Jaqueline Breeden, provider assisted me with the visit. Your program currently operates with a three – star license effective August 2, 2022, earning (7) 6points in the education component, (2) points in the program standards component (for completing a 3-month self-study) and 1 quality point by (serving no more than two infants under one year of age). The last annual compliance visit was conducted 1/9/25. The last fire inspection was conducted on 9/23/25, and your facility was approved for daytime care and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was 98% as of today. I visited each indoor and outdoor learning environment. Children enrolled were engaged in free play activities. The children were responsive. The provider tended to each child’s needs. You stated that you are not transporting children at this time. When you decide to start back transporting children, make sure to follow all the state and local rules and regulations regarding transportation. You stated that you do not give medication at your FCCH. Lunch consisted of hot dogs, wheat bread, baked beans, apple sauce, and milk. The following violations were documented: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. The laundry room door was unlocked. Inside there were 14 laundry detergents, 3 aerosol cans, and a container of bleach. .1719 (a)(7) 716 Electrical outlets not in use were not covered. In the playroom, one outlet on the back wall near the steps was uncovered. This was corrected by putting in a plug cover. 10A NCAC .1719(a)(27) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. Documentation was not on file for one child. GS 110-91(1); .1721(a)(2) 1831 A valid qualification letter was not on file and available for review at the facility. Today I verified in the ABCMS portal that your qualification letter is valid and expires 10/21/30. However, a copy of your qualification letter was not on file during today's visit. Locked areas shall include those that are unlocked with a combination, electronic, or magnetic device, key, or equivalent locking device.
