Last Annual Compliance visit – 3/10/25 18-month compliance history from 3/1/24 – 8/29/25 = 100% Last Sanitation Inspection – 5/1/25 - Superior Water Testing (Lead) – completed 7/29/23; due again 7/29/26 Last Fire Inspection – 5/15/25 – Satisfactory; Daytime Care Only *Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. Send fire inspections to me within seven (7) days of the inspection. C. Kercheval, Assistant Administrator of Admissions, was present and assisted me with the visit. The facility currently operates with a Notice of Compliance, issued 11/8/2021. In the event you are interested in applying for full licensure in the future, the QRIS Modernization rules have been adopted. Information is available on the DCDEE website. This facility is owned by Lighthouse Christian Fellowship, Inc. According to the NC Secretary of State's website, this non-profit corporation is current and active as of 8/29/25. Notice of compliance information and contact information were current. Please contact me prior to any information change regarding the ownership of this facility, license updates, or contact information (address, phone, or email). A new school administrator has been hired. I received a completed Administrator Preservice Form. I monitored the classrooms and spaces used for child care and outdoor playground for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2025 and Item Numbers Listing, effective 4/2025. I monitored two staff records and all program records. The facility does not offer transportation. Center Observations: I completed a walk-through of the facility and monitored all indoor and outdoor spaces. I observed children participating in routines and developmentally appropriate activities both indoors and outdoors. The children in Space 1 were playing on the playground while the teacher supervised and interacted with them. The children in Space 2 were finishing up snack before washing hands and joining the children from Space 1 on the playground. The large complex climber was made inaccessible to the children while it is being repaired because of rusted components. Staff members interacted positively with the children. Lunches are brought from home and stored in classroom refrigerators. After lunch, some children go home. The remaining children nap on linen-covered cots. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. A teacher was alone on the playground with sixteen (16) children, three through four years of age for approximately two (2) minutes. GS 110-91(7);.0713(a-d) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Medication permission forms for an Epi-pen and Benadryl were authorized from August 27, 2025 - July 1, 2026, instead of the required six-month authorization time frame. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4)(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to you, C. Kercheval. We reviewed the visit summary and violation(s) documented during today’s visit. Correct these immediately. Send evidence of correction to me so that I receive it no later than 9/16/25. Your compliance verification letter needs to state your facility name and license number and list each violation by Item # and describe in detail when and how each violation has been corrected. Do not include names of children in the compliance letter. If needed, use their initials. Send compliance verification letter in an email from your center’s official email address, info@beaconobx.com, to my email address, Marjorie.White@dhhs.nc.gov. You may include the compliance verification as an attachment or in the body of the email. I will email a sample compliance verification letter to you. If sufficient information is not received by the due date, a follow-up visit may be conducted. Please be aware that any information submitted by you is legal documentation. If it is determined the information provided in the compliance letter is not true, this may be considered falsification of information. A violation was documented that requires a follow-up visit. An unannounced visit will be made soon to verify correction of the staff/child ratio violations. You are still required to send a compliance letter addressing the two violations documented today. Technical Assistance With Documented Violations: Staff/Child Ratios - Staff/child ratios and group sizes based on the age of the children in care help to ensure the safety of children and allow staff to provide the care, attention, and learning opportunities for each child. A teacher was left alone on the playground with sixteen (16) children, three through four years of age, when a second teacher went indoors for personal reasons. The teacher left on the playground positioned herself to maintain visual supervision of all the children on the playground. The assistant administrator and two teachers stated that they just forgot that the staff/child ratios in a group with children who are three years of age is 1:15 instead of 1:20. Preferred center staff/child ratios for each classroom is 1:12. They discussed the situation and stated in the future, the two teachers will first determine how many children are present and the required staff/child ratio when the two classes play together on the playground. Then if a teacher needs to go indoors, they will use the walkie talkie or cell phone to contact one the administrative personnel to step outdoors to help maintain staff/child ratios and supervision. Also consider that the playground is a large space to supervise, and one person left alone to supervise such a large space even when staff/child ratios are met, can be a challenge. Please be aware that repeated violations related to staff/child ratios may result in an administrative action. Standing Authorization for Medication – Medication dosage for young children has to be updated more frequently because they are growing so quickly between