This program currently operates with a temporary license effective December 15, 2025, to June 15, 2026. Applicable child care requirements were discussed with you, Kyra Cummings, Lead Teacher. All indoor and outdoor areas approved for child care were monitored during this visit. Their Temporary License was posted, and they were in compliance with all restrictions noted on the license. Upon my arrival children were observed engaging in free play and routine activities. Teachers demonstrated positive interactions with children and were observed assisting and supervising children along with maintaining staff/child ratios at all times. Children were also observed playing outside. A teacher was observed changing a child’s diaper following the correct procedures. Children were also observed eating lunch and during nap time. For lunch the children were served beef ravioli with tomato sauce, corn, diced pears, and milk. They do participate in the Child and Adult Care Food Program. During this visit, I monitored two (2) new staff files. I did not re-monitor previous staff files; however, I did observe that all staff files were available for review and I reviewed the Staff and Training worksheets that were completed on March 10, 2026, to verify that all staff’s CPR/FA, criminal records checks, and SIDS training were current. In addition, I monitored program record files. Please note it is your responsibility to ensure that all required information is on file, current, and available for review at any time. During my visit conducted on March 10, 2026, three (3) trees with red berries were observed on the toddler playground #2. The berries were accessible to the children. This was not cited as a violation during that visit because the type of tree and whether the berries were toxic could not be confirmed. However, they were advised to contact a professional landscaper or other qualified professional to confirm if the berries were toxic. During this visit I observed that all three (3) trees had been cut down and removed from the playground. You stated that it was never determined if the berries were toxic but to be on the safe side a decision was made to cut down the trees and remove them from the playground. Overall attendance and enrollment were collected today. The attached space, capacity and enrollment worksheet reflects the enrollment and attendance information along with the staff that were present and caring for children today. At the conclusion of this visit, the visit summary was reviewed with you, and you were provided with an opportunity to ask questions, seek clarification, and request technical assistance. You were also provided with a copy of this visit summary to keep for your records. Violation Number Comment Rule 532 All children were not held or placed in feeding chairs or other appropriate apparatus to be fed. An infant was observed laying in a bouncy seat with a bottle. This was corrected during the visit when a caregiver removed the infant from the bouncy seat and held them while bottle feeding. 10A NCAC 09 .0902(b) 861 Prohibited styrofoam and foam rubber products were accessible to children under 3 years of age and/or approved foam products were used without proper supervision. Children under the age of 3 had access to a foam ball while playing on the playground. This was corrected during the visit when the ball was removed from the playground. .0604(q) 871 Center staff did not comply with the safe sleep policy. A wedge was observed around an infant while they were laying on a cot. This was corrected during the visit when the wedge was removed from the cot. 10A NCAC 09 .0606(a) The three (3) violations documented above were corrected immediately during this visit; therefore, you are not required to submit a letter of compliance to me. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit may negatively affect your compliance score. As of today, your compliance history is 95%. Your compliance history was reviewed with you, and you were also given a copy to keep for your records. Rated License: I discussed with you the Quality Rating and Improvement System (QRIS) during the visit I conducted with you on March 10, 2026. During that visit you informed me that you would go with Pathway #2: Classroom & Instructional Quality for Child Care Centers. During this visit you requested additional time to complete all the requirements for Pathway #2 and to also allow all staff to get their education information processed by WORKS. You stated that you should be able to complete all requirements by May 15, 2026. Please note that all requirements for Pathway #2 should be completed by the agreed upon date. Your Temporary License will expire on June 15, 2026, and extensions are not granted. I also discussed with you that it is imperative that all staff education information be uploaded and evaluated in WORKS no later than May 15, 2026. Failure to do so may affect the star level that your program receives. I will process your rated license using the information at that time. Keep in mind that in order to continue to be eligible for subsidy you will need to transition to at least a 3-star rated license at the end of your temporary time period. You requested clarification regarding serving almond milk to children who can't tolerate cow's milk. I discussed with you that almond milk is not considered nutritionally equivalent to cow's milk unless it meets specific USDA nutrient requirements. Fortified soy milk can meet USDA requirements if it is nutritional equivalent