From June 23, 2025, to December 23, 2025 you were issued a temporary license. On 12-24-25, a QRIS, Pathway 2, 3-star license was issued. The center currently operates with a 3-star license, operating at 30 sq. ft. inside, 100 outside & ratios at .0713 (a). My visit was conducted with you, Aisha, Director. During our walk-through, I shared technical assistance with you on the childcare rules and laws. Since this was your 1st visit since your temporary license, LeAnne Simpkins, Lead Child Care Consultant, accompanied me. A checklist was used to note the requirements monitored. The children were observed participating in outdoor time, eating lunch, and preparing for nap. I monitored required documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, program records, equipment, furnishings, and medications requirements. No transportation requirements were monitored since the center does not transport children. VERIFICATION OF RECORDS: *Last fire inspection=12-18-25 (send me your 2026 inspection within 5 days of completion) *Sanitation inspection=6-4-26 (send me your 2027 inspections). The last lead water test was completed on 6-1-25 and will be due by 6-1-28. *EPR training was completed by Alisha 11-13-17. *The EPR plan was last updated on 6-1-26 *I reviewed the NC Secretary of State’s website and observed the owner of the facility (WEW Group, LLC) listed as current-active. Technical Assistance was offered on how to correct the violations cited and how to maintain compliance. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately. Email a corrective action letter BY 6-30-26 to michelle.ezzell@dhhs.nc.gov Violation Number Comment Rule 612 Each child's bed, cot, or 2" mat was not individually assigned and identified. Two classrooms did not have a current cot list. 15A NCAC 18A .2821(b) & (c) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One employee (S. Brooks) criminal record expired on 4-29-26. She must submit a new CBC within 15 days. I monitored three classrooms for supervision, staff/child ratios, group sizes, capacity, postings, materials, discipline, interactions and health and safety. I also verified that children go outside daily, weather permitting. Space 2: Two teachers were present with seven infants. One infant was absent today. The infants were observed exploring materials, having a bottle, napping and being consoled by the teachers. Teachers were observed interacting with the children in a positive manner. Safe sleep checks were completed as required. Reminder to keep safe sleep checks on file for at least 30 days. Feeding schedules were posted on the inside of the cabinet door above the sink. Bottles were labeled and dated. Space 3: Two teachers and one floater were with eight children, all one-year-old. The children were exploring materials and interacting with the teachers. It is important to ensure that all staff have current education in the DCDEE WORKS portal. Three children did not have receipt of the NC Summary of the Law on file. GS 110-102 432 The center did not have developmentally appropriate equipment and materials accessible daily. Books were located in a cabinet in space 2, classroom for infants. The books were removed from the cabinet and placed where the infants could access the books. GS 110-91(12);10A NCAC 09 .0509(1) 832 There was no written emergency medical care (EMC) plan. The plan was completed during the visit. 10A NCAC 09 .0802(a) 1030 Application for employment and date of birth was not on file for all staff. Three staff did not have an application on file. .0302(d)(1)(A) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff did not have a medical on file on the first day of work. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Four staff did not have emergency information on file on or before the first day of employment. .0701(a) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. Three children did not have receipt of the prevention of shaken baby syndrome policy. .0608(b)(1-6) Technical assistance: The incident reports were stored in a notebook. A log was present but it wasn’t completed. Be sure to document the incidents on the incident log so it is organized. Dividing the incident reports by month would be helpful and easy to maintain. Behavior notices were also located with the incident reports. It is best to separate the two so it is clear which reports are incidents, and which ones are behavior. In space 8, there was a container with sharpies and adult scissors that children could reach, (half wall) Be sure to place these types of items 5 feet above the floor or out of reach of children. If a child is exempt from immunizations due to a medical or religious reason, keep in mind there may be times if there is an outbreak of a communicable disease, that children may not want to be in care. (Measles) The violations cited today were corrected during the visit. No action is required at this time. Please complete the staff and training worksheets for all staff.
