    Name of Operation: CORA'S CARING HANDS Facility ID: 92001166 Consultant: MICHELLE EZZELL Operation Type: Family CC Home Case Number: Visit Date: 11/10/2025 Number Present: 2 Completed Date: 11/10/2025 Age: From 1 To 2 Total Minutes: 120 Time In: 10:15 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Rated License Assessment Announced/Unannounced: Unannounced The purpose of my unannounced visit today was to conduct a rated license assessment. Your last annual compliance visit was on 4-22-25. Today’s visit was conducted with you Bernetta Jack, Provider/Owner. The home currently operates with a 3-star license. Two children were present and observed sleeping in the primary care area of the FCCH which is in the living room at the entrance of the FCCH. I monitored safety issues, posted documents, and program records. **Verification of records: The last lead water test was on 11-6-25 *Your EPR plan was updated on 10-1-25 *Your last fire drill and outdoor inspection was on 10-1-25. The last lockdown drill was on 9-1-25. The following violation was observed and must be corrected immediately. Technical assistance was discussed with you how to correct the violation and maintain compliance. Please email a signed and dated correction action letter to michelle.ezzell@dhhs.nc.gov by 11-24-25. Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The FCCH provider has not linked her and her household members criminal record qualifying letters in the ABCMS portal. G.S. 110-90.2 & .2703(r) The following Technical Assistance was given: *ABCMS PORTAL Linking Criminal Background Checks for FCCH’s – FCCH Providers and household members/substitutes/etc. need to log into the Criminal Background Check Portal on the DCDEE website and to link the facility license ID number that’s listed on your license to their qualifying letter. It is important to do this so DCDEE can contact the facility if a random criminal history check uncovers them with a criminal charge or conviction working in a child care facility. It is also a way for staff at the CBC Unit to keep you updated on the status of out-of-state checks if a new household member/substitute, etc. has a Provisional Qualification. Instructions on how to link to the facility has been provided by newsletters and emails and at visits. In the future, new household members/substitutes, etc. need to link within five days of hire or transfer to your facility. Review Criminal History Record Check Requirements for Child Care Centers in the NC Child Care Requirements 10A NCAC 09 .2703. If you have any questions regarding the criminal background check process, dial 1-800-859-0829 and ask for the criminal background unit. *A step-by-step handout to help you with the ABCMS portal was emailed to you in November 2024, February and March 2025 and given to you during today’s visit. ******Rated License Documentation: ****** *You have chosen Pathway 2 and qualify for a 3-star license. *You meet the minimum licensed capacity options *You completed the Family & Community Engagement Foundations Practices & one additional choice has been met. Your additional choice is: Communication Option: You have chosen to meet the two-way communication with families. *CQI Plan has been completed. *Curriculum-You currently have the Home CEO Academy curriculum. This is currently not an approved curriculum. *You will conduct annual child observations on each child and keep records of these observations. *You will complete 5 hours of coaching and mentoring. (NOTE: We discussed keeping these observations/coaching notes in a small binder, so they can be monitored). *Education Standards: You have your FCCH credential and 9 ECE hrs. *Prior to today’s visit, your 18-month compliance history was 100 %. *Your Rated License paperwork will be forwarded to DCDEE to be verified and processed. A new license will be mailed to you from DCDEE.