to cow's milk. Before serving soy milk you should check the label to ensure it meets the USDA standards. This program is currently operating under a Temporary License effective December 15, 2025 to June 15, 2026. Applicable childcare requirements were discussed with you, Nancy Higgins, Administrator. During this visit I monitored all indoor and outdoor areas of the facility. Also, during this visit children were observed actively engaging with their assigned teacher and participating in various free-choice centers, including fine motor activities, books, blocks, and dramatic play. Breakfast and nap time routines were also monitored. Positive and developmentally appropriate interactions between staff and children, staff/child ratios, and supervision were monitored throughout the visit. The temporary license was posted and in compliance with all restrictions noted on the license. For breakfast the children were served sausage patties, grits, mixed fruit (diced), and milk. Three (3) children’s files and eight (8) staff files were reviewed during this visit. The owner operates two (2) additional locations so two (2) staff were transferred from those locations to this location, one (1) of which was the administrator. In addition, all program records were reviewed during this visit. Overall enrollment and attendance information were collected. The attached Space Capacity and Enrollment Worksheet reflect this information, along with the names of staff present and actively caring for children during this visit. At the conclusion of the visit, the visit summary was reviewed with you, and you were provided with an opportunity to ask questions, seek clarification, and request technical assistance. You were also provided with an electronic copy of this visit summary for your records. Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. A current schedule was not posted in classrooms #1, #5, and #6. This was corrected during the visit when a current schedule was posted in each of those classrooms. GS 110-91(12);.0508(a) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. Children in classroom #5 didn't wash their hands before they were served breakfast. 15A NCAC 18A .2803(c) 861 Prohibited styrofoam and foam rubber products were accessible to children under 3 years of age and/or approved foam products were used without proper supervision. A foam rubber block was accessible to children who were one year old in space #6. This was corrected during the visit when the foam rubber block was removed from the classroom. .0604(q) The three (3) violations documented above must be corrected immediately. The following violations were corrected during this visit #415 and #861. A signed and dated letter of compliance must be submitted to me no later than February 11, 2026, clearly detailing how violation #608 has been corrected. During the visit you were provided with information about the three (3) QRIS Pathways. It was discussed with you that you should begin to review the different pathways and choose the best pathway for your center. A new Pathways to the Stars Rules Roll Out module is now available in the DCDEE Moodle digital learning platform. To access the links in the application you can visit: the DCDEE website at: www.ncchildcare.nc.gov and click on the provider tab and then scroll down and click on the provider documents and forms and then click on the Application for Assessment for a Rated License for Centers. During this visit I also demonstrated how to access the QRIS information, and the application which are both located on the DCDEE website: www.ncchildcare.nc.gov. Questions about QRIS Modernization (Reform) may be sent to DCDEE_QRIS@dhhs.nc.gov. I will process your star rated license using the education information that is in WORKS at that time. So, it is imperative that all staff education information has been processed through WORKS no later than May 2026. Keep in mind that in order to be eligible for subsidy payments at the end of the temporary time period, your center must transition to a three to five star rated license. As requested, clarification was provided regarding requirements for staff responsible for the care of infants. It was discussed that all staff who are responsible for caring for infants must have SIDS training. If SIDS training has not be completed, it must be completed within the first two (months) of caring for infants. In addition it was discussed that at least one (1) staff member who has completed SIDS training must be present in the infant classroom at all times. I also discussed with you that because you have completed EPR training you are required to upload your EPR plan in the NC Risk Management Portal within four months of becoming licensed. This program is currently operating under a Temporary License effective December 15, 2025 to June 15, 2026. Applicable childcare requirements were discussed with you, Nancy Higgins, Administrator. During this visit I monitored all indoor and outdoor areas of the facility. Also, during this visit children were observed actively engaging with their assigned teacher and participating in various free-choice centers, including fine motor activities, books, blocks, and dramatic play. Breakfast and nap time routines were also monitored. Positive and developmentally appropriate interactions between staff and children, staff/child ratios, and supervision were monitored throughout the visit. The temporary license was posted and in compliance with all restrictions noted on the license. For