LeAnne Simpkins, Lead Child Care Consultant accompanied me. The FCCH operates with a 3-STAR license. Since the license was issued the following visits has been made: 2-27-25-Rated license assessment, 6-26-25 “other” visit to approved transportation and on 7-21-25 “other” visit to increase the capacity to ten (10) children ages 2 to 12 years old. Upon arrival, Jalise was present with seven (7) preschool children. A little over an hour later, Savannah, 2nd provider arrived to assist us during the visit. The children were playing with age-appropriate toys in the primary care area of the FCCH which is the living and dining room. The entire home is used for childcare except the bedrooms and the kitchen was blocked off with a baby gate. Age-appropriate toys, and child size furniture are in this room, and the walls are decorated with children’s posters, teaching supplies and art. Jalise stated she lives in the home alone and Savannah lives next door. The children play outside in the fence in back yard. There are three (3) pets that reside in the home (one Yorkshire terrier and two standard poodles). When children are present, they are either outside in the fenced area the children play in or in the mudroom located beside the kitchen. The FCCH is enrolled in the Chatham County Partnership for children food program. Jalise stated no children have any medications at the FCCH at the current time. A checklist was used to note the requirements I monitored today. All children’s files were monitored along with program files and both provider’s files. You were approved for transportation, and two cars were approved for use. One is a gold chevy malibu and one is a grey Nissan Sentra. Both registrations are expired and cannot be used until the registration is current. **VERIFICATION OF RECORDS: *EPR Training completed on=1-26-25 by Savannah. *The last shelter-in-place and fire drill was on 10-2-25 and the last outdoor inspection was on 10-1-25. *The lead water test was completed on 9-19-24. Technical Assistance was offered on how to correct the violations cited and how to maintain compliance. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately. Email a corrective action letter BY 11-25 to michelle.ezzell@dhhs.nc.gov We reminded you how to properly send a corrective action letter before leaving today. A corrective action letter means you have corrected all the items of non-compliance and is a legal document. Two children's (KF & KT) medicals were not on file within 30 days of enrollment. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Two Providers criminal background checks have not been linked to the ABCMS portal. G.S. 110-90.2 & .2703(r) 1857 The operator did not develop an Emergency Preparedness and Response Plan within four months of completing the Emergency Preparedness and Response in Child Care training and/or the Plan was not completed on the template provided by the Division. No EPR has been completed. The link was reviewed during today's visit. *ABCMS PORTAL Linking Criminal Background Checks for FCCH’s – FCCH Providers and household members/substitutes/etc. need to log into the Criminal Background Check Portal on the DCDEE website and to link the facility license ID number that’s listed on your license to their qualifying letter. It is important to do this so DCDEE can contact the facility if a random criminal history check uncovers them with a criminal charge or conviction working in a child care facility. It is also a way for staff at the CBC Unit to keep you updated on the status of out-of-state checks if a new household member/substitute, etc. has a Provisional Qualification. Instructions on how to link to the facility have been provided by newsletters and emails and at visits. In the future, new household members/substitutes, etc. need to link within five days of hire or transfer to your facility. Review Criminal History Record Check Requirements for Child Care Centers in the NC Child Care Requirements 10A NCAC 09 .2703. Should you need assistance please contact the Criminal Background Check Unit at (919) 814-8401 and someone will assist you. I also gave you a handout during today’s visit. During today’s visit TA was given. LeAnne showed you on her computer how to navigate in the ABCMS portal and the EPR portal. I sent a step-by-step guide to navigate in the ABCMS portal to both providers. *PET VACCINATIONS-Proof of rabies must be on file for your pets indicating if the rabies shots are a one- or three-year vaccination. The veterinarian should give you documentation or a receipt of this service. *We discussed using the children’s file checklist located on DCDEE’s website as a guide to assist you with due dates of children’s records. We also discussed the importance of records to protect you and the children. *Transporting children can be dangerous. It's important to have updated children's records on file and with you when transporting. It's your responsibility to make sure