space #1, three landscaping railroad ties are used at the bottom of the chain link fence at the back of the playground space. One of the landscaping railroad ties has a wooden peg protruding from the top of the landscaping railroad ties and accessible to children. .0601(c) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In Space #3, on September 5, 2023, at 1:45pm and 2:00pm, the sleep positions were not documented for one child. .0606(g) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. The facility does not implement an approved curriculum. .2802(d) The following Technical Assistance regarding the violations observed during today’s visit was provided: Safe Sleep Checks I suggested reviewing with staff how to complete the safe sleep checks properly. I also suggested creating a plan to check safe sleep checks to ensure documentations is recorded correctly. -Playground Hazards As a reminder, playground hazards, such as protrusions, need to address, removed, and made inaccessible to children. -Children’s Health Assessments: As a reminder, children’s health assessment records are required to be on file for review within thirty (30) days of a child’s enrollment. I discussed creating a schedule on a calendar as a reminder for when children’s health assessments and other documentation are due for children’s records. I also suggested using the children’s file checklist to document when required records are due. -Implementing an Approved Curriculum: As a reminder, four-star programs, who care for four-year-old children, must implement an approved curriculum. I asked Mrs. Queen what curriculum the child care facility, It’s A Small World Daycare, was using. She stated the child care facility uses the resource guide North Carolina Foundations for Early Learning and Development. Jennifer Roberts, Lead Child Care Consultant, accompanied me on this visit. The Secretary of State website was checked on August 16, 2023, and your business, It’s a Small World Daycare, was not listed. If any changes need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. On August 3, 2023, the following allegation was received. There are concerns that incident reports are not prepared as required. The allegation was discussed with you and all staff members. You and all staff members had an opportunity to ask questions, state perceptions of the situation and provide pertinent information in response to the allegation. We reviewed the following records during the visit today, classroom attendance, arrival and departure records for children enrolled, incident reports, and incident logs. A walkthrough of the facility was conducted to record enrollment. You, Tracy Queen, Owner/Administrator, stated no school age children are enrolled in the licensed building and are cared for in the unlicensed building on the premises. The unlicensed building has a large sign posted on the front of the building that says, “Queen Bee 2” and a phone number was listed. Jennifer Roberts, asked you if you would like to change the age range of the children listed on your license and you said you wanted to keep it the same because you planned to license the unlicensed building in the future and care for infants and toddlers in that building that is currently unlicensed. Based on observations during the visit, reviewing incident logs and incident reports, the allegation was unable to be confirmed, therefore was unsubstantiated. The following violations unrelated to the allegation were observed today: Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. On August 14, 2023, the departure time for one (1) child was not maintained. The arrival times for forty-eight children were maintained on August 16, 2023, but forty-one children were in attendance during the visit; therefore the departure time was not maintained for seven children on August 16, 2023. 10A NCAC 09 .0302(d)(4) 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. In space #4, one (1) toddler, one (1) year of age was being cared for with ten (10) additional children, three (3) and four (4) years of age. 10A NCAC 09 .0713(a)(6) 1301 Center did not maintain a record of daily attendance. The attendance records were not maintained for each child listed on the arrival and departure records. Fifty-nine children were listed on the arrival and departure records for August 16, 2023, and forty-nine children were listed on the daily attendance records. GS 110-91(9) 1756 Enhanced staff/child ratios and group sizes were not met. In Space #2, at 11am the staff/child ratio was observed as one staff member was caring for ten children. Two children under the age of two years were present in the classroom. The classroom staff/child chart posted stated that children two years old were enrolled. The staff stated all children present today were two years old. Two children’s files were reviewed and two of the ten children in space #2 were one (1) year of age. In space #4, one caregiver was caring for eleven (11) children during indoor free play, prior to lunch, and during nap time, one (1) toddler, one year of age, eight (8) children, three years of age and two (2) children, four years of age. One (1) child’s file was reviewed to verify the age of the child, one (1) year of age. 10A NCAC 09 .2818 The violation(s) documented must be corrected immediately. On or before August 30, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. and what plan will be implemented to prevent these violations from occurring again. I suggested creating a system and review with staff of the procedures of recording sign-in and sign-out times and other required information daily to ensure accurate records are kept. I suggested that each space maintains their own arrival and departure records. -Staff/Child Ratios We discussed the child care rule 10A NCAC 09 .2818 Enhanced Staff/Child Ratios for a Rated License for Child Care Centers which states requirement for meeting enhance ratios and the number of children allowed in each child care space based on the classroom staff to child ratio poster that is posted in each classroom. Child care ratios are based on the age of the youngest child in care. These ratios must be always maintained. I suggest you implement a plan to ensure staff/child ratios are maintained at all times. -We discussed child care rule 10A NCAC 09 .0713(a)(6) regarding Staff/child ratios and grouping of children. Children one (1) year of age may not be grouped with children three (3) years of age and older. We discussed possible solutions for maintaining staff/child