breakfast the children were served sausage patties, grits, mixed fruit (diced), and milk. Three (3) children’s files and eight (8) staff files were reviewed during this visit. The owner operates two (2) additional locations so two (2) staff were transferred from those locations to this location, one (1) of which was the administrator. In addition, all program records were reviewed during this visit. Overall enrollment and attendance information were collected. The attached Space Capacity and Enrollment Worksheet reflect this information, along with the names of staff present and actively caring for children during this visit. At the conclusion of the visit, the visit summary was reviewed with you, and you were provided with an opportunity to ask questions, seek clarification, and request technical assistance. You were also provided with an electronic copy of this visit summary for your records. Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. A current schedule was not posted in classrooms #1, #5, and #6. This was corrected during the visit when a current schedule was posted in each of those classrooms. GS 110-91(12);.0508(a) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. Children in classroom #5 didn't wash their hands before they were served breakfast. 15A NCAC 18A .2803(c) 861 Prohibited styrofoam and foam rubber products were accessible to children under 3 years of age and/or approved foam products were used without proper supervision. A foam rubber block was accessible to children who were one year old in space #6. This was corrected during the visit when the foam rubber block was removed from the classroom. .0604(q) The three (3) violations documented above must be corrected immediately. The following violations were corrected during this visit #415 and #861. A signed and dated letter of compliance must be submitted to me no later than February 11, 2026, clearly detailing how violation #608 has been corrected. Failure to submit your compliance letter by the stated deadline or failure to contact me regarding a revised timeline may result in a follow-up visit, and violations could be cited. If you are unable to meet the required deadline of February 11, 2026, you must contact me prior to that date with a proposed timeline for completing the required corrections. In some cases, this time frame may be extended. Important Notice: If your compliance letter states that corrections have been made and it is later determined they were not corrected, this could be considered falsification of information, and an administrative action could be taken against your license. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit, could negatively affect your compliance score. As of today, your compliance history is 92%. Your compliance history was reviewed with you, and you were also given a copy to keep for your records. Rated License Assessment: Pathways to the Stars: The NC Quality Rating and Improvement System (QRIS) QRIS is a systemic approach to assess, improve, and communicate the level of quality in child care. It encourages programs to engage in continuous quality improvement and families may use the star rating to make informed decisions when choosing a child care facility. During the visit you were provided with information about the three (3) QRIS Pathways. It was discussed with you that you should begin to review the different pathways and choose the best pathway for your center. A new Pathways to the Stars Rules Roll Out module is now available in the DCDEE Moodle digital learning platform. To access the links in the application you can visit: the DCDEE website at: www.ncchildcare.nc.gov and click on the provider tab and then scroll down and click on the provider documents and forms and then click on the Application for Assessment for a Rated License for Centers. During this visit I also demonstrated how to access the QRIS information, and the application which are both located on the DCDEE website: www.ncchildcare.nc.gov. Questions about QRIS Modernization (Reform) may be sent to DCDEE_QRIS@dhhs.nc.gov. I will process your star rated license using the education information that is in WORKS at that time. So, it is imperative that all staff education information has been processed through WORKS no later than May 2026. Keep in mind that in order to be eligible for subsidy payments at the end of the temporary time period, your center must transition to a three to five star rated license. As requested, clarification was provided regarding requirements for staff responsible for the care of infants. It was discussed that all staff who are responsible for caring for infants must have SIDS training. If SIDS training has not be completed, it must be completed within the first two (months) of caring for infants. In addition it was discussed that at least one (1) staff member who has completed SIDS training must be present in the infant classroom at all times. I also discussed with you that because you have completed EPR training you are required to upload your EPR plan in the NC Risk Management Portal within four months of becoming licensed. You can access that portal at: https://rmp.nc.gov/portal/portal.aspx I also discussed with you preservice requirements for lead teachers and teachers. Technical Assistance: Technical assistance was provided regarding handwashing requirements to prevent the spread of germs. Technical assistance was provided regarding safety concerns associated with foam rubber materials for children under the age of three (3).