you have proper car seats and your car is in working order before transporting children. A visit was made to the FCCH on 6-26-25 by LeAnne Simpkins, Lead Licensing Consultant. During the visit, a visit summary was reviewed with you going over the transportation requirements. Refer to rule .1723 in your rule book for all transportation requirements. (22) "Family Child Care Home", as defined by G.S. 110-86(3)(b) means a childcare arrangement located in a residence where, at any one time, more than two children, but less than 11 children, receive child care. Family child care home operators must reside at the location of the family childcare home. *ANNUAL TRAINING HOURS-In order to get credit for on-going training hours, you must attach proof of training certificates, college transcripts or report cards. This is indicated at the bottom of the on-going training form. I highlighted it during today’s visit for you. *While reviewing records, it was discovered Jalisa’ address in the WORKS database and on her driver’s license indicated a different address. We discussed her address must be current in DCDEE’s database and she can go online and updated her address with DMV. *ATTENDANCE RECORDS must be kept for at least a year. I suggest putting attendance records in a notebook so they will stay in order and easily accessible. *FILE FOLDER LABELS-One notebook is currently being used to hold all the program and provider files for the business. I suggest you use a mobile filing box such as a milk crate with hanging file. I gave you a list of file folder labels to assist you if you choose to use this filing system. *SECRETARY OF STATE: The owner of your FCCH is Atru Stars Family Child Care LLC. According to the Secretary of States website, the status is Active-NOT current. You will need to make the status CURRENT/ACTIVE. It may be that you need to pay your annual fees and/or complete your annual report. This must be completed immediately to avoid an administrative action against the license. *Savannah’s criminal background check (CBC) qualifying letter will expire 11-24-25. A new CBC must be submitted by 11-24-25. *OUTSIDE PLAY AREA: The outside play area is a large fenced in playground. Three dogs are also allowed to play in this area. We discussed blocking off part of this area for the dogs to play and urinate and defecate without the children walking in an area that the dogs used the bathroom. ******QRIS Rated License (STARS): ****** *We were going to discuss the 3 QRIS pathways and Savannah suggested doing this at a time she can concentrate and take notes. Upon arrival, Jalise was present with seven (7) preschool children. A little over an hour later, Savannah, 2nd provider arrived to assist us during the visit. The children were playing with age-appropriate toys in the primary care area of the FCCH which is the living and dining room. The entire home is used for childcare except the bedrooms and the kitchen was blocked off with a baby gate. Age-appropriate toys, and child size furniture are in this room, and the walls are decorated with children’s posters, teaching supplies and art. Jalise stated she lives in the home alone and Savannah lives next door. The children play outside in the fence in back yard. There are three (3) pets that reside in the home (one Yorkshire terrier and two standard poodles). When children are present, they are either outside in the fenced area the children play in or in the mudroom located beside the kitchen. The FCCH is enrolled in the Chatham County Partnership for children food program. Jalise stated no children have any medications at the FCCH at the current time. A checklist was used to note the requirements I monitored today. All children’s files were monitored along with program files and both provider’s files. You were approved for transportation, and two cars were approved for use. One is a gold chevy malibu and one is a grey Nissan Sentra. Today’s visit was completed with the one of the providers, Jalisa. She was present with five children. The children were all three years old. The children were engaged in activity centers, exploring materials, and interacting with each other. The provider was observed interacting with the children as they completed days of the week sheets and practiced learning shapes and letters. The children went outside to play and washed their hands upon coming back inside. They also had lunch and washed hands and rested afterwards. Lunch met the nutritional requirements. Savannah arrived about 11:45am. Jalisa left at 12:15pm. All required postings were observed posted. Attendance was current and was available for review. A pet was added since the last visit. Pet records were reviewed and were current. Children’s files contained the pet acknowledgement form. The last fire drill was conducted 2/4/25 at 9:30am. Outdoor inspections were not completed for January or February 2025. The last outdoor inspection was completed December 13, 2024.
