Stephanie Hasty, provider assisted me with the visit. Your program currently operates with a four- star license, issued September 22, 2022, earning 7 points in education, 2 points in program for completing a 3-month self- study and 1 quality point for completing a 30 hour or longer business training. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. Your last annual compliance visit was conducted January 28, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was 100% as of today. Your last fire inspection was conducted on November 5, 2025, and your facility was approved for daytime and nighttime care only. I visited each indoor and outdoor space. Children were cared for in a nurturing and caring manner. The license restrictions were maintained and found in compliance. The children were observed during teacher directed activities. Lunch consisted of lima beans, sliced turkey, mac and cheese, blue berries and milk. The following violations were documented: Violation Number Comment Rule 1855 The operator did not review the FCCH EPR Plan with additional caregivers on an annual basis. Documentation was not on file for W. Smith (additional caregiver). .1729(a )(10) 1891 Individuals providing care to children did not receive 16 hours of orientation prior to beginning caregiving activities. Documentation was not on file for W. Smith (additional caregiver). .1729( c) 2025 Operator and/or staff members did not complete a professional development plan within one year of employment. Documentation was not on file for W. Smith (additional caregiver). .1703(i) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before 1/21/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violation was corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. All Family Child Care Home providers are now required to test their water for lead poisoning. 10 A NCAC 09. 1707 Building Requirements (2) all family child care homes must be free of lead poisoning hazards as defined in G.S. 130A (7) and asbestos hazards; You submitted your water for testing to Clean Water for Carolina Kids on 5/7/24. Make sure to follow through on all instructions. Today we discussed moving forward with renewing your star rated license. You decided to wait to decide on which programmatic pathway you want to choose. Technical assistance was given on the following: • I suggest dedicating a designated time to review the different programmatic pathways. • Schedule a phone call or TEAMS meeting with me your consultant when you do not have children in attendance, to discuss which pathway would be best for your program. • Meet with at least 3 other FCCH providers to discuss which pathway they chose and why.
