Vevely Malloy, director assisted me with today’s visit. Ms. Malloy had to leave early. Davia DeBerry, assistant director, revied and signed today's visit summary. Your program currently operates with a five star license, issued April 11, 2022, earning 7 points in education, 7 points in program ( meeting enhanced space, ratios minus 1, and an ERS score of 5.0 with no room lower than a 4.0) and 1 quality point for having all lead teachers with an AAS or higher in early education: Restrictions include: No cooking allowed in building; Day time care only. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four year-old children are enrolled. The license was posted, and the restrictions were in compliance. Due to inclement weather, your facility operated on a 2 hr delay today. Your fire and sanitation inspections remain current. Your last sanitation inspection remains current and was completed on March 26, 2025, with a “Superior” classification. Your last fire inspection remains current and was conducted October 10, 2025, and your facility was approved for daytime and overnight care only. The center's compliance history was reviewed with the operator. The program’s compliance history was 97% as of today. The NC Secretary of State website was reviewed today, and STATELINE COMMUNITY ACTION INC was listed as current- active. I visited each indoor and outdoor space. The children were observed during teacher directed activities and hygiene routines. Lunch consisted of turkey, cheese, WG bread, fruit cup, chips, and milk. The following violations were documented during today’s visit: Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Documentation was not on file for one staff member. .0701(a) 1769 The health assessment did not include a dental screening. Documentation was not on file for three children. .3005 (a)(5) 1802 The child care provider and/or household member who incurred pending charges, indictments, or convictions since their last qualification letter was issued by the Division, did not notify the operator of these charges within five business days or before returning to work. The operator did not notify the Division of these pending charges, indictments, or convictions within one business day of being notified by the child care provider or household member. Two staff members had not been added to the system. G.S. 110-90.2 & .2703(m) 1898 Staff did not complete the health and safety training within one year of employment. Your program currently operates with a five – star license effective April 11, 2022, earning (7) points in the education component, (6) points in the program standards component (enhanced ratio/space and an average ERS score of 5.0/ no room lower than 4.0) and 1 quality point by (all lead teachers have an AAS or higher in early childhood or child development). Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety six percent as of today. The NC Secretary of State website was reviewed on today and State Line Community Action Association, Inc. was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were preparing for their Christmas production, and personal care routines, teacher directed and free play activities. The caregivers were interacting and meeting the developmental needs for each of the children. One new staff file was monitored for compliance. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Vevely Malloy, director assisted me with the visit. Your program currently operates with a five star license, issued April 11, 2022, earning 7 points in education, 7 points in program ( meeting enhanced space, ratios minus 1, and an ERS score of 5.0 with no room lower than a 4.0) and 1 quality point for having all lead teachers with an AAS or higher in early education: Restrictions include: No cooking allowed in building; Day time care only. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The facility was closed for spring break. There were no children in attendance during today’s visit. I will be back when children are in attendance to complete today’s visit. Due to a lack of internet connection, a handwritten copy of today’s visit was left with you at the conclusion of the visit. The sanitation inspection was completed March 14, 2024, with a “Superior” classification. The last fire inspection was conducted October 25, 2023, and your facility was approved for daytime and overnight care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety- four percent as of today. The NC Secretary of State website was reviewed today, and STATELINE COMMUNITY ACTION INC was listed as current- active. The following violations were documented during today’s visit: Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child was enrolled 8/22/23. The child’s medical report was dated 3/7/22. GS110-91(1) 1769 The health assessment did not include a dental screening. Documentation was not on file for one child. The child’s enrollment date was 8/28/23. .3005 (a)(5) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before 4/16/24, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. We discussed the fall zone for the swings. Your fall zone will need to measure at least 14 feet from the seat of the swings in front and behind the swings. The sides will need to measure at least 6 ft. Please see the rule below: 10A NCAC 09 .0605 OUTDOOR LEARNING ENVIRONMENT IN CHILD CARE CENTERS (m) (2) Swings shall have protective surfacing that extends two times the length of the pivot point to the surface below. The surfacing shall be to the front and rear of the swing. (3) Tot swings shall have protective surfacing that extends two times the length of the pivot point to the bottom of the swing seat. The surfacing shall be to the front and rear of the swing. Tot swings are defined as swings with enclosed seats. Effective July 1, 2023 25 (4) Tire swings shall have protective surfacing that extends a distance of six feet plus the measurement from the pivot point to the swing seat and six feet to the side of the support structure. Davina DeBerry, administrative assistant assisted me with the visit. Your program currently operates with a five – star license effective May 24, 2022, earning (7) points in the education component, (6) points in the program standards component (enhanced ratio/space and an average ERS score of 5.0/ no room lower than 4.0) and 1 quality point by (all lead teachers have an AAS or higher in early childhood or child development). Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety five percent as of today. The NC Secretary of State website was reviewed on today and State Line Community Action Association, Inc. was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were preparing to go outside, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. New files for staff were reviewed. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. The following staff files were reviewed for compliance: D. McNeil and C. Alford Fire and sanitation inspections have not occurred since the last visit and remain current. The following violations were documented: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Documentation was not on file for one staff member. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. I highly suggest allowing the children an opportunity to play with quite games at the table and offer mats for those needing a nap after lunch. It is important that all children are afforded an opportunity to rest on a cot/mat when they desire to take a nap. The children in space #2, were preparing to go outside. The children were engaged in toileting and personal hygiene practices. The meal head count attendance sheet did not match the number of children in the room. 14 children ages 5 years old- 5 years old were checked off as present for the attendance and breakfast today. 13 children attended class today, as stated by the teacher. When taking attendance for preschool age children it is highly suggested to take a “roll call.” Call out each child’s name and as the answer mark them present or absent on your attendance sheet. It is very important to make sure all attendance sheets are accurate and complete for ALL official purposes.