ratios and grouping of children when staff are not available to work. -Maintain Accurate Attendance Records We discussed child care rule 10A NCAC 09 .0302(d)(3) and .0304(g) regarding maintaining daily attendance records. The arrival and departure records were reviewed in addition to the attendance report in each space that children are enrolled. Fifty-nine children were listed on the arrival and departure records, the arrival time for fifty-one children had been recorded today, the departure time for three children had been recorded prior to our arrival and forty-one children were observed in the facility during the visit. Consultation: As a reminder, children enrolled in the licensed program must be cared for in licensed building. Children in the licensed building and children in the unlicensed building cannot intermingle. Record Retention: A record of daily attendance must be maintained and available for review. Children who receive subsidy services must have daily attendance recorded. 10NCAC 09.0302(d)(3) Specific arrival and departure times are to be documented. 10A NCAC 09 .0302(d)(4) As a reminder, providers who wish to participate in the Subsidized Child Care Assistance Program are required to enroll through the NC FAST Provider Portal, enroll in Direct Deposit with the state’s direct deposit vendor, and sign the Provider Agreement. To be eligible to participate in the SCCA Program, providers must have a rated license of 3 stars or higher; religious-sponsored programs (also known as G.S. 110 - 106) must have a Notice of Compliance letter and comply with all requirements of Chapter 110 General Statutes Child Care Facilities and Chapter 9 Child Care Rules. In space #1, fifteen children were listed on the attendance report, in space #2, ten children were listed on the attendance report, in space #3, nine children were listed on the attendance report, in space #4, nine children were listed on the attendance report, and in space #5, six children were listed on the attendance report. GS 110-91(9) 1756 Enhanced staff/child ratios and group sizes were not met. In Space #2, at 11am the staff/child ratio was observed as one staff member was caring for ten children. Two children under the age of two years were present in the classroom. The classroom staff/child chart posted stated that children two years old were enrolled. The staff stated all children present today were two years old. Two children’s files were reviewed and two of the ten children in space #2 were one (1) year of age. In space #4, one caregiver was caring for eleven (11) children during indoor free play, prior to lunch, and during nap time, one (1) toddler, one year of age, eight (8) children, three years of age and two (2) children, four years of age. One (1) child’s file was reviewed to verify the age of the child, one (1) year of age. 10A NCAC 09 .2818 The violation(s) documented must be corrected immediately. On or before August 30, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. and what plan will be implemented to prevent these violations from occurring again. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Technical assistance on violations was reviewed with you. Areas discussed included: --Arrival/Departure Times We discussed daily sign-in and sign-out times must be recorded when children arrive and depart the child care facility to ensure accurate records are kept. I reminded you, Tracy Queen, Owner/Administrator, to document who is signing children in and out daily on the record. I suggested creating a system and review with staff of the procedures of recording sign-in and sign-out times and other required information daily to ensure accurate records are kept. I suggested that each space maintains their own arrival and departure records. -Staff/Child Ratios We discussed the child care rule 10A NCAC 09 .2818 Enhanced Staff/Child Ratios for a Rated License for Child Care Centers which states requirement for meeting enhance ratios and the number of children allowed in each child care space based on the classroom staff to child ratio poster that is posted in each classroom. Child care ratios are based on the age of the youngest child in care. These ratios must be always maintained. I suggest you implement a plan to ensure staff/child ratios are maintained at all times. -We discussed child care rule 10A NCAC 09 .0713(a)(6) regarding Staff/child ratios and grouping of children. Children one (1) year of age may not be grouped with children three (3) years of age and older. We discussed possible solutions for maintaining staff/child ratios and grouping of children when staff are not available to work. -Maintain Accurate Attendance Records We discussed child care rule 10A NCAC 09 .0302(d)(3) and .0304(g) regarding maintaining daily attendance records. The arrival and departure records were reviewed in addition to the attendance report in each space that children are enrolled. Fifty-nine children were listed on the arrival and departure records, the arrival time for fifty-one children had been recorded today, the departure time for three children had been recorded prior to our arrival and forty-one children were observed in the facility during the visit. Consultation: As a reminder, children enrolled in the licensed program must be cared for in licensed building. Children in the licensed building and children in the unlicensed building cannot intermingle. Record Retention: A record of daily attendance must be maintained and available for review. Children who receive subsidy services must have daily attendance recorded. 10NCAC 09.0302(d)(3) Specific arrival and departure times are to be documented. 10A NCAC 09 .0302(d)(4) As a reminder, providers who wish to participate in the Subsidized Child Care Assistance Program are required to enroll through the NC FAST Provider Portal, enroll in Direct Deposit with the state’s direct deposit vendor, and sign the Provider Agreement. To be eligible to participate in the SCCA Program, providers must have a rated license of 3 stars or higher; religious-sponsored programs (also known as G.S. 110 - 106) must have a Notice of Compliance letter and comply with all requirements of Chapter 110 General Statutes Child Care Facilities and Chapter 9 Child Care Rules. If you decide that you want to license the building beside the currently licensed building, a completed and approved building, fire, sanitation inspection and lead water test are required. You would then contact your child care consultant for next steps to license the building.
