Your last annual compliance visit was on 3-25-25. Today’s visit was conducted with you Yoly Statley, Provider. The home currently operates with a 4-star license. The children were observed learning about Egypt in the primary care area of the FCCH which is a room to the left of the front door. The FCCH is set up with centers for the children. You stated the children have access to the bathroom, kitchen and living room, all within the open, floor plan. The children play outside in backyard and there are no pets in the home. I used a checklist to monitor compliance. You’re approved to transport children and drive a Kia Telluride. You have two (2) substitutes, K. Bethea (Ms. B) and J. Jimenez, that cares for your school-age child and assists you with the other enrolled children. Ms. Jimenez was present in the home today assisting you with all the children, including your B. Staley (BS) school-age child. **Verification of records: The last lead water test was conducted on 3-20-24. *Your EPR training was completed on 9-25-21 & your EPR plan was updated on 9-3-25. *The last lockdown drill was on 11-3-25; the last fire drill and outdoor inspection was on 1-1-26. *You have five (5) people listed in the ABCMS system. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Little Achievers Daycare, LLC) listed as current-active. The following violation was observed and corrected during the visit. Technical assistance was discussed with you how to correct the violation and how to maintain compliance. Violation Number Comment Rule 1886 Required locked storage was not secure with combination locks, electronic, or magnetic devices, key, or equivalent locking devices. I observed rubbing alcohol and lotions under the bathroom sink. The sink was locked with an unapproved lock. During the visit, the provider removed these items to the kitchen where she had an approved lock. .1719 (a) (7) We discussed the requirements for additional substitutes as found in rule .1729. One caregiver, K. Bethea (Ms.B) will need 15 annual training hours. Ms. B has 20 + years of childcare experience and she operated a licensed FCCH for over 20 years. Ms. Jimenez is currently in college and will receive her BS degree in Behavioral Social Science, she will need 5 annual on-going hrs. a year. *The only approved locks are combination lock, lock and key, electronic or magnetic locks. This was discussed on your 12-7-22 visit summary. ******QRIS/Rated License Documentation: ****** *You have chosen PATHWAY 2 Classroom & Instructional Quality and appear to qualify for a 5-star license. *FAMILY & COMMUNITY ENGAGEMNT FOUNDATIONAL PRACTIVES (Rule .3220) has been chosen with 3 additional choices, with at least one in each category. *CQI PLAN for operator-Rule .3221 and Rule .1703 (i)-You must identify at least one professional goal every year. This is a goal that builds over time, using established goals to support the FCCH operator in achieving and sustaining continual improvements. Refer to your COI plan to verify what must be included in the COI plan. In one year, you will need to complete the bottom half of the CQI plan. *APPROVED CURRICULUM and FORMATIVE ASSESSMENT-The Provider uses two curriculums which are: Explorations with Young Children: A Curriculum Guide from the Bank Street College of Education and CEO Academy. She uses Desired Results Developmental Profile (DRAP) Assessment as her formative assessment. *ASSESSMENTS-Provider understands she must share conduct assessments for all children and share them with families at least twice a year. Child Care observations must take place every year on each child, using the approved formative assessment and the observations must be discussed at least twice with the child’s parents. Proof of these observations must be kept in your FCCH. These will be monitored on an annual basis. I suggest you keep a notebook with dividers for each child, so all observations are kept in one place. *COACHING/TRAINING OPTIONS-Rule .3206-You must participate in one of the following activities regarding classroom and instructional quality practices-Proof of completion must be on file in your FCCH: -Annual completion of 5 hours of coaching and mentoring by another FCCH provider with a star rating of at least 5-Stars (must be the same of higher star level as your FCCH) OR a community partner (like WCSS or CCSA, etc.) -Annual completion of 5 on-going training hours. These are in addition to your regular annual training hours. -Annual completion of 0.5 CEU’s, continuing education unit (5 contact hours) in addition to your regular annual training hours. *EDUCATION-Provider has a Grad. degree in Business Administration and is qualified as a level III Administrator and has been a licensed FCCH since January 2021. *Prior to today’s visit, your 18-month compliance history was 100%. ***********CAPACITY CHANGES: ************* *You have requested that your capacity change from 8 to 10 children the restriction removed stating you can have 5 preschool and 3 school-age children. You stated you want to care for a maximum of 10 children, since all the children currently in care are older than 24 months. We discussed that you must maintain compliance with one of the 3 options with a maximum capacity of 8, 9, or 10 children at all times. 1. A maximum of eight children, with no more than five children who are from birth to 5 years of age, plus three school-age children. 2. A maximum of three children from birth to 24 months of age, plus three children from 2 to 5 years of age and three school-age children up to 13 years of age, for a total of nine children. 3. A maximum of 10 children if all children are older than 24 months of age. *On 11-20-25, Wake County sent a letter stating you can have a total of 10 children ages 0-12 years old. *On 11-4-25, Your HOA sent an email stating that you can increase from 8 to 10 children, ages 0 to 12 years old. *Remember, since you’re a 5-star FCCH, you agree that you can have “no more than 4 children under the age of 24 months of the children who are birth-5 years of age when meeting NCGS 110-91(7)(b)(1)”. This is also documented on your Pathway 2 assessment form. ***NOTE: We’ve been discussing your QRIS and capacity change since October 2025, you were waiting until all the children enrolled were at least 24 months old and you had enough supplies for 10 children. *The rooms and areas children receive care is an open floor plan with an exit at grade. *There’s at least one fire extinguisher within 40 feet of the area. *The FCCH maintains fire safety, evacuation and lockdown plan. *There’s a plug-in carbon monoxide alarm in the room where the children sleep and in the garage. *You do not have any gas appliances. *An electric smoke detector and battery backup are in the room where children sleep and an additional smoke alarm in the areas used by the children. -Your signature on this visit summary & on the rated license worksheets verifies this information is current, and you understand the information discussed and documented. Your Rated License paperwork will be forwarded to DCDEE to be verified and processed. A new license will be mailed to you from DCDEE.
