This facility operates with a One-Star License, effective January 1, 2008. The last sanitation inspection was completed on May 21, 2025, earning a “Superior” classification. The last fire inspection was completed on October 16, 2025, and was approved for day time only. The last Annual Compliance visit was completed on February 5, 2025. Head Master, W Trevino, was present and available to assist. Fifteen (15) children age ranging three (3) to five (5) years of age were present. Children were observed in free choice play, teacher directed activities, and toileting and handwashing routines. All staff members interacted with children in a positive manner. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records, and all program records were monitored. All program records and children’s records were found to be in compliance. One (1) violation was documented for staff record keeping. Due to internet connectivity issues, a one-page, handwritten visit summary was reviewed and left with you at the completion of the visit. A detailed, computer-generated visit summary will be emailed to you. The following violations of child care requirements were documented during the visit. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. Nineteen (19) children age ranging three (3) to four (4) years of age were present. Nine (9) children out of the nineteen (19) were observed in an unlicensed and unapproved space. Children were observed in free choice play, teacher directed activities, and toileting and handwashing routines. All staff members interacted with children in a positive manner. The unlicensed space was observed and measured during the visit. Davis. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First Aid training was not completed before is expired for W. Biggs-Trevino. I explained to you, W. Briggs-Trevino, that the children utilizing the unapproved space could not continue to be in the space due to it being unlicensed. You stated, “I am not going to close this classroom down, but I will continue to do what it takes to get it licensed.” Measurements of the space were taken during the visit. STAFF RECORD KEEPING Administrator, W. Briggs-Trevino, did not completed CPR and First Aid training before the last training expired. The last CPR and First Aid training expired 8/1/2024. This facility operates with a One-Star License, effective January 1, 2008. The last sanitation inspection was completed on May 22, 2024, earning a “Superior” classification. The last fire inspection was completed on March 26, 2024, and was approved for day time only. The last Annual Compliance visit was completed on March 13, 2024. Lead Teacher, A. Woodard, was present and available to assist. Administrator, W. Briggs-Trevino was available to assist later in the visit. Nineteen (19) children age ranging three (3) to four (4) years of age were present. Nine (9) children out of the nineteen (19) were observed in an unlicensed and unapproved space. Children were observed in free choice play, teacher directed activities, and toileting and handwashing routines. All staff members interacted with children in a positive manner. The unlicensed space was observed and measured during the visit. The following requirements were monitored during today's visit: Staff/Child Ratio, Supervision, Specialized trainings, Storage of Hazardous Products, Storage of Medication, Adequate/Approved Space, General Safety, Staff Records, Program Records, and License Posted and Permit Restrictions. A violation for use of unlicensed/unapproved space was documented. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. Due to connectivity issues, a hand-written visit summary was reviewed and left with you. Any violations observed were also discussed with you. A detailed, computer-generated visit summary will be emailed to you. The following violations of child care requirements were documented today. Davis. .0608(d)(1-4) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. I explained to you, W. Briggs-Trevino, that the children utilizing the unapproved space could not continue to be in the space due to it being unlicensed. You stated, “I am not going to close this classroom down, but I will continue to do what it takes to get it licensed.” Measurements of the space were taken during the visit.
