This facility currently operates with a Five-Star license issued August 18, 2017. The last annual compliance visit was completed March 18, 2025. A checklist was used to note the requirements I monitored during the visit. The last sanitation inspection was completed on February 11, 2026, earning a superior classification and “7” demerits. The last satisfactory fire inspection was completed September 29, 2025, and was approved for day time care only. D. Williams, Lead Teacher and K. Suther, was present and available to assist. Seventeen (17) children, ages four (4) and five (5) years old were present. Children were observed during teacher directed activity, handwashing and toileting, group time, screen time (sing along), free play with developmentally age appropriate toys, preparing for lunch, eating lunch, preparing for nap, and napping. Lunch consisted of: ribs or popcorn chicken, mash potatoes, string beans, sliced apples and mixed fruit cup. All staff interacted with children in a positive manner. Off-site forms were monitored. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. NC Pre-K requirements were monitored during the visit. The following violations were documented today: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Principal was not linked in ABCMS Portal. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. You can mail or email me verification of compliance regarding the violations that were documented. If you choose to mail, two signed and dated copies must be sent and received by March 16, 2026. Please be aware that any written information submitted by you regarding correction of the violations documented during today's visit is legal documentation. It is important that all information submitted be accurate and truthful. Please be aware that if it is determined that information submitted was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility's license could be issued. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always if they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. CONSULTATION: Compliance History: Prior to today's visit, this facility has maintained an 18-month compliance score of 100%. PROGRAM RECORDS: Fire Drills/Emergency Drills, Fire inspection, Sanitation report, First Aid/CPR Poster, Playground Outdoor Inspections, EMC Plan and License were reviewed and current. CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children records were monitored. Children records reviewed were in compliance. You should review other children’s records not monitored to ensure compliance for the same information. Ten percent (10%) or more of staff records were monitored. Staff records reviewed were in compliance. Staff should review records that were not monitored to ensure compliance. DCDEE WORKS: All staff should ensure they have an education qualification letter on file. Each staff member should create an account, apply for DPI teacher or DPI teacher assistant, and submit transcripts and licensure if not already done so. PROVIDER DOCUMENTS/FORMS: You should visit the website often to ensure you’re using all updated forms. All requirements must be included if you choose to use your own forms electronically or hard copy. Child care rules updated effective July 1, 2023, and January 1, 2024. Criminal Background Check: All staff were verified in ABCMS Portal. Principal K. Suther has a valid qualification letter through 4/28/2026 in the ABCMS person summary but needs to be linked to ABCMS employee roster in ABCMS Portal. I reviewed with you D. Williams the ABCMS Technical Assistance to assist with linking new hires. I encouraged you to call CBC unit if you have any issues with linking process. TECHNICAL ASSISTANCE: Technical assistance was provided today during the visit. I assisted you with ABCMS Portal and I reviewed the information and instructions from ABCMS provider technical assistance. I left a copy of ABCMS Technical Assistance Instructions with you. QRIS RATED LICENSE TRANSITION QRIS: I discussed and reviewed with you K. Suther/Principal and D. Williams/Lead Teacher the Pathways to Stars QRIS modernization information from the DCDEE website. We discussed pathway 1 and you stated that you are choosing Pathway 1. I strongly recommend that you visit the North Carolina Rated License Assessment Project’s website at ncrlap.org. There are a variety of videos and infographics with information about ITERS-3, ECERS-3 and SACERS in completing pathway #1. Find information about self-study, updated resources, credit hour trainings, and outreach assessment opportunities from the North Carolina Rated License Project (NCRLAP). I encourage you to continue working with FGV Smart Start for technical assistance related to preparing for pathway 1. I reviewed the Application for Rated License Assessment and each form and where to access them on the DCDEE website under the "provider" then go to "provider documents and forms". I recommend for you to review and print Pathways to the Stars rules, Section .3200, Standards for Two through Five Star Rated Licenses which became effective July 1, 2025. Click on the tab under "What's New" and then scroll down to "QRIS MODERNIZATION" you will then find the information for each pathway, questions and answers, and resources. The Division understand that you may have questions as you consider this information. Questions about QRIS Modernization (Reform) may be sent to DCDEE_QRIS@dhhs.nc.gov. I strongly encourage you to start preparing for the Rated License Assessment. I discussed and reviewed the new questions and answers FAQ sheet located on the Division website. We have a tentative plan to submit the application for a rated license assessment in March of 2027. Please contact me if you have any additional questions or need clarification. This facility currently operates with a Five-Star license issued August 18, 2017. The last annual compliance visit was completed May 9, 2023. Due to printer difficulties, a one-page summary was completed and left with D. Williams. A detailed generated copy will be emailed to the facility. The last sanitation inspection was completed on October 9, 2023, earning a superior classification and “0” demerits. The last satisfactory fire inspection was completed September 21, 2023, and was approved for daytime care only. D. Williams, Lead Teacher, was present and available to assist. Seventeen (17) children, four (4) and five (5) years of age were present. Children were observed napping, toileting, handwashing and eating snack. All staff interacted with children in a positive manner. This facility participates in the NC PreK program and is owned and operated by the Vance County School System. The following violation of child care requirements were documented. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection was not completed within 12 months of the previous inspection date September 19, 2022. Inspection was not submitted within one week of the inspection visit. 10A NCAC 09 .0304(a) The violation was corrected during the visit. TECHNICAL ASSISTANCE/CONSULTATION: FIRE INSPECTIONS: Fire inspections should be completed within one year of the previous inspection date. Inspections also should be submitted to the consultant within a week of the inspection date. The fire inspection was current; however, the inspection was not completed within the year of the inspection date of September 19, 2022. The inspection was completed on September 21, 2023, therefore this was corrected during the visit. SANITATION: Children’s Environmental Health has newly readopted child care sanitation rules. You should visit our website under the services tab, then select child care rules, law and public information, and then section 2800 Sanitation of Child Care to review newly readopted rules. STAR RATED LICENSE (SRL): The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024, and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. Your program is in cohort one and this is your preparation year. In preparation of the reassessment, I encourage you to visit NCRLAP website to review the cohort modules.
