The center currently operates with a 3-star license, operating at 25 sq. ft. inside, & ratios at .2818 (b). My visit was conducted with you, Jennifer, Director. The children were observed participating in teacher-directed activities, free play, playing outside & eating lunch. A checklist was used to monitor compliance. The facility does not transport children; therefore, these requirements were not monitored. VERIFICATION OF RECORDS: *Last fire inspection-6-4-25 (send me your 2026 inspection within 5 days of completion). *Sanitation inspection 11-14-24 (scan me your 2025-2026 inspections within 5 days). *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Knightdale Station Preschool, Inc.) listed as current-active. *The Lead water test expires 4-6-27. *The last EPR was updated on 1-9-25. EPR training was completed by Amanda Tripp, Owner, on 11-30-17. *The center uses an approved curriculum, High Scope. The following violation was observed and must be corrected immediately. Email me a corrective action letter by 12-2-25 indicating how the violation has been corrected. Email to michelle.ezzell@dhhs.nc.gov Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Baby wipes that had a warning on the label indicating to "keep out of reach of children" were in unlocked cabinets in space one through four. 2. In space three, unapproved locks were used on a cabinet. 10A NCAC 09 .0601(a) *****TECHNICAL ASSISTANCE/REMINDERS: ******* * APPROVED LOCKS: SANITATION RULE 15A NCAC 18A .2828-STORAGE- Locked storage rooms and cabinets shall include those which are unlocked with a combination lock, electronic or magnetic device, keypad, key, or equivalent locking device. Keys and electronic or magnetic unlocking devices shall be kept out of the reach of a child and shall not be stored in the lock. * Storage of Hazardous Items- Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Items that require locked storage were improperly stored under the diaper changing tables. It is extremely important that staff take the required time to lock doors, store hazardous items properly either in locked storage or five feet up from the finished floor. *BOOKS-In space 4, the classroom for toddlers had numerous books, although the pages were folded from being washed. We discussed adding additional hardback books in this space. *We discussed eight (8) employees need to be added to the ABCMS portal. The ABCMS portal should be updated every six (6) months. ****QRIS Rated License Reminders***** *Remember to have staff’s education information on file with DCDEE/Workforce and their information must be current in WORKS database. *We discussed you have chosen Pathway 2. You have completed the Family and Community Engagement Standards form and you’re currently working and the CQI form for the facility. You’re working on the CQI for each staff member and staff’s education standards. *You use an approved curriculum and Formative Assessment called, High Scope. Amanda Tripp, owner, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Tripp, Jennifer Mazariegos, director, and additional staff members. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member yelled, used inappropriate language, and was physically aggressive with her own child, age three, who is an enrolled child, in the presence of other children. My visit was conducted with you, Mrs. Jennifer Marzariegos, Director. The children were observed participating in teacher directed & free play activities. A checklist was used to monitor for compliance. I monitored for required posted documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, program records, equipment, furnishings, and medications requirements. The facility does not transport children; therefore, these requirements were not monitored.