infancy and preschool. The medication permission form for chronic conditions must be reauthorized every six months by the parent to verify that the listed dosage and other information are still correct. Medication permission forms for an Epi-Pen and Benadryl were authorized by the parent to be administered from 8/27/2025 to 7/1/2026 instead of a six-month authorization period from 8/27/2025 – 2/27/2026. You stated that the parent completed this information last year and must have forgotten about the six-month time frame. Remember that it is your responsibility to review medication, medication permission forms and medical action plans before accepting them in your center. Have the parent reauthorize both medications for six months (8/27/2025 – 2/27/2026). Then in February 2026, the parent can review and update the medication permission forms for another six months. For your compliance letter, state the date when both medication permission forms have the required six-month authorization period. Do not send copies of the updated medication permission forms. At this point, they are easily pulled and removed but if allowed to continue to grow and spread, they will compact the sand and reduce the resiliency of the cushioning in the event of a fall. Consider playing a number game with the children. Pick a number between 1 and 5. If the number is 3, everyone picks 3 grass sprigs or weeds and puts them in the bucket. Before you return indoors, count the weeds with the children and maintain an on-going graph indoors. You will be working on math skills and helping to keep the playground weed-free! You have made the large complex climber inaccessible with caution tape until you can get the rusted components replaced or sanded, smoothed and re-painted or covered with vinyl. Sex Offender Registry and Notification Email - North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. Last Annual Compliance visit – 3/10/25 18-month compliance history from 3/1/24 – 8/29/25 = 100% Last Sanitation Inspection – 5/1/25 - Superior Water Testing (Lead) – completed 7/29/23; due again 7/29/26 Last Fire Inspection – 5/15/25 – Satisfactory; Daytime Care Only *Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. I monitored the classrooms and spaces used for child care and outdoor playground for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2025 and Item Numbers Listing, effective 4/2025. I monitored two staff records and all program records. The facility does not offer transportation. Center Observations: I completed a walk-through of the facility and monitored all indoor and outdoor spaces. I observed children participating in routines and developmentally appropriate activities both indoors and outdoors. The children in Space 1 were playing on the playground while the teacher supervised and interacted with them. The children in Space 2 were finishing up snack before washing hands and joining the children from Space 1 on the playground. The large complex climber was made inaccessible to the children while it is being repaired because of rusted components. Staff members interacted positively with the children. Lunches are brought from home and stored in classroom refrigerators. After lunch, some children go home. The remaining children nap on linen-covered cots. I observed and documented the following violations during today’s visit. The teacher left on the playground positioned herself to maintain visual supervision of all the children on the playground. The assistant administrator and two teachers stated that they just forgot that the staff/child ratios in a group with children who are three years of age is 1:15 instead of 1:20. Preferred center staff/child ratios for each classroom is 1:12. They discussed the situation and stated in the future, the two teachers will first determine how many children are present and the required staff/child ratio when the two classes play together on the playground. Then if a teacher needs to go indoors, they will use the walkie talkie or cell phone to contact one the administrative personnel to step outdoors to help maintain staff/child ratios and supervision. Also consider that the playground is a large space to supervise, and one person left alone to supervise such a large space even when staff/child ratios are met, can be a challenge. Please be aware that repeated violations related to staff/child ratios may result in an administrative action. Standing Authorization for Medication – Medication dosage for young children has to be updated more frequently because they are growing so quickly between infancy and preschool. The medication permission form for chronic conditions must be reauthorized every six months by the parent to verify that the listed dosage and other information are still correct. Medication permission forms for an Epi-Pen and Benadryl were authorized by the parent to be administered from 8/27/2025 to 7/1/2026 instead of a six-month authorization period from 8/27/2025 – 2/27/2026. Weeds and grass were starting to move into fall zones. At this point, they are easily pulled and removed but if allowed to continue to grow and spread, they will compact the sand and reduce the resiliency of the cushioning in the event of a fall. Consider playing a number game with the children. Pick a number between 1 and 5. If the number is 3, everyone picks 3 grass sprigs or weeds and puts them in the bucket. Before you return indoors, count the weeds with the children and maintain an on-going graph indoors. You will be working on math skills and helping to keep the playground weed-free! You have made the large complex climber inaccessible with caution tape until you can get the rusted components replaced or sanded, smoothed and re-painted or covered with vinyl. Sex Offender Registry and Notification Email - North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center.
