    Name of Operation: KNIGHTDALE HEAD START Facility ID: 92001437 Consultant: MICHELLE EZZELL Operation Type: Center Case Number: Visit Date: 5/7/2025 Number Present: 52 Completed Date: 5/7/2025 Age: From 2 To 5 Total Minutes: 235 Time In: 09:40 AM Time Out: 01:35 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance during your annual compliance visit. The center currently operates with a 5-star license, operating at 30 sq. ft. inside, & ratios at 1818 (c). The license is in a harmless stage. My visit was conducted with Ms. Alicia McNeil, Education Disability Specialist. Ms. Reid, Director, was not present. A checklist was used to note the requirements I monitored. All CBCs were in compliance. Twelve staff (12) files were spot checked for compliance. The center has 14 classrooms; 5 classrooms are open and the remaining 9 classrooms are closed. The center has two (2) NC Pre-K classrooms occupying space #1, & 2, two (2) Early Head Start classrooms and one (1) Wake Three School classroom. The children were observed playing in centers and outside. The center does not transport children; therefore, these requirements were not monitored. The center has several playgrounds and uses two of them, the preschool playground located at the front entrance on the side of the building (not on Todd Rd. side) and uses the older toddler/2’s playground at the opposite side of the building. VERIFICATION OF RECORDS: *Last fire inspection=5-1-25 (send me your 2026 inspection within 5 days of completion). The last fire drill was on 4-28-25 & SIP was on 3-7-25. *Sanitation inspection=3-5-25 (forward me your 2025-26 inspections) *The center has well-water. The well water analysis will expire 3-5-27. *The last lead water test was on 7-23-24 & expires 7-23-27. *The EPR plan was last updated 7-8-24. EPR training was completed by Ms. Reid on 6-20-22. *The last outdoor playground inspection was completed on 5-1-25 by Ms. Reid. Ms. Reid conducted Playground Safety training on 1-29-20. *Current Curriculum-Smart Teach (formerly known as Creative Curriculum Teaching Strategies Gold TSG). *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Telamon Corporation) listed as current-active. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately. Please email me a signed and dated corrective action letter with your ID # and facility name BY 5-21-25 to michelle.ezzell@dhhs.nc.gov. Remember to address the violation/item number explaining how the violation is now corrected. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. Two wooden swings located on the playground used for the toddlers and two-year-old children had peeling paint and splinters. These swings were blocked off today, so children could not use them. G.S. 110-91(6); .0601(b) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. Two children's action plans were on the WCPSS form and did not have the required information. The forms were missing what signs the child may have as a "trigger" to give the emergency medication. .0801(b) *****TECHNICAL ASSISTANCE/REMINDERS: ******* *If an employee needs 20 hours of on-going training hours, CPR & 1st Aid cannot count toward their training hours. *Health and Safety Trainings are due every 5 years. *When you link each staff member’s criminal background information in the ABCMS database. The date will go on line #7 on your staff/training worksheet. A Director’s meeting was conducted yesterday, and the ABCMS instructions were reviewed and emailed to you. This must be done ASAP. If you need assistance, contact the criminal record unit. Additional information is on DCDEE’s website. For NCID questions call 919-814-6326 or 919-754-6000. *MEDICAL ACTION PLANS= Growing up healthy is every child's right and every parent's dream for their children. Significant advances in improving the overall health and development of children with chronic illnesses have been made. Having a medical action plan in place when caring for children with health issues is vital in ensuring their health and safety. These health issues could include allergies, asthma, or other chronic conditions that require specialized health services. The maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember that a medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. Permission to administer emergency medications must be updated every 6 months by the parent. Lisa Terry, Wake County Health Care Nurse contacted the center and gave technical assistance to administrative staff concerning the Medical action form. If the center chooses to use the medical action form used by Wake County Public Schools, a section must be added as to what triggers the child may have so staff will know when to give the emergency information to the child. *OUTDOOR EQUIPMENT=must be free of hazards such as splinters. The swing located on the playground used for toddlers and two-year-old children needs sanding and painting. During today’s visit, this area was blocked off by using ribbon/tape. This is a temporary fix. Ms. McNeil stated the swings will be removed from the playground. The wooden sand box enclosures will need sanding and painting within the next year, due to splinters. ****Rated License Reminders***** *Remember to have staff’s education information on file with DCDEE/Workforce and their information must be current in WORKS database. *PROGRAM ASTANDARDS: For trainings on environment rating scales go to www.ncrlap@uncg.edu and/or contact Wake County Smart Start or Child Care Services Association.
