This is a religious sponsored center that is operated under a G.S. 110-106. The Notice of Compliance was issued on September 15, 2023. The restrictions include daytime care only, children under 2 ½ years old in rooms with direct exits only. Capacity 59. Lu Ann Rhyne, assistant director, assisted me with today’s visit. The last annual compliance visit was completed on April 3, 2025. Staff/child ratio, group size, supervision, use of licensed space, space capacity and license restrictions were monitored. The last sanitation inspection was conducted on March 4, 2026, earning a superior rating. Your fire inspection remains current and was conducted on September 2, 2025. The program’s compliance history was 98% as of today. The NC Secretary of State website was reviewed on today and Stewartsville Baptist Church, Inc. was listed as current-active. I visited each indoor and the outdoor areas. The children were observed playing in center areas and eating lunch. The teachers attended to each child in a nurturing and positive manner. E. Baines background check expired August 4, 2019. A new valid qualification letter will need to be on file within 14 days. If a valid qualification letter is not on file within 14 days, she will not be able allowed on the premises. Ms. Baines was not at work today. The director will need to take the following trainings: - EPR - SIDS - Playground Safety - First Aid - CPR Lunch consisted of fish sticks, mac-n-cheese, carrots, apple sauce, and milk. The following violations were observed during today's visit: Violation Number Comment Rule 548 For children 2 years of age and older, a minimum of 60 minutes of outdoor time throughout the day was not provided. Children ages 0-12 in care for less than five hours per day was not provided at least 30 minutes of outdoor time throughout the day. Dim lighting makes these cues difficult to detect. Immediate Response to Distress Children may cough, vomit, experience breathing difficulty, or become tangled in bedding. The restrictions include daytime care only, children under 2 ½ years old in rooms with direct exits only. Capacity 59. Lu Ann Rhyne, assistant director, assisted me with today’s visit. The last annual compliance visit was completed on April 3, 2025. Staff/child ratio, group size, supervision, use of licensed space, space capacity and license restrictions were monitored. The last sanitation inspection was conducted on March 4, 2026, earning a superior rating. Your fire inspection remains current and was conducted on September 2, 2025. The program’s compliance history was 98% as of today. The NC Secretary of State website was reviewed on today and Stewartsville Baptist Church, Inc. was listed as current-active. I visited each indoor and the outdoor areas. The children were observed playing in center areas and eating lunch. The teachers attended to each child in a nurturing and positive manner. E. Baines background check expired August 4, 2019. A new valid qualification letter will need to be on file within 14 days. If a valid qualification letter is not on file within 14 days, she will not be able allowed on the premises. Ms. Baines was not at work today. The director will need to take the following trainings: - EPR - SIDS - Playground Safety - First Aid - CPR Lunch consisted of fish sticks, mac-n-cheese, carrots, apple sauce, and milk. The following violations were observed during today's visit: Violation Number Comment Rule 548 For children 2 years of age and older, a minimum of 60 minutes of outdoor time throughout the day was not provided. At the completion of the visit, the Visit Summary was reviewed with Ms. McMillian. Ms. Blair Chavis, last day of employment as daycare director was January 31, 2024. Melinda McMillian’s start date as director was August 12, 2024. Ms. McMillian assisted me with today’s visit. Make sure to email me a copy of the Preservice for Administrator form as soon as possible. The center's compliance history was reviewed with the operator. The program’s compliance history was 93% as of today. The children were observed in outdoor activities, to include large group activities and teacher directed activities. Lunch consisted of hot dogs, baked beans, oranges, bread and milk. Repeat violations of child care requirements can lead to an administrative action. During today’s visit you stated that you had opened another classroom. This classroom has not received the required building and sanitation inspections. A fire inspection was completed to include the fourth classroom on 8/15/23. There was a miscommunication about the process required to add this classroom to your licensed space. Today you were able to close this classroom and add the enrolled children in space #1 and space #3. You and the School Administrator have contacted all inspectors and will submit the approved inspections specifying that Space #4 which is also designated as Space E21 on the original floor plan located on file with DCDEE to me before this space can be included in your notice of compliance. Today, I measured space #4 and it will be submitted as supporting documentation with your approved inspections to increase your current capacity and issue a new notice of compliance. You may use the new space when the Notice of Compliance is issued. There is a new resource available through the Healthy Social Behaviors (HSB) Project that supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina.
