H. Best, Administrator, was present and available to assist with today’s visit. Your program operates with a four-star license issued on April 5, 2023, earning seven (7) points in the education component, two (2) points in the program standards component and one (1) quality point for all lead teachers and teachers have at least five (5) years of EC work experience in no more than two (2) different centers. The last annual compliance visit was conducted on September 5, 2024. The sanitation inspection was completed on January 17, 2025, with a “Superior” classification “0” demerits. The fire inspection was completed on August 7, 2025, and the center was approved for daytime care only. The license was posted, and the restrictions were in compliance. Twenty-six (26) children aged zero (0) to seven years old were present during the visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I found supervision and staff/child ratios to be in compliance. Children throughout the center were participating in group activities, free-play, and personal care routines. Children were observed engaging in gross-motor play in the outdoor play area. The caregivers were interacting and meeting the developmental needs for each of the children. Children were observed eating the following for lunch, chicken nuggets, corn, pears, bread, and milk. Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. The following violations were documented during today’s visit: Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. In the outdoor play environment, the grass was growing past the ankles and needed to be cut. There was also an active hill on the playground where toddler and preschool aged children play. 15A NCAC 18A .2832(a) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Teresa A. McMillan had a CBC letter on file; however, the letter expired on August 10, 2025. G.S. 110-90.2(b) & .2703(n)&(o) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before September 4, 2025. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. TECHNICAL ASSISTANCE: CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children’s records were monitored. The files were well organized and in compliance. Staff files were available but not reviewed. The staff and training worksheet was not available for review. I will come back on August 27, 2025 to review staff files. All current employee's criminal background check (CBC) letters were reviewed and verified in the ABCMS portal. CRIMINAL BACKGROUND CHECK: Teresa A. McMillan had a CBC letter on file; however, the letter expired on August 10, 2025. According to the ABCMS portal the CBC application is in process as of August 13, 2025. Ms. McMillan has fourteen (14) days from today, Thursday, August 21, 2025, to have a qualifying CBC letter on file. If Ms. McMillan does not receive a qualifying CBC letter by day fourteen (14), she may not return to the child care facility on day fifteen (15). Once Ms. McMillan receives her CBC letter please retain the letter in her staff file. SANITATION: In the outdoor play environment, the grass was growing past the ankles. Please remember grass needs to be maintained to prevent unwanted pests in the outdoor play environment. There was also an active ant hill on the playground where toddler and preschool aged children play. Please treat the ant hill as soon as possible. TRANSPORTATION: Your 15-passenger mini-bus with the license plate KKM-1468, met all transportation requirements. ABCMS PORTAL/MOODLE TRAINING: You have completed the ABCMS Moodle training and have connected all current staff to your ABCMS portal. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. CLEAN WATER FOR CAROLINA KIDS: According to the Clean Water for Us Kids website, you tested your water on June 19, 2024, you have completed the lead paint testing and asbestos testing. Update the worksheets when changes are made, such as hiring new staff, additional coursework, or training. We discussed making sure you submit your education transcripts to the “WORKS” system, so your education can be evaluated. Beginning In October, child care consultants will begin discussing the new rules in Section .3200; Standards for Two through Five Star Rated Licenses during licensing visits. Consultants will review the pathway options, identify facility needs, answer questions, and work with the facility operator to establish an individualized timeline for transition to a new rated license within 12 months based on the pathway chosen. The required information was not available on the form provided by the doctor. Please have Ms. Blow have her doctor fill-out the Medical Assessment form and retain in her staff file. The staff files were well organized and available for review. MEDICATION: In space number one (1) there was a tube of nystatin cream. The medicine was not in its original pharmacy labeled container. Please remember any medicines that are prescribed by a doctor must be in their original container and have the pharmacy label on it. You have tried several times to contact RTI to understand next steps and you were told you would receive a call to discuss the results. EMERGENCY PREPAREDNESS AND RESPONSE (EPR) PLAN: As a reminder your EPR plan will need to be updated in the NC Risk Management Portal (https://rmp.nc.gov/portal/) on or prior to September 7, 2024. Your asbestos testing has been completed and the information was uploaded on August 19, 2024, there was asbestos found in the hallway ceiling. You have not been contacted since the testing was completed. You accessed your reports through the Clean Water for Us Kids. You have tried several times to contact RTI to understand next steps and you were told you would receive a call to discuss the results. The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. I also informed you that I sent an email to you on March 5, 2024, with the following information included. I also attached to that email three (3) flyers that discuss the process for testing, for lead in the water, lead in paint, and asbestos. You completed the lead water testing on April 2, 2021, you need to complete the water lead testing again. Please remember centers must test their water every three years. You stated you have requested your testing kits and are waiting for them to be delivered. According to Clean Water for Us Kids, you have started enrollment for the lead paint and asbestos testing. The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. CHILD A CARE RULE CHANGES: The Child Care Commission adopted child care rule changes in January 2024. Changes relate to definitions; lead and asbestos for centers and family child care homes; building requirements for family child care homes; multi-unit child care centers; and criminal background checks. Consultants will assist as you begin to review and implement the changes, but please note, some of the rule changes may or may not impact your facility. An example is the rules in section .2600 for multi-unit child care center. These rules are specific to child care centers with multiple licensed centers within one building.
