You, Connie White, Owner/Operator, assisted me with the visit today. The facility currently operates with a one-star rated license issued on September 19, 2013. The program's license restrictions include the following: maximum of 10 children if all children are older than 24 months of age, children in care on ground level only, daytime care, and second shift. Two (2) Children were enrolled during 2nd shift. The last annual compliance visit was conducted on January 8, 2025. You stated that your family child care home was closed from December 22, 2025, through January 4, 2026. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent (100%) as of January 5, 2026. The Secretary of State website was checked on January 6, 2026, and your business Connie’s Little Angels., was not listed. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. You stated you have twelve children enrolled and six children were present today. During today’s visit, the children were observed during indoor free play, art activities, bathroom routines, hand washing, lunch, and naptime. Drills: The most recent fire drill was conducted on January 5, 2026, at 7:10am and recorded. The most recent lockdown emergency drills were conducted on December 1, 2025, at 8:30am and recorded. The most recent playground safety inspection was conducted on January 5, 2026. The fire extinguisher was fully charged and was located on the floor next to the food pantry and on the floor next to the outdoor door. QRIS Modernization Pathway to the Stars Discussion: We discussed your program re-applying for an Environment Rating Scale Re-assessment to maintain your current one-star rated license. You stated that you are not interested in applying for a star rated license with the new QRIS Modernization Pathways to the Stars. Files Reviewed: Children’s files, program files, and staff files were monitored today. Ten (10) children's files were reviewed during today’s visit. Your file was reviewed during today’s visit. You were given an opportunity to ask questions. You signed the visit summary during the visit today and received a copy at the end of the visit. A Verification of Required Information for Operator and Additional Caregivers and the Family Child Care Operator’s Statement of Responsibility forms were completed during today’s visit. Each document was verified and signed by you during today’s visit. You stated that you and your husband are the only people living in the residence. You also stated that your two sons do not live at your residence. Lunch was observed and consisted of ham, cheese cubes, orange slices, pinto and great northern mixed beans, bread, and milk. The following violation was observed during today’s visit. Violation Number Comment Rule 1920 Indoor and outdoor area was not checked daily for debris, vandalism, broken equipment, and animal waste. On outdoor gross motor space used by children for gross motor play, there were to pieces of broken equipment with hazards accessible to children. One red faded trike with cracked tire and a broken top of the plastic toy lawnmower. .1719(a)(17) Comments: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance provided for the violations cited during today’s visit. Playground Checks: I discussed with you do conduct daily playground checks to ensure all toys and equipment are in good repair. I also discussed with you to remove all broken toys and equipment prior to children accessing the outdoor gross motor area. The violation was corrected during the visit. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Consultation: Criminal Background Checks/Qualifying Letters: I reviewed with you the child care rule 10A NCAC 09 .2703 CRIMINAL HISTORY RECORD CHECK REQUIREMENTS FOR CHILD CARE PROVIDERS (q) which stated that any individual over the age of 15 who resides, as defined in 10A NCAC 09 .0102(40), in a child care facility on either a temporary or permanent basis shall comply with the requirements of Paragraph (a) of this Rule within 5 days of either moving into the facility or within 5 days after their 16th birthday. ABCMS Portal: I shared with you that your facility’s ABCMS Provider Portal roster was reviewed and is current. -Technical Assistance: Reach out to your local Resource and Referral Agency for additional technical assistance and support. Child Care Connections of Cleveland County at (704) 487-7397. . Lead Testing: We discussed the test results that were available for review on the Clean Classrooms for Carolina Kids website for lead testing water testing, lead-based paint testing, and asbestos testing for your family child care home. The lead water testing results were completed on March 8, 2024. On January 6, 2026, I checked the Clean Classrooms for Carolina Kids website, and it stated that your facility had was exempt for lead-based paint or asbestos testing. Lead water testing must be completed every three years. I discussed this with you and staff during the visit today and offered suggestions on providing a safe environment during naptime.